Во второй части обзора рассматривается проблема сурфактант-терапии острого респираторного дистресс-синдрома (ОРДС) у взрослых и детей младшего и старшего возраста. Излагаются сведения о результатах сурфактант-терапии и профилактики ОРДС у больных с тяжелой комбинированной травмой, ингаляционными поражениями, осложнениями при тяжелых расширенных операциях на грудной клетке, при тяжелых пневмониях, в том числе при двусторонней пневмонии на фоне гриппа A/H1N1. Представлены данные о применении сурфактанта в акушерской клинике и профилактике первичной дисфункции легочного трансплантата при трансплантации легких. Обсуждаются результаты многолетнего использования сурфактант-терапии в России, свидетельствующие о возможности существенного (до 20%) снижения уровня смертности при ОРДС. Кроме того, рассмотриваются примеры сурфактант-терапии других заболеваний легких, таких как стойкие ателектазы, ХОБЛ и бронхиальная астма, а также туберкулез легких.

Pulmonary Surfactants for Acute and Chronic Lung Diseases (Part II)

Part 2 of the review considers the problem of surfactant therapy for acute respiratory distress syndrome (ARDS) in adults and young and old children. It gives information on the results of surfactant therapy and prevention of ARDS in patients with severe concurrent trauma, inhalation injuries, complications due to complex expanded chest surgery, or severe pneumonias, including bilateral pneumonia in the presence of A/H1N1 influenza. There are data on the use of a surfactant in obstetric care and prevention of primary graft dysfunction during lung transplantation. The results of long-term use of surfactant therapy in Russia, suggesting that death rates from ARDS may be substantially reduced (to 20%) are discussed. Examples of surfactant therapy for other noncritical lung diseases, such as permanent athelectasis, chronic obstructive pulmonary diseases, and asthma, as well tuberculosis, are also considered.

Part 2 of the review considers the problem of surfactant therapy for acute respiratory distress syndrome (ARDS) in adults and young and old children. It gives information on the results of surfactant therapy and prevention of ARDS in patients with severe concurrent trauma, inhalation injuries, complications due to complex expanded chest surgery, or severe pneumonias, including bilateral pneumonia in the presence of A/H1N1 influenza. ОРДС давно известен врачам под такими названиями, как некардиогенный отек легких, шоковое легкое, мокрое легкое и другие. Современное определение ОРДС было введено D.G. Ashbaugh (1967), а критерии диагностики выработаны Американо-Европейской согласительной конференцией (АЕСК). В диагностике синдрома острого повреждения легких (СОПЛ) и ОРДС, согласно рекомендациям АЕСК, опираются на 4 критерия: острое начало основного заболевания (сепсис, тяжелая пневмония, множественная травма, ингаляционная травма и др.), тяжелая гипоксемия, ре-

Introduction

For a long time ARDS has been known under the following names: noncardiogenic pulmonary edema, shock lungs, wet lungs and others. The modern definition of ARDS was introduced by D. G. Ashbaugh (1967), while the diagnosis criteria were developed by the American-European Consensus Conference (AECC). According to AECC recommendations, diagnosis ARDS is established on the basis of four criteria: acute onset of the main disease (sepsis, severe pneumonia, multiple trauma, inhalation injury and others); severe hypoxemia, refractory to oxygen therapy; bilateral infiltrates on chest radiography;

В зависимости от тяжести гипоксемии, в т.ч. соотношения PaO2/FiO2, ставили диагноз СОПЛ (PaO2/FiO2 меньше 300 мм но больше 200 мм или ОРДС (PaO2/FiO2 меньше 200 мм норма 360—400 мм Двадцатилетний опыт применения этих критериев показал их несовершенство. Так, такой критерий, как двусторонние изменения на рентгенограмме легких, отсутствовал у 30% больных в течение первых суток развития ОРДС. Мороз В. В., Власенко А. В., Голубев А. М. и др. отказываются от термина СОПЛ и полагают, что ранняя диагностика ОРДС должна учитывать степень ин-терстициального отека, т.е. содержание внесосудистой жидкости в легких [2]. В 2011 г. на Берлинском форуме согласительная комиссия экспертов также отказалась от термина синдрома острого повреждения легких и предложила использовать новые критерии (Берлинские критерии) для установления диагноза ОРДС [3]. Эти критерии нацелены на надежность, достоверность и объективную оценку функционального состояния больного. На основании анализа лечения 4457 больных ОРДС было предложено оценивать синдром по степени тяжести в соответствии со значением соотношения PaO2/FiO2: легкая степень ( PaO2/FiO2 < 300 mm но > 200 mm средняя степень (PaO2/FiO2 < 200 mm но > 100 mm и тяжелая (PaO2/FiO2 < 100 mm Оказалось, что рентгенологические данные, уменьшение комплайенса, величина положительного давления в конце выдоха (ПДКВ) и дыхательный минутный объем не влияют на прогноз исхода ОРДС, поэтому эти параметры не были включены в конечный вариант Берлинских критериев. ОРДС легкой, средней и тяжелой степеней тяжести характеризовались следующими показателями летальности: 27, 32 и 45%, соответственно (р<0,001). Среднее время ИВЛ для выживших больных составило 5, 7 и 9 дней, соответственно (р<0,001). Полагают, что по сравнению с критериями АЕСК Берлинские критерии дают более корректный прогноз.

Патофизиология ОРДС достаточно сложна, в ее основе лежат молекулярные механизмы системной воспалительной реакции, развивающейся в процессе течения основного заболевания, послужившего причиной развития ОРДС. В результате вазоконстрикции легочных капилляров выброса эндотелинов и других агрессивных цитокинов, «залипания» лейкоцитов на стенках капилляров легких и повышения проницаемости альвеолокапиллярной мембраны, выхода лейкоцитов и белков плазмы крови в альвеолярное пространство происходит ингибирование легочного сурфактанта и повреждение А-П. Это приводит к нарушению синтеза сурфактанта de novo и нарушению его реутилизации. Развиваются микроателектазы и сливные ателектазы, шунтирование кровотока и развитие тяжелой, рефрактерной к кислородотерапии, гипоксемии.

