ULTRASOUND DIAGNOSTIC CRITERIA FOR ACUTE HEMATOGENOUS EPIPHYSAL OSTEOMYELITIS IN CHILDREN

Relevance. Early diagnosis and timely treatment of acute hematogenous epiphyseal osteomyelitis (AHEO) largely determine the outcome of the disease, especially in young children. Early rehabilitation of the primary focus and complex treatment leads to recovery in 95% of patients. However, with all the evidence of such an effect of early diagnosis of AHEO, the problem is still far from being solved. And among the reasons for maintaining this state of affairs, the variety of clinical manifestations and the lack of information content of diagnostic criteria used in practical healthcare are most often noted.

Purpose of the study. To study the role of ultrasound scanning in the early diagnosis of OHEO in children.

Materials and methods of research. The results of diagnosis and treatment of 50 patients aged from birth to 1 year who were treated in the department of neonatal surgery of the 2nd clinic of Samarkand State Medical University for the period 20212022 were studied.

Ultrasound examination was carried out according to the standard technique using an ultrasound scanner equipped with linear sensor.Scanning was performed in transverse and longitudinal sections relative to the axis of the limb. Ultrasound diagnosis of epiphyseal osteomyelitis was carried out by evaluating the following signs: edema and thickening of soft tissues in the area of the affected limb segment, thickening of the articular capsule, violation of the integrity of the articular capsule, increased echogenicity of intraarticular contents, visualization of the focus of hyperechogenicity against the background of reduced or normal echostructure of the epiphysis, decreased echogenicity of the epiphyseal cartilage , changing the shape of

the epiphyseal cartilage, determining the periosteal reaction, unevenness and usuration of the metaphyseal zone of the bone

Results of the study and their discussion. Analysis of archival and own material showed that in 92% (46) cases, periosteal thickening was detected by ultrasound scanning, in 82% (41) subperiosteal abscesses were recognized. And what is especially important, in 90% (48) of children of the first year of life, ultrasound scanning made it possible to visualize areas of bone destruction in the metaepiphyseal zone. 96% of patients had edema and infiltrative changes in soft tissues.

Conclusions. Thus, ultrasound scanning can be used as a non-invasive method in the complex of diagnostic measures when examining young children with suspected OHEO. Ultrasound also allows early detection of edema and infiltrative changes in the muscles and soft tissues, as well as accumulation of fluid in the joint. The method is safe due to the absence of radiation exposure, highly informative, and its frequent use is possible.
