DOCTOR AND PATIENT

ABSTRACT
The article examines the relationship between a doctor and patients. The issues of professional competence of a doctor, social and psychological contacts between a doctor and a patient are raised. Basic concepts of dianthology, its principles and obligations of the doctor. The most common socio-psychological problems that negatively affect the process of communication between a doctor and a patient are listed. It also examines the meaning of the concepts of «Medical secrecy» and «Iatrogeny» for the doctor and patient. The main mistakes in the behavior of a typical patient at a doctor's appointment are indicated.

Mutual relationship: The relationship between doctor and patient is a central part of healthcare and medical practice. The relationship between doctor and patient is one of the foundations of modern medical care. The quality of the patient-doctor relationship is important to both parties. The values and views of the doctor and patient about illness, life and available time play a role in building this relationship. A strong doctor-patient relationship will lead to private, quality information about the patient's and family's illness. Improving the accuracy of the diagnosis and expanding the patient's knowledge of the disease all come with a good doctor-patient relationship. In cases where such a relationship is poor, the physician's ability to make a full assessment is compromised and the patient is more likely to distrust the diagnosis and proposed treatment, resulting in a lack of competence to actually follow medical advice, leading to poor health outcomes. In these circumstances, as well as in cases where there is a genuine disposition in the opinions of doctors, a second opinion from another doctor may be requested or the patient may turn to another doctor whom he trusts more.
Medical deontology plays an important role in the interaction of a doctor and a patient. Medical ethics or deontology is the discipline of the section of applied ethics, the rules and norms for the interaction of a doctor with colleagues and a patient. It is believed that the basic principles of medical ethics were formed by Hippocrates in his «Oath» In his «Oath» Hippocrates gave 9 aesthetic principles and doctor's recommendations. These principles and obligations not only relate to the attitude towards their patient, but also to the entire activity of the doctor: 1) The principle of respect for the teacher and the duty of front workers;

2) The principle of no harm;
3) The principle of assistance;
4) The principle of caring for the position of the patient and the importance of his interests;
5) The principle of respect for life and the negative attitude of euthanasia
6) The principle of respect for life and negative attitudes towards abortion;
7) Obligation to refuse intimate relations with the patient;
8) The principle of personal excellence;
9) The principle of confidentiality - medical secrecy;
What does medical confidentiality mean? Why should the doctor keep it confidential?
A medical secret is an instruction on the fact of seeking medical help, the patient's state of health, the diagnosis of his disease and other information obtained during his examination and treatment. At the same time, the doctor is obliged not to disseminate information about the patient's condition without his consent, even to his close people. If the doctor does not comply with the rules for keeping medical confidentiality, then criminal punishment will be applied to him. And at the same time, it can cause the occurrence of iatrogenic diseases.
What is Iatrogeny? Iatrogeny is a painful disorder of improper behavior of a medical worker - a careless statement by a doctor about the severity and prognosis of a disease, a story about a poor outcome of a similar disease in another patient, or disseminated information about a given patient by a doctor. In the pre-war edition of the Great Medical Encyclopedia, this is a term denoting the effect of a doctor on a sore spot of a therapeutic effect on a patient, an idea is created that aggravates his morbid state or forms a complex of new psychological diseases. Not a relationship, not trust on the part of the patient to the attending physician.
Conclusion: For my conclusions, I will give one example for the relationship between a doctor and a patient from clinical practice, which shows that treatment cannot be effective without a constant relationship between the doctor and the patient. A 36-year-old patient with type 1 diabetes mellitus consulted a doctor with complaints of an increased glycemic level for 2 months, an irregular heart rhythm and high blood pressure. The blood sugar level rises to 22.5 (normally 3.3-7.8 mmol / l), while we can say that the patient is on this awareness of the disease. The patient was prescribed treatment that lasted two years. From time to time, the patient developed hypoglycemic conditions of varying severity. Because of this, she independently canceled insulin for 7-8 months. As a result, her sugar level is repeatedly increased. The doctor persistently asked the patient to resume the administration of insulin, but the patient did not listen to the recommendations. As a result, the patient ended up in a state of ketoacidosis in a

hospital with a glycated hemoglobin level of 10. After hospitalization, the patient realized her mistake and began to trust the doctor.
Here we see that the doctor tried to introduce the patient to the stage of attempting to act, but apparently they did not have a trusting relationship, since it was not possible to convince the patient. So that such unfavorable outcomes do not arise during treatment, the doctor must be polite and careful when communicating, and between the doctor and the patient there must be a mutual attitude and trust in each other. The doctor must have a subtle psychological intuition, and a constant work of consciousness is necessary here. Success in treatment is possible with a combination of trusting human relationships and scientific achievements. And for this, a technically equipped doctor must not only treat, but also be able to talk with his patient.