pulmonary artery wedge pressure <18 mm Hg or absence of clinical evidence of left ventricular failure [1]. The diagnosis of acute lung injury (ALI) or acute respiratory distress syndrome (ARDS) depends on hypoxemia severity, i.e. PaO2/FiO2 ratio: ratio < 300 corresponds to ALI), ratio >200 corresponds to ARDS. In norm, PaO2/FiO2 ratio is 360—400 mm Hg. The use of these criteria during 20 years demonstrated their imperfection. For example, such criterion as bilateral infiltrates on chest radiography was absent in 30% of patients during the first 24 hours of ARDS development. A. V. Vlasenko, A. M. Golubev and V. V. Moroz suggested to stop the use of the term ALI. They believe that early ARDS diagnostics should be based on the degree of interstitial oedema, i.e. on the quantity of extravascular lung fluid [2]. In 2011, using a consensus process, a panel of experts declined the term ALI and developed the Berlin Definition, new criteria for ARDS diagnosis [3]. They were based on feasibility, reliability, validity, and objective evaluation of the performance. On the base of the results of patient-level meta-analysis of 4188 patients with ARDS, it was proposed to estimate the severity of the syndrome according to PaO2/FiO2 ratio: mild (200 mm Hg; PaO2/FiO2 < 300 mm Hg), moderate (100 mm Hg; PaO2/FiO2 < 200 mm Hg), and severe (PaO2/FiO2 < 100 mm Hg). It turned out that the four ancillary variables (radiographic severity, low respiratory system compliance, the meaning of positive end-expiratory pressure, and corrected expired volume per minute) did not contribute to the predictive validity of severe ARDS for mortality and were removed from the definition. Stages of mild, moderate, and severe ARDS were associated with increased mortality: 27%, 32%, and 45%, respectively, (P < 0.001). Compared with the AECC definition, the final Berlin Definition had better predictive validity for mortality.

ARDS pathologic physiology is quite complicated. Its basis is molecular mechanisms of systemic inflammatory reaction progressing as a result of the main disease which has caused ARDS development. Due to lung capillary vasoconstriction and discharge of endothelines and other aggressive cytokines; leucocyte sticking on lung capillary walls and the increasing of permeability of alveolar-capillary membrane; leucocyte and plasma proteins leakage into alveolar space, lung surfactant inhibition and alveolocytes type 2 damage occur. All these features resulted in de novo surfactant synthesis and its reutilization failure. Finally, micro atelectasis and confluent atelectasis, blood flow by-pass and severe hypoxemia, refractory to oxygen therapy were developed.

In spite of the improvement of respiratory support methods, following the principle of safety in CMV, open lung concept [4], the use of patient's face-down position and so on, the mortality from ARDS is still very high. According to G.D. Rubenfeld [5], every year 190, 000 patients with ARDS are registered in the USA, and the mortality is 39.1%. Such factors as age, sex, race, CMV strategy, and etiology of lung injury influence ARDS mortality rate [6]. There is no total ARDS statistics in Russia,

Несмотря на совершенствование способов респираторной поддержки, следования принципам «безопасной» ИВЛ, концепции «открытого легкого» [4], использование положения больного на животе и т.д., летальность при ОРДС остается очень высокой. По данным Rubenfeld G.D., в США ежегодно регистрируется 190 000 больных ОРДС, при этом летальность составляет 39,1% [5]. Отмечается также, что такие факторы, как возраст, пол, расовая принадлежность, стратегия проведения ИВЛ, этиология повреждения легкого влияют на уровень летальности при ОРДС [6]. В России нет общей статистики по частоте встречаемости ОРДС, однако экстраполяция частоты ОРДС в США на население России дает показатель около 80 000 случаев в год.

1. Терапевтическая эффективность препаратов легочного сурфактанта при лечении острого респираторного дистресс-синдрома у взрослых

В настоящее время сурфактант-терапия ОРДС крайне редко используется в мировой практике. Это связано с тем, что ни один зарубежный препарат не доказал своей эффективности в МРКИ, поэтому препараты сурфактанта не разрешены для применения у взрослых за рубежом. Российский препарат сурфактант-БЛ на основании результатов многоцентровых неконтролируемых испытаний (1998—2002 гг.), показавших существенное (в 2—4 раза) снижение смертности у больных, ответивших на введение препарата положительными сдвигами в оксигенации, разрешен для лечения ОРДС в 2003 г. и успешно используется во многих ОРИТ.

Как уже упоминалось выше, результаты II фазы МРКИ и пилотных исследований эффективности природных препаратов сурфактанта при лечении ОРДС показали существенное улучшение оксигенации, уменьшение времени ИВЛ и снижение летальности [6, 7]. Однако при проведении третьей фазы МРКИ как природных, так и синтетических препаратов сурфак-танта зарегистрирована положительная динамика параметров оксигенации и продолжительности ИВЛ (промежуточные точки исследования) без улучшения конечного результата — уменьшения летальности [8]. Тем не менее при МРКИ американского препарата Infasurf получено достоверное снижение летальности при ОРДС у детей (от 3 до 21 года) при прямом повреждении легких [9].

Представляется целесообразным подробно остановиться на анализе многолетних МРКИ препарата Venticute как примере исследования препаратов сур-фактанта за рубежом. В 2000 г. в результате изучения эффективности препарата в гетерогенной группе больных ОРДС при прямом и непрямом повреждении легких были получены отрицательные результаты в отношении снижения летальности больных. Однако дополнительный анализ результатов испытаний (post-hoc analysis) показал достоверное снижение летальнос-

1. Therapeutic efficacy of lung surfactant preparations for the treatment of acute respiratory distress syndrome in adults

Nowadays, ARDS surfactant therapy is very rare in world practice. Main reasons include absence of preparations efficiency of which has been proved in multicentre randomized controlled trial (MRCT. Russian surfactant preparation, Surfactant-BL, has obtained permission for ARDS treatment in 2003 in the ground of multicentre uncontrolled clinical trials (1998—2002), which demonstrated essential (2—4 times) reduction in mortality in patients who responded to the preparation administration with positive shifts in oxygenation. Since then Surfactant-BL has been successfully used in many intensive care units (ICU).

As noticed before the results of the second phase of MRCT and pilot trials of the efficiency of natural surfactant preparations for ARDS treatment have shown essential improvement in oxygenation, the reduction of CMV period and mortality decrease [6, 7]. However, the third phase of MRCT of both natural and synthetic preparations demonstrated positive dynamics of oxygenation figures and CMV settings (intermediate investigation point) without any improvement in outcome — mortality reduction [8]. At the same time, MRCT of American preparation Infasurf showed significant decrease in mortality in ARDS in children (from 3 to 21 years old) with direct lung injury [9].

