Rehabilitation processes acceleration is the main objective of rehabilitation during postsurgical period. The standard of rehabilitation after surgical fixation of lumbar spine (LS) includes the following: remedial gymnastics (RG), massage and physiotherapy, which often cause timely pain and edema increase during postsurgical period of rehabilitation. In this connection people are afraid of vertical adjustment and physical activity. These symptoms are offered to neutralize with the help of preliminary cryo-massage [19]. Materials. 30 patients-sportsmen of neurosurgery department of Sverdlovsk Regional Clinical Hospital (16 men and 14 women at the age of 24±4,2) had remedial gymnastics (30 minutes) (the 1st group) after LS operative therapy from the first day after the operation, in the experimental group (the 2nd group) cryo-massage was used [19]. Research methods. Rehabilitation results were estimated according to physical and psychological state self-assessment with the help of “SF-36”test-questionnaire. Results. Rehabilitation effectiveness is proved by patients’ life quality improvement. During physical and psychological health self-assessment with the help of “SF-36” test respondents underlined both separate and overall components improvement. Total physical component improved from 79,38±15,73 till 87,98±17,06 (t=-1,96 p<0,002), total psychic component improved from 75,31±12,44 till 84,01±14,65 (t=-4,078 p<0,001) before and after the course of rehabilitation. Conclusion. The results showed that cryo-massage use after LS operative therapy provides early vertical adjustment and quicker rehabilitation of the injured tissues, 90% of patients started to train one month after the operation.

INTRODUCTION

The number of injured sportsmen increases [7,13,16], which is the reason for surgical therapy [2, 11]. One of the most defenceless parts of the body is lumbar spine (LS) [9]. Rehabilitation processes acceleration and invalidization prevention or decrease are the main objectives of rehabilitation during postsurgical period [17].

The effectiveness of cold influence is proved: local inflammation, tissue hypoxia decrease, muscle spasm removal, regional blood flow and microcirculation in tissues improvement [10,12,18]. It is known that during the process of cooling arterial and venous blood flow considerably decreases [14]. Cryotherapy is widely used for sports injuries rehabilitation and also for rehabilitation acceleration at trainings and during competitions [1, 6]. Regenerative potential of cryotherapy is based on anesthetic, antiedematous, anti-inflammatory, relaxing and also general effects, which lead to skin and tissues temperature decrease [5]. Early vertical adjustment after operative therapy considerably decreases rehabilitation period and improves life quality of patients [20]. The standard of rehabilitation [17] after surgical therapy of lumbar spine (LS) includes the following: remedial gymnastics (RG), massage and physiotherapy, which often cause timely pain increase and fear of vertical adjustment. Cryo-influence before remedial gymnastics can neutralize these symptoms, provide early vertical adjustment and, as a result, accelerate rehabilitation after LS operative therapy.

MATERIALS AND METHODS

Material and research organization. At neurosurgery department of Sverdlovsk Regional Clinical Hospital 30 patients-sportsmen were examined (16 men and 14 women at the age of 24±4,2), who go in for mountain skiing, artistic gymnastics, motor-and car racing, after operative dynamic fixation of LS. Rehabilitation program is

described in previous research work [19]. Cryo-influence [till -3° C ] was realized with the help of Med4-Ellit system, patented NASA technology [3], with low-profile bands, which provide 360-degree coverage, big contact area for cold therapy optimization of the main parts of the body and anatomically projected for better adjacency and comfort. All these things improve the results of influence and increase satisfaction with rehabilitation process. Med4-Ellit system creates its own, long-term cold in order to increase vasoconstriction, decrease blood flow, inflammation and edema, muscle cramps, local metabolic disorders and anesthetize safely, which prevents fear of vertical adjustment. Cryo-influence (10 minutes) in the experimental group was used directly after surgical therapy and then before remedial gymnastics.

Research methods: physical and psychological state self-assessment was held with the help of "SF-36"test-questionnaire [21]. The received results according to each group the respondents were handled with the help of traditional biostatistics methods [15]. The average values, average errors were calculated and the respondents were divided in percentage. As values distribution in each group was determined as normal, validity of differences according to average-group values was determined according to Student (t) criterion, and in features distribution according to Fisher's ratio test (F). 95% level of validity was accepted (t>1,96 u F>4,09).

THE RESULTS OF THE STUDY AND THEIR DISCUSSION

Research results. Rehabilitation effectiveness indicators are the following: no pain syndrome, the volume of movements increase, no motional and sensory disorder, working capacity rehabilitation.

Clinical studies estimation among the patients with LS injuries in dynamics, before and after 3 weeks of physical

rehabilitation course, is presented in table 1.

The offered program with cryo-massage use in complex rehabilitation helped to achieve early vertical adjustment and

patients' life quality improvement. During physical and psychological health self-assessment in SF-36 test there was both separate and total components improvement.

In case of normal distribution within this range there will be 68% of features values, 95% of values are within the limits of two sigma units and 99% are within the limits of three sigma units.

CONCLUSION

The results of rehabilitation among sportsmen after LS operative therapy using remedial gymnastics methods and cryo-massage showed, that early use of local cooling provides emotional tension decrease before remedial gymnastics, fear decrease, readiness for early vertical adjustment, quicker rehabilitation of injured tissues, sportsmen's working capacity rehabilitation and their life quality improvement.
