Во второй части статьи, открывающей серию публикаций о способах суицидов, с привлечением научных, ис-торико-художественных данных, сообщений СМИ и врачебного опыта показаны типология и условия суицидальных прыжков с высоты, клинико-социальные характеристики суицидентов, факторы риска, в свете разработки общих и целевых профилактических мероприятий как элемента типовой многоуровневой антикризисной программы.

Выбор смерти посредством прыжка (падения) с высоты (например, со скалы, моста, из окна) -среди малораспространённых методов суицида (обычно 2-5% зарегистрированных самоубийств в экономически развитых странах) с значимыми местными отличиями [1].

Дифференциальный диагноз затруднён.

The choice of death by jumping (falling) from a height (for example, from a cliff, a bridge, from a window) is among the less common methods of suicide (usually 2-5% of registered suicides in economically developed countries) with significant local differences [1].

Differential diagnosis is complicated.

An example of this is the urban legend of Don Harper Mills, The Strange Death of Ronald Opus. Ronald jumped in desperation from the roof of a 10-story building, leaving a farewell letter. But things are not so obvious (from the Web).

Показателем занижения отчётности самоубийств как таковых служит доля смертей с неопределёнными намерениями. В частности, о жестоких методах, как повешение или падения с высоты сообщают чаще, чем об отравлениях.

Интервью с выжившими «прыгунами» могут дать полезную (в зависимости от типа постсуицида) информацию о причинах поступка и помочь разработать превентивное вмешательства.

Interviews with jumper survivors can provide useful (depending on the type of postsuicide) information about the causes of the behavior and help develop preventive interventions.

На выбор метода самоубийства влияет ряд факторов. Социальные модели самоубийства можно понять из таких основных понятий, как социальная приемлемость метода (культура и традиция) и его доступность (возможность).

Мужчины чаще избирают жестокие методы, требующие решимости [3, 4], но гендерное соотношение «прыгунов» менее отчётливо, чем при других способах суицида.

Не выявлены (пока) несомненные социально-демографические и клинические особенности выбирающих самоубийства с помощью прыжков, но метод чаще использован больничными пациентами и заключенными при отсутствии доступа к другим средствам суицида. Жертвы суицидальных прыжков большей частью мужчины старше 60 лет, трезвые [5]. Две трети трагедий в доме жертвы высотой не менее семи этажей. Жертвы часто страдали депрессией и обычно не оставляли предсмертной записки.

Большинство суицидов медийных персон (Википедия) из окон / балконов своих домов или квартир (54%), гостиниц (10%). Около % (24%) - с мостов, из окон торговых центров, библиотек, больниц (8%) и тюрем (4%) [6].

Прыжок с высоты отнесён к «жестоким» методом самоубийства. У выбравших такую смерть априори выражена мотивация саморазрушения. Их следует отличать («проклятый вопрос суици-дологии») от избравших, например, самоотравление с меньшим риском гибели, оставшихся в живых или склонных к несуицидальным самоповреждениям.

Не легче поиск различий с выбравшими огне-стрел (на переднем плане - доступность средства суицида) и самоповешение (летальность зафиксированных случаев до 90%), тогда как при самоотравлении смертность «заметно ниже» [7, 8].

Относительно малые выборки «суицидентов-прыгунов» неоднородны в клиническом аспекте.

Детерминанты суицидального поведения (СП).

Психически больные выбирают жестокие методы [9].

A number of factors influence the choice of suicide method. Social models of suicide can be understood in terms of such basic concepts as the social acceptability of the method (culture and tradition) and its availability (opportunity).

Men are more likely to choose violent methods that require determination [3, 4], but the gender ratio of jumpers is less distinct than with other methods of suicide.

No clear socio-demographic and clinical features have been identified (yet) for those who choose suicide by jumping, but the method is more commonly used by hospital patients and prisoners in the absence of access to other means of suicide. Victims of suicidal jumps are mostly men over 60 years of age, sober [5]. Two-thirds of the tragedies in the victim's house are at least seven stories high. The victims often suffered from depression and usually did not leave a suicide note.