It seems to be useful to focus on the analysis of years-long MRCT of another surfactant preparation, Venticute.. In 2000, the preparation study in heterogeneous group of ARDS patients with direct and indirect lung injury provided negative results in mortality reduction. Additional analysis of the study (post-hoc analy-sis),however, demonstrated significant mortality decrease in patients with direct lung injury (pneumonia, aspiration of gastric content) [8, 10]. The authors suggested that negative results caused by many reasons. They included the failure of the idea of surfactant therapy, poor surfactant formulation, late surfactant administration, too short period of surfactant therapy, wrong way of administration, inadequate dose, wrong patient selection, researcher' preconception, preparation toxicity, incorrect estimation of outcome and inadequate intensive care. According to the authors, the most important factors were late start of preparation administration, insufficient course of surfactant therapy and heterogeneity of patients. According to MRCT Protocol of 2000, the beginning of surfactant administration was 48—72 hours after CMV start. Treatment period was 24 hours (50 mg/kg, every 6 hours). ARDS resulted from sepsis, multiple traumas, massive blood loss, hemotransfusion, pneumonia, aspiration of gastric content and others.

Среди них — несостоятельность идеи сурфактант-терапии, плохое качество препарата сурфактанта, позднее начало введения, недостаточная длительность курса сурфактант-те-рапии, неправильный способ его введения, недостаточная доза, неправильный отбор больных, предвзятость исследователей, токсичность препарата, неправильная оценка исходов и «недостаточные» реанимационные мероприятия. Они посчитали, что важнейшими из перечисленных факторов являются позднее начало введения препарата, недостаточная продолжительность курса терапии и гетерогенность группы больных. Согласно протоколу МРКИ 2000 г., время начала введения препарата Уепйси1е составляло 48—72 часа после начала ИВЛ. Продолжительность курса лечения составляла одни сутки (50 мг/кг каждые 6 часов). Объектом исследования являлись больные ОРДС на фоне сепсиса, множественной травмы, массивной кровопо-тери и гемотрансфузии, пневмонии, аспирации желудочного содержимого и др.

Возможные причины противоречивости результатов клинических испытаний препаратов сурфактанта при лечении ОРДС могут быть результатом несовершенства Протоколов, по которым проводились эти МРКИ. По мнению ряда авторов, существенные ошибки были допущены при определении времени начала сурфактант-терапии, терапевтических доз и длительности применения сурфактанта [10—13]. Проф. И. А. Козлов предположил, что позднее начало сурфактант-терапии, согласно этим Протоколам, обусловлено тем, что препараты сурфактанта не относили к средствам лечения первого ряда и полагали, что они должны использоваться только тогда, когда другие респираторные и нереспираторные методы лечения оказываются неэффективными (персональное сообщение, апрель 2013). Нами ранее было показано, что эффективность препаратов сурфактанта выявляется при первом введении в течение первых суток развития дыхательной недостаточности при ОРДС, тогда как начало сурфак-тант-терапии в течение вторых суток оказывается неэффективным [11, 13].

1.1. Терапевтические дозы. Как было указано выше, препараты сурфактанта различаются по терапевтической эффективности при лечении ОРДС, поэтому выбор дозы препарата чрезвычайно важен. В большинстве исследований для лечения ОРДС использовали очень высокие дозы препаратов, вплоть до 400 и 800 мг/кг массы тела [7, 8, 10]. Как правило, расчет дозы производится на основе представлений о высокой концентрации белков плазмы крови (ингибиторов легочного сурфактанта) в альвеолярном пространстве. Эти представления, с нашей точки зрения, являются ошибочными, так как содержание белков плазмы крови измеряли в суммарном объеме жидкости (200 мл) БАЛ больных ОРДС, полученной при объединении 10 по-

from imperfection of Protocols used in MRCT. Some authors believe that the time of the start of surfactant therapy, therapeutic doses and treatment duration were established with errors [10—13]. I. A. Kozlov supposed that late beginning of surfactant therapy in the Protocols of MRCT was due to the fact that surfactant therapy was not considered as a first-line treatment. The authors of the Protocols believed that surfactant preparations should be used only when other respiratory and non-respiratory methods of the treatment do not work (personal communication, April, 2013). We have shown earlier that surfactant efficacy could manifest itself when surfactant was administered for the first time on day 1 of respiratory insufficiency development in ARDS, whereas start of the therapy on day 2 was not effective [11, 13].

1.1. Therapeutic doses. Surfactant preparations differ from each other in therapeutic efficacy for ARDS treatment thus the dose choice is very important. The majority of researchers used very high doses of the drug for ARDS treatment up to 400 or 800 mg/kg of body mass [7, 8, 10]. Usually, dose estimation is based on the information about the high plasma protein concentration (inhibitors of lung surfactant) in alveolar space. We believe that these ideas are were correct because plasma protein content was measured in total BAL fluid liquid of ARDS patients (200 ml) and was obtained by putting together 10 sequent portions of lavage fluid [14, 15]. We supposed that multiple sequent lavages contribute to the plasma protein leakage from blood flow to alveolar space through damaged alveolocap-illary membrane. Hypothesis test demonstrated that protein content in sequent lavages was different: a portion with high protein content interchanges the portion with low protein content. Plasma protein leakage into alveolar space takes place because of the artificially created concentration gradient [16, 17]. Another argument for lower surfactant doses is based on the data on the surfactant content in mammalian lungs. The content of surfactant in healthy adult is 3—15 mg/kg of body mass [16]. Preclinical and clinical trials show that efficient therapeutic dose of Surfactant-BL is 6 mg/kg of body mass per administration. Taking into account the fact that Surfactant-BL biological half-life is 10—12 hours [18], it has been recommended to administer preparation twice every 24 hours. Clinical trial results proved the necessity to administer the preparation every 12 hours till the achievement of PaO2/FiO2 ratio > 300 mm Hg. Depending on the severity of patient's condition, Surfactant-BL is administered during 1—2 days. However, in ARDS induced by A/H1N1 influenza, Surfactant-BL was being administering during 4—5 days. The preparation was also efficient when surfactant therapy started on the 3d—5th day of respiratory insufficiency development [19—21].

In case of Venticute, the preparation was administered at a dose of 50 mg/kg 4 times a day, and then the course of surfactant therapy was stopped [8, 10]. We think that the period of surfactant therapy was not long enough.

1.2. Mode of administration. According to experimental data, only 4.5—10% of preparation reaches alveolar

Проверка этой гипотезы показала, что содержание белка последовательно меняется: порции с большим количеством белка чередуются с порциями с минимальным содержанием. Происходит транслокация белков плазмы крови в альвеолярное пространство по искусственно создаваемому градиенту концентрации [16, 17]. В качестве дополнительного аргумента в пользу применения меньших доз препаратов сурфак-танта свидетельствуют данные о его содержании в легких млекопитающих. У здорового взрослого человека содержание сурфактанта составляет 3—15 мг/кг массы тела [16]. В процессе доклинических и клинических испытаний нами было показано, что эффективная терапевтическая доза сурфактанта-БЛ составляет 6 мг/кг на введение. Принимая во внимание то обстоятельство, что время полувыведения сурфактанта-БЛ составляет 10—12 часов [18], было рекомендовано вводить препарат 2 раза в сутки. Результаты клинических испытаний подтвердили необходимость введения сурфактанта-БЛ каждые 12 часов до достижения PaO2/FiO2 > 300 mmHg. В зависимости от тяжести больного сурфак-тант-БЛ вводят в течение 1—2 суток, однако при ОРДС на фоне гриппа A/H1N1 нередко сурфактант-терапию проводили в течение 4—5 дней, и даже при введении препарата на 3 — 5 день развития дыхательной недостаточности он был эффективен [19—21].

Как было отмечено выше, при исследовании препарата Venticute его вводили по 50 мг/кг 4 раза в сутки и на этом заканчивали курс [8, 10]. Т. е. в данном случае курс сурфактант-терапии был недостаточно продолжительным.

1.2. Способы введения препаратов. Согласно экспериментальным данным, при ингаляционном способе введения только около 4,5—10% препарата попадает в альвеолярное пространство. Показано, что ингаляционный способ введения препаратов сурфактанта менее эффективен в сравнении с микроструйным или болюсным как у новорожденных [22], так и у взрослых [23]. В эксперименте при лечении ОРДС показано, что введение препарата в большом объеме эмульсии более эффективно, так как приводит к более равномерному распределению препарата в легких. Так, препарат Surfaxine рекомендуется использовать в виде лаважа большим объемом эмульсии (400—600 мл) [24]. Мы полагаем, что введение такого большого объема жидкости больному с тяжелой гипоксемией совершенно не оправдано и чрезвычайно опасно. Само введение такого объема жидкости приводит к длительному снижению оксигенации крови и требует немедленного «ужесточения» параметров ИВЛ. При МРКИ препарата Surfaxine у больных ОРДС выявлено достоверное увеличение частоты развития эпизодов гипоксии, обструкции эндотрахеальной трубки, полиорганной недо-

Clinical practice showed that inhalation of surfactant preparation is less efficient than micro fluid or bolus administration in newborns [22] and adults [23]. Experimental data showed that preparation administration in bigger volume is more efficient because it leads to more homogenous preparation distribution in lungs. For example, preparation Surfaxin is recommended for usage as lavage of big emulsion volume (400—600 ml) [24]. We think that the administration of such a big volume of liquid to the patient in critical condition with severe hypoxemia has not been justified and remains very dangerous. The administration of such fluid volume itself causes long-term reduction of blood oxygenation and requires immediate strengthening of CMV settings. Surfaxin MRCT in ARDS patients showed significant increase of frequency of saturation reduction, endo-tracheal tube obstruction, multiple organ failure (MOF), sepsis, renal insufficiency, pneumothorax and thromboem-boli of pulmonary artery. In 2012 Ari Diozon reported on significant increase in mortality rate observed in Surfaxin group compared to placebo group (http://www.bad-drug.net/bad-drug/surfaxin).

Currently the most efficient mode of administration is believed to be endobronchial bolus administration to each lung segment in combination with lung recruitment maneuver, which provides the preparation delivery to weakly aerated parts of lung parenchyma [13, 25, 26].

2. The use of surfactant preparations for ARDS treatment in direct and indirect lung injury

Considerable experience of Surfactant-BL use for the ARDS treatment has been gained in Russia. Multicenter uncontrolled clinical trials of Surfactant-BL in heterogeneous groups of patients were carried out from 1998 to 2002 in six leading hospitals of Moscow and Saint-Petersburg. Based on results of the trials, in 2003 the preparation was permitted for ARDS treatment in adults. Thus, nowadays there are opportunities to evaluate the results of more than ten-year usage of Surfactant-BL in medical practice. When using the surfactant preparation, technologies of treatment of different pathological states were improving. These states include sepsis, multiple traumas complications after cardiosurgeries, inhalation traumas, obstetrics and gynecologic pathology, viral pneumonia and others. The ways of preparation administration, doses, length of treatment, combinations with other pharmaceutical and respiratory technologies have been developed.

Below the experience of regular employment of Surfactant-BL for ARDS treatment in Russian hospitals is described.

2.1. Surfactant therapy of ARDS due to sepsis, multiple trauma and complications after surgeries. A. B. Vlasenko et al. studied surfactant therapy in 81 patients with ARDS of different genesis [2, 25—27]. The patients data included: Lung Injury Score (LIS) was 2.5; Acute Physiology and Chronic Health Evaluation (APACHE) II score was 17.6±2.4;

В настоящее время наиболее эффективным считают посегментное эндобронхиальное болюсное введение препаратов сурфактанта в комбинации с «маневром открытия легких», что предполагает доставку препарата в невентилируемые участки легочной ткани [13, 25, 26].

2. Применение препаратов сурфактанта при лечении ОРДС при прямом и непрямом повреждении легких

В России накоплен большой опыт применения препарата сурфактант-БЛ для лечения ОРДС. Многоцентровые неконтролируемые клинические испытания препарата в гетерогенных группах больных проходили с 1998 г. по 2002 г. в шести ведущих клиниках Москвы и Санкт-Петербурга. По результатам этих испытаний препарат был разрешен для лечения ОРДС у взрослых в 2003 г. Таким образом, в настоящее время имеется возможность рассмотреть результаты более чем десятилетнего применения препарата в клинике. По мере использования сурфактанта-Б Л совершенствовались технологии применения препарата при различных патологических состояниях: сепсисе, множественной травме, осложнениях после кардиохирургических операций, игнгаляционной травме, акушерской и гинекологической патологии, вирусных пневмониях и др. Отрабатывались способы введения препарата, дозы, продолжительность курса лечения, комбинации с другими фармакологическими и респираторными технологиями.

Нам кажется уместным остановиться на опыте ряда клиник в России, регулярно использующих сур-фактант-терапию в лечении больных ОРДС.