Most suicide jumps committed by media people (Wikipedia) from the windows / balconies of their houses or apartments (54%), hotels (10%). About % (24%) jumps were made from bridges, from the windows of shopping centers, libraries, hospitals (8%) and prisons (4%) [6].

Jumping from a height is classified as a "cruel" method of suicide. Those who chose such way to die a priori expressed the motivation of self-destruction. They should be distinguished ("the damn question of suicidology") from those who chose, for example, self-poisoning with a lower risk of death, survivors or prone to non-suicidal self-harm.

It is not easier to find differences with those who chose firearms (in the foreground -the availability of a means of suicide) and self-hanging (mortality of recorded cases is up to 90%), while with self-poisoning, mortality is "noticeably lower" [7, 8].

Relatively small samples of "suicidal jumpers" are heterogeneous in the clinical aspect.

Depressive symptoms (such as despair) are expected to be present in most jumpers (but

Импульсивность облегчает переход от суицидального намерения, с которым можно сосуществовать годами, до рокового поступка и, возможно, определяет суицидальный риск в большей мере, чем планирование [12].

Наиболее распространённым мотивом суицида служит импульс - побег из невыносимых житейской ситуации или душевных мук [13]. Импульсивность увеличивает смертельность методов суицида при их доступности и социально-культуральной приемлемости [7].

Необходим мониторинг способов «типовых и специфичных паттернов» самоубийств и суицидальных попыток (опыт суицидологического регистра Тюменской области), чтобы в целевых стратегиях по ограничению доступа к средствам самоубийств и информационной работе в русле первичной профилактики учитывались переменные характеристики проблемы при создании адресных программ по предотвращению самоубийств на основе различий внутри страны в уровнях и способах суицида, социо - демографических и клинических характеристик жертв. Международные или межкультурные сравнения методов самоубийства помогут лучше понять взаимосвязь между этими факторами и обеспечивают основу превентивных стратегий.

Большая часть ограничительных мер превенции суицидальных прыжков с высоты должны выполняться лицами вне сферы охраны психического здоровья, часто не предполагающими участие в этом общем деле. Однако ограничение бесконтрольного доступа к ядохимикатам, лекарственным препаратам (особо - психотропным) более осуществимо, чем к «мылу и верёвке» или подоконнику.

Вложение в профилактику (в частности, страховочные сети) гуманно и экономически выгодно. Выжившие после «неудачных» суицидальных прыжков или прерванного суицида в большинстве своем не совершают впредь попыток, что указывает полезность своевременных профилактических мер.

The results of the review are applicable to suicide prevention strategies in general and jumping in particular. Limiting access to the means of suicide, as shown in population studies, is an effective approach to reduce the risk of suicide. Prevention programs should focus on preventing the spread of new suicide methods (and new places to die) by implementing effective media guidelines. There is a need to better understand national, regional and local patterns of suicide and understand the underlying mechanisms.

It is necessary to monitor the methods of "typical and specific patterns" of suicides and suicide attempts (for example, experience of the suicide registry of the Tyumen region), so that targeted strategies to limit access to suicide means and information work in line with primary prevention take into account the variable characteristics of the problem when creating targeted suicide prevention programs based on within-country differences in levels and modes of suicide, socio-demographic and clinical characteristics of victims. International or cross-cultural comparisons of suicide methods will help to better understand the relationship between these factors and provide the basis for preventive strategies.

Much of the restrictive action to prevent suicidal jumping must be carried out by individuals outside the mental health field, often with no involvement in the common cause. However, limiting uncontrolled access to pesticides, drugs (especially psychotropic ones) is more feasible than to "soap and rope" or a window sill.

Investing in prevention (in particular, safety nets) is humane and economically beneficial. Survivors of "failed" suicide jumps or aborted suicide are mostly not re-attempted, indicating the usefulness of timely preventive measures.