2.1. Сурфактант-терапия ОРДС на фоне сепсиса, множественной травмы и осложнений после расширенных операций. А. В. Власенко и соавт. изучали возможности сурфактант-терапии ОРДС при лечении 81 пациента с ОРДС различного генеза [2, 25—27]. Параметры тяжести состояния больных были следующими: LIS = 2,5; APACHE II 17,6±2,4; SOFA 10,2±2,1; MODS 9,5±1,2 баллов. Авторы оценивали эффективность эндобронхиального введения сурфактанта БЛ в разные сроки развития заболевания (от 6 до 78 часов с момента снижения PaO2/FiO2 < 200 мм рт. ст.) в сочетании с приемом «открытия легких», а также раздельно. Препарат вводили в дозе 6 мг/кг каждые 12 часов до стойкого улучшения газообмена в легких и достижения PaO2/FiO2 > 300 мм рт. ст. Раннее начало сурфактант-терапии (до 24 часов от манифестации ОРДС) приводит к достоверному улучшению показателей газообмена и биомеханики легких, уменьшению частоты

Sequential Organ Failure Assessment (SOFA) score was 10.2±2.1; Multiple Organ Dysfunction Score (MODS) was 9.5±1.2. The efficacy of endobronchial Surfactant-BL administration was estimated at different points of disease development (from 6 to 78 hours since PaO2/FiO2 < 200 mm Hg). Surfactant treatment was carried out in combination with lung recruitment maneuver and without it. Surfactant-BL was administered at a dose of 6 mg/kg, every 12 hours till stable gas exchange improvement and the achievement of PaO2/FiO2 > 300 mm Hg. Early start of surfactant therapy (not longer than 24 hours from ARDS manifestation) resulted in significant improvement in gas exchange parameters and lung biochemistry, the reduction of frequency of hospital-acquired pneumonia development, the decrease of CMV treatment period and time in ICU. The results of Surfactant-BL therapy depended on the degree of lung damage: is the latter was much worse in cases of massive lung collapse (initial PaO2/FiO2 < 150 mm Hg) [26, 27]. Combined usage of Surfactant-BL and lung recruitment maneuver was more efficient than separate application of these methods of treatment. The combination of different technologies for ARDS treatment including surfactant therapy led to significant reduction of mortality rate from ARDS to 20% [2, 27].

2.2. Surfactant therapy of ARDS induced by the operations on heart and major vessels. A. E. Bautin and others [28] studied the efficiency of surfactant therapy for the treatment of ARDS developed after operations on heart and aorta. The authors demonstrated that Surfactant-BL provided more opportunity to remove more paients from mechanical ventilation (72.2% of patients receiving surfactant were removed from ventilation versus only 47.4% of patients in the control group). The improvement in gas exchange was stable and finally led to the reduction in mortality rate to 33.3% compared to 52.6% in the control group. The combination of lung recruitment maneuver and surfactant therapy reduced mortality to 23% [29, 30]. I. A. Kozlov and A. A. Romanov [31] also used early Surfactant-BL administration and lung recruitment maneuver. They showed that this combination leads to stable recovery of lung oxygenating function in ARDS patients after operation on heart. I. A. Kozlov and V. N. Poptsov [32] employed NO inhalation for oxygenation improvement in 53 patients with ALI and ARDS after cardiac surgery. It was demonstrated that 23 patients did not respond positively to this therapy. Endobronchial Surfactant-BL administration to those patients resulted in significant increase in oxygenation and then helped them to recover from critical condition. Single surfactant administration was enough for stable improvement in arterial oxygenation and ARDS regression in 18% of patients. The other patients required 2—3 surfactant administrations. The mortality rate in a group of patients who did not have surfactant therapy was 50%, while in a group treated with surfactant mortality was 39%. The survivors (NO + Surfactant-BL) experienced CMV during 8.6±0.9 days compared to 13.5±1.6 days in control group (p<0.05), the time in ICU was 11.0±1.6 days for survivors and 16.4±2.0 days for control group (p<0.05).

Оказалось, что результат терапии сурфактантом-БЛ зависит от степени повреждения легких: он значительно ниже при распространенном ателектазировании (исходном Ра02/И02 < 150 мм рт. ст.) [26, 27]. Применение препарата в сочетании с приемом «открытия легких» также более эффективно по сравнению с раздельным использованием этих методов. Комбинация различных технологий ведения больных с ОРДС, включающая сурфактант-терапию, позволила авторам снизить летальность от ОРДС до 20% [2, 27].

2.2. Сурфактант-терапия ОРДС развившегося вследствие операций на сердце и крупных сосудах. А. Е. Баутин с соавт. [28] изучали эффективность сур-фактант-терапии при ОРДС после операций на сердце и аорте. Авторы показали, что применение препарата сурфактант-БЛ позволило экстубировать трахею у 72,2% пациентов, тогда как в контрольной группе таких больных было только 47,4%. Достигнутое улучшение газообмена носило устойчивый характер и привело к снижению летальности до 33,3% при 52,6% в контрольной группе. Впоследствии использование комбинации маневра мобилизации альвеол и сурфактант-терапии позволило снизить летальность до 23% [29, 30]. И. А. Козлов и А. А. Романов [31] также применили раннее введение сурфактанта-БЛ и маневр «мобилизации альвеол» и показали, что в 85,7% случаев эта комбинация приводит к стойкой нормализации оксигенирующей функции легких у больных с ОРДС после операций на сердце. И. А. Козлов и В. Н. Попцов [32] использовали ингаляции N0 для улучшения оксигенации у 53 больных с СОПЛ и ОРДС после кардиохирургических операций. Выяснилось, что 23 из них не отвечали позитивными сдвигами на эту терапию. Добавление эндобронхиального введения сурфактанта-БЛ этим 23 больным позволило существенно повысить оксигена-цию и вывести большинство из них из критического состояния. Однократного введения сурфактанта-БЛ было достаточно для стойкого улучшения артериальной оксигенации и регресса ОРДС у 18% больных, остальным пациентам потребовалось 2—3 введения препарата. В группе больных, не получавших сурфактант-БЛ, летальность составила 50%, а в группе получавших — 39%. У выживших пациентов (N0 + сурфактант-БЛ) длительность ИВЛ составила 8,6±0,9 суток против 13,5±1,6 в группе сравнения (р<0,05), длительность нахождения в ОРИТ — 11,0±1,6 суток против 16,4±2,0 суток (р<0,05).

2.3. Профилактика и лечение ОРДС при ингаляционной травме и ожоговой болезни. Поражение респираторной системы в структуре органной дисфункции при ожогах поверхности тела колеблется от 45 до 95%, а частота ОРДС — от 75 до 90% в зависимости от площади поражения. В 100% случаев при длительной ИВЛ в случае ингаляционной травмы и ее комбинации с ожогами поверхности тела развиваются пневмония и гнойный тра-хеобронхит, а частота ОРДС достигает 60—90% [33, 34].

2.3. The prophylaxis and treatment of ARDS induced by inhalation trauma and burn injury.

Respiratory system damage induced by body surface burns varies from 45% to 95%, while ARDS frequency fluctuates from 75% to 90% depending on the area of the injury. Long CMV at inhalation trauma and at its combination with body surface burns results in pneumonia and purulent bronchitis development in 100% of patients, ARDS frequency reaches 60—90% [33, 34].

M. Yu. Tarasenko and others [35—37] employed Surfactant-BL for the prophylaxis and treatment of ARDS in 37 patients (from 18 to 60 years old) with severe ther-mochemical damage of respiratory tract and without severe predisposing diseases. Burn area fluctuated from 12% to 53% of body surface. All the patients had unfavorable prognosis. 19 patients (treatment group) received Surfactant-BL in combination treatment of burn injury, 18 patients (control group) were not administered Surfactant-BL. All casualties had standard infusion-transfusion therapy, inotropic support, necessary antibacterial therapy, nutritional support. Hb level was kept not less than 100 g/l. All the patients were on pressure-controlled CMV by means of Puritan-Bennet 760 ventilator. CMV settings were as follows: tidal volume (TV) — 5—6 ml/kg; Peak — 28—30 cm H2O; PEEP — 8—12 cm H2O and FiO2 — 0.5—0.6. The patients of the control group suffered from increasing respiratory insufficiency at developing ARDS. The patients mainly were refractory to the intensive treatment and respiratory support (FiO2 — up to 0.7—0.8; Ppeak — up to 35—38 cm H2O; PEEP — up to 8—12 cm H2O). 16 of 18 patients of the group died during the first week after injury. The mortality rate was 89.9% [37]. The patients of the treated group received the first surfactant dose within 16—56 hours after the injury (32.5 hours on the average). Before the administration of the main dose BAL (10 ml of 0.1% emulsion of Surfactant-BL) had been carried out. After that the main dose (3 mg/kg) of the preparation was administered equally into right and left bronchi every 12 hours. Surfactant therapy was considered completed after reaching ratio of PaO2/FiO2 > 300 mm Hg and holding CMV at FiO2 < 0.4.

After the recovery of gas exchange in lungs and hemodynamic stabilization, the patients of the treated group waived CMV on 4nd—15th day after the start of surfactant therapy. 16 of 19 patients in this group survived in spite of initial unfavorable prognosis. The mortality rate in this group was 18.8% [37].

Based on these results, the authors proposed the technology of combination surfactant treatment of inhalation injury, which was used for emergency medical services including the fire in cafe «Chromaya loshad» in Perm in 2009. P. A. Brigin from Burn Centre of Research Institute of First Aid N. A. Sklifasovsky, A. A. Alexeev from Burn Centre of the A.V. Vishnevsky Institute of Surgery and O. N. Popechen and others from emergency care hospital in Minsk [38] have had the similar results of surfactant treatment of the patients with thermoinhalation trauma and the patients with ARDS induced by sepsis and burn

[35—37] использовали сурфактант-БЛ для профилактики и лечения ОРДС у 37 больных (от 18 до 60 лет) с тяжелыми термохимическими поражениями дыхательных путей без тяжелой предсуществующей патологии. Площадь ожогов варьировала от 12 до 53% поверхности тела. Прогноз для жизни у всех пациентов оценивался как неблагоприятный. 19 больных (основная группа) получали сурфак-тант-БЛ в комплексном лечении ожоговой травмы, и 18 человек (контрольная группа) не получали препарат. Всем пострадавшим проводилась стандартная ин-фузионно-трансфузионная терапия, ионотропная поддержка, необходимая антибактериальная терапия, парентеральное питание. Уровень гемоглобина поддерживался не ниже 100 г/л. Все пациенты находились на ИВЛ с контролем по давлению аппаратами «Пуритан-Беннет-760». Параметры ИВЛ были следующими: ДО — 5—6 мл/кг, Ppeak — 28—30 см H2O, PEEP — 8—12 см H2O и FiO2 — 0,5—0,6. У пострадавших контрольной группы, которым не проводилась сурфактант-терапия, нарастали явления дыхательной недостаточности на фоне развивающегося ОРДС. Проводимые интенсивные лечебные мероприятия и усиление агрессивной аппаратной поддержки (FiO2 — до 0,7—0,8, Ppeak — до 35—38 см Н2О, РЕЕР — до 10—12 см Н2О) в подавляющем большинстве случаев не привели к положительному эффекту 16 из 18 пострадавших этой группы погибли в течение первой недели после травмы. Летальность составила 89,9% [37]. Пациентам основной группы первое введение сурфактанта-БЛ начинали в течение 16—56 часов после поражения (в среднем 32,5 часа). Перед введением основной дозы проводили бронхо-альвеолярный лаваж 10 мл 0,1%-ной эмульсии сур-фактанта-БЛ, затем вводили основную дозу препарата (3 мг/кг) поровну в правый и левый главные бронхи каждые 12 часов. Сурфактант-терапию прекращали при достижении соотношения PaO2/FiO2 > 300 мм Hg и возможности проведения ИВЛ с FiO2 < 0,4.

После восстановления газообмена в легких и стабилизации гемодинамики пострадавшие основной группы были экстубированы на 4—15-е сутки после начала введения сурфактанта-БЛ. Из 19 пациентов основной группы выжили 16, несмотря на начальный неблагоприятный прогноз. Летальность в этой группе составила 18,8% [37].

На основании полученных результатов авторы предложили технологию комплексной сурфактант-те-рапии ингаляционной травмы, которая была использована при оказании помощи пострадавшим в ЧС, в том числе при пожаре в кафе «Хромая лошадь» в Перми в 2009 году. Сходные результаты лечения больных и пострадавших с термоингаляционной травмой и больных ОРДС на фоне сепсиса и ожоговой болезни получили также П. А. Брыгин из ожогового центра НИИ скорой помощи им. Склифосовского и А. А. Алексеев из ожогового центра НИИ хирургии им. А. В. Вишневского, а также О. Н. Почепень и соавт. из больницы скорой помощи г. Минска [38]. Сурфактант-терапия при ожогах

It should be mentioned that surfactant preparation Alveofact was successfully used for the treatment of inhalation trauma for the first time by N. Pallua and others in 1998 [40].

2.4 Surfactant therapy of ARDS induced by obstetric and gynecologic pathologies. Surfactant therapy allows to significantly reduce maternal mortality.This is why it is worth of focusing on critical care in obstetrics and gynecology. It was shown that obstetrics patients including pregnant women suffered from ARDS relatively rare (0.016—0.035% of all acts of delivery). However, ARDS was shown to be direct and associated cause of maternal death in 20—98% of cases, and in 20—50% caused their babies' death [41]. The main factors leading to ARDS development in obstetric patients are: aspiration of the gastric content, tocolytics effect, preeclampsia, eclampsia, pyelonephritis, chorioamnionitis, endometritis, septic abortion, thromboembolia, amniotic fluid embolism, bacterial and virus pneumonia, massive hemorrhage and hemo-transfusion [42].

I. I. Kukarskaya, M. V. Shvechkova and others [43] studied 62 obstetric patients who received Surfactant-BL in combination therapy of ALI and ARDS. ALI/ARDS diagnosis was established according to AESK criteria. Direct lung injury was caused by community-acquired pneumonia developed during the acute respiratory virus infection (ARVI) and influenza A/H1N1 (2009—2010) in 12 patients (19.4%), aspiration of gastric content in 5 patients (8.1%). Indirect lung injury was triggered by severe preeclampsia in 12 patients (19.4%), massive hemorrhage, hemorrhagic shock and massive hemotransfusion in18 patients (29.0%), sepsis in 10 patients (16.1%), amniotic fluid embolism in 2 patients (3.2%) and severe extra genital diseases in 3 patients (4.8%). All patients with ARDS induced by ARVI and influenza A/H1N1had pre-term delivery because of rapid development of respiratory failure. PaO2/FiO2 was within 100—150 mm Hg, and SpO2 was less than 90%. Surfactant-BL administration was combined with the therapy of the main disease and respiratory support. The preparation was administered by three ways: 1) endobronchial administration was carried out through fibrotic bronchoscope (150 mg into every bronchus, every 12 hours) in 3 patients (4.8%); 2) endobronchial bolus administration was implemented through catheter in side-lying position of the patient, leaving the patient in such position 60—120 min, then the procedure was repeated on the other side in 52 cases (83.9%) including 11 patients who also had lung recruitment manoeuvre; 3) inhalation administration was performed through nebulizer at a dose of 75 mg, every 12 hours in 7 patients (11.3%). The patients who experienced inhalation administration had unassisted breathing, but the features of developing respiratory failure and hypoxemia were considered as ALI, and surfactant administration was used to prevent ARDS

дыхательных путей используется также на Украине (препарат Сукрим) [39]. Следует отметить, что для лечения ингаляционной травмы препарат сурфактанта (Л1уеа£ас1) впервые успешно применили Ра11иа N. и со-авт. в 1998 году [40].

2.4. Сурфактант-терапия ОРДС в акушерско-ги-некологической клинике. Целесообразно остановиться на лечении критических состояний в акушерско-ги-некологической практике, так как сурфактант-терапия позволяет существенно улучшить ситуацию с материнской смертностью. Показано, что ОРДС у беременных и родильниц диагностируется сравнительно редко (в 0,016—0,035% ко всем родам), но может быть непосредственной и сопутствующей причиной смерти женщин в 20—98% случаев и их детей в 20—50% [41]. Основные причины развития ОРДС у беременных: аспирация желудочного содержимого, воздействие токолитиков, преэклампсия/эклампсия, пиелонефрит, хориоамнио-нит, эндометрит, септический аборт, тромбоэмболия, эмболия околоплодными водами, бактериальная и вирусная пневмонии, массивные кровотечения и гемо-трансфузии [42].

Кукарская И. И. и соавт. [43] обследовали 62 акушерских больных, у которых в комплексной терапии СОПЛ и ОРДС был использован сурфактант-БЛ. Диагноз СОПЛ/ОРДС ставили на основании критериев АЕСК. Прямое повреждение легких зарегистрировано при внебольничной пневмонии на фоне ОРВИ и гриппа А/НШ1 (2009—2010 гг.) у 12 пациентов (19,4%), аспирация желудочного содержимого — у 5 больных (8,1%). Непрямое повреждение легких произошло при тяжелой преэклампсии у 12 больных (19,4%), острой массивной кровопотере и массивной гемотрансфузии у 18 больных (29,0%), сепсисе — 10 (16,1%), эмболии околоплодными водами — 2 (3,2%) и тяжелой экстрагенитальной патологии — 3 (4,8%). Все больные с ОРДС на фоне ОРВИ и гриппа А/НШ1 были родоразрешены досрочно в связи со стремительным нарастанием дыхательной недостаточности. Соотношение Ра02/РЮ2 было в пределах 100—150 мм рт. ст., а 8р02 — ниже 90%. Введение сур-фактанта-БЛ на фоне лечения основного заболевания и респираторной поддержке осуществляли тремя способами: 1) эндобронхиально с помощью фиброброн-хоскопа (по 150 мг в каждый бронх с интервалом 12 часов) — 3 случая (4,8%); 2) эндотрахеально болюсно через катетер в положении больной на боку, оставляя больную в этом положении на 60—120 мин, а затем процедуру повторяли на другом боку — 52 случая (83,9%), из них у 11 больных — в сочетании с маневром «открытия легких»; 3) ингаляционно через небу-лайзер по 75 мг через каждые 12 часов — 7 больных (11,3%). Пациенты, которым препарат вводился инга-ляционно, дышали самостоятельно, но признаки нарастающей дыхательной недостаточности и гипоксемии расценивались как СОПЛ, и введение препарата являлось профилактикой развития ОРДС. У больных, получавших ИВЛ, перед введением препарата прово-

Surfactant-BL was administered within 3—4 hours from the manifestation of severe hypoxemia in 17 cases (27.4%), within 12 hours in 23 cases (37.1%), within 24 hours in 19 cases (30.6%) and 48 hours in 3 cases (4.8%).

All the patients demonstrated deterioration of gas parameters and biochemical lung characteristics. However, six hours after endobronchial surfactant administration, oxygenation index and blood saturation exceeded significantly initial gas exchange figures. PaO2/FiO2 ratio increased to 220—240 mm Hg that corresponded to increase by 80—100% compared to initial level. Within the first 24 hour of combination treatment FiO2 was reduced to 40—50% and PEEP decreased to 8—10 cm H2O. The maintenance of necessary level of oxygenation required Surfactant-BL administration every 12 hours during 2—3 days. Deterioration of blood gases immediately after administration can be explained by the procedure of bron-choscopy itself and fluid instillation into lung. Combination application of surfactant and lung recruitment maneuver prevents this reaction.

Long experience of surfactant treatment in obstetric patients showed that surfactant administration should be carried out till stable improvement of gas exchange and X-ray findings. Early termination of surfactant therapy after reaching short-term positive effect can lead to occasional hypoxemia progress and multiple organ dysfunction syndrome development.

From 2005 to 2012, when surfactant therapy was used in combination treatment of ALI and ARDS, in Tyumen Perinatal Centre only 3 of 62 ARDS maternity patients died (mortality 4.8%) with diseases which were not connected with lung injury [43].

2.5 Surfactant therapy in combination ARDS treatment in virus pneumonia. During the epidemic of A/H1N1 in professional literature and mass media there was information that as a result of the development of severe bilateral pneumonia induced by A/H1N1 virus mortality rate is considerably higher compared to seasonal-flu epidemic. According to Antoine Flahault, professor infec-tiologist, the mortality rate from ARDS induced by A/H1N1 virus was 100 times higher compared to the mortality rate from seasonal flu [44]. In many countries where epidemic broke out, the mortality of the CMV patients was 41—65% [45]. From the very beginning, it was found out that high-risk groups of the severe bilateral pneumonia and ARDS development are young people under 20 years old (approximatelynearly 50%), pregnant women (up to 15%) and obese people (> 10%).

In foreign countries, antivirus treatment and emergency care including CMV and extracorporeal membrane oxygenation were used for the treatment of those patients. Surfactant therapy was not used as there were no surfactant preparations which had been permitted for adults

Сурфактант-БЛ вводили в течение 3—4 часов от момента проявлений тяжелой гипоксемии в 17 случаях (27,4%), 12 часов — 23 (37,1%), 24 часов — 19,6 (30,6%) и 48 часов — 3 (4,8%).

Во всех случаях в течение первых двух часов после введения препарата отмечалось ухудшение показателей газообмена и биомеханических свойств легких. Однако в последующем показатели газообмена постепенно улучшались, и в среднем через 6 часов после эн-добронхиального введения препарата индекс оксигена-ции и насыщение крови кислородом достоверно превышали исходные значения. Соотношение PaO2/FiO2 повышалось до 220—240 мм Hg (увеличение на 80—100% по отношению к исходному уровню). Уже в течение первых суток комплексной терапии удавалось снизить FiO2 до 40—50% и PEEP до 8—10 см вод. ст. Поддержание необходимого уровня оксигена-ции требовало введения сурфактанта БЛ каждые 12 часов в течение двух — трех суток. Ухудшение показателей газов крови сразу после введения авторы связывали с самой процедурой бронхоскопии и ин-стилляцией жидкости в легкие. Сочетанное использование препарата и «маневра открытия легких» предотвращало эту реакцию.

Многолетний опыт применения сурфактант-те-рапии у акушерских больных показал, что введение сурфактанта-БЛ необходимо продолжать до стойкого улучшения параметров газообмена и рентгенологических данных. Преждевременная отмена сурфактант-те-рапии при достижении кратковременного положительного эффекта может привести к повторному прогрессированию гипоксемии и, как следствие, развитию синдрома полиорганной недостаточности.

За время использования сурфактант-терапии в комплексном лечении СОПЛ и ОРДС с 2005 по 2012 гг. из 62 больных с ОРДС, получавших лечение в ОРИТ Перинатальнолго центра г. Тюмени, умерли только три родильницы (летальность 4,8%), от причин, не связанных с повреждением легких [43].

2.5. Сурфактант-терапия в комплексном лечении ОРДС при вирусных пневмониях. Во время эпидемии свиного гриппа A/H1N1 в специальной литературе и средствах массовой информации появлялись сообщения о том, что при развитии тяжелой двусторонней пневмонии на фоне гриппа A/H1N1 отмечается существенно большая смертность по сравнению с сезонными эпидемиями. Так, по мнению профессора инфекциониста Antoine Flahault, летальность от ОРДС на фоне гриппа A/H1N1 была в 100 раз выше таковой для эпидемий сезонного гриппа [44]. Во многих странах, где проходила эпидемия свиного гриппа, летальность больных, находившихся на ИВЛ, составила 41—65% [45]. Достаточно быстро было выяснено, что группами риска развития тяжелой двусторонней пневмонии и

[45]. But there are some data on the efficiency of preparation Curosurf in 24 ARDS patients (12 patients in the main group and 12 patients in control group) in pilot investigation in Czech Republic [46]. The authors demonstrated significant reduction in mortality in patients who received surfactant preparation.

In Russia in some regions, surfactant treatment for A/H1N1 patients with bilateral pneumonia and ARDS was used. The first data were published by A. V. Alekseev and others (S. P. Botkin Clinical Infectious Diseases Hospital, Saint-Petersburg) at the height of epidemic in December 2009 [19]. The treatment of eight patients with severe ARDS induced by A/H1N1 flu was described. All the patients had unfavourable prognosis. Together with antivirus treatment, respiratory support and other emergency care, the patients were administered 150 mg of Surfactant-BL 2 times a day, during 3—5 days. All the patients survived.

Later the results of the treatment of 48 ARDS patients from 22 to 59 years old (including 21 pregnant women and 9 patients with obesity of III—IV degree) were published [20, 21]. A/H1N1 diagnosis was confirmed with serological test and PCR. The patients were divided into two groups. The first group comprised 23 patients (including 12 pregnant women) who received respiratory support, antivirus treatment (oseltamivir, 150 mg 2 times a day during 10 days) and Surfactant-BL at a dose of 150 mg 2 times a day during 3—5 days endobronchially. The second (control) group comprised 25 patients (among them 9 pregnant women) at the same condition who received the same therapy but Surfactant-BL. All the patients were in extremely critical condition. After admission they were immediately put into ICU. The patients had dyspnoe, 28—34 breathings a minute, and SatO2 < 92% at oxygen administration 7—10 l/min through a mask. If the patient condition were not improved within 3 hours, patients were transferred to invasive lung ventilation (BIPAP, CMV) with the following settings: FiO2 = 0.8—1.0, PEEP = 14—20 cm H2O, Pinsp = 30—35 cm H2O. During the first 2 days, sedation and muscle relaxation were employed to keep the high level of PEEP. As a rule this therapy did not lead to a stable positive result. In the majority of cases the static compliance did not fall down below 35—40 ml/cm H2O. X-ray examination registered bilateral pneumonia and ARDS in all the patients.

In the first group initial PaO2/FiO2 ratio was 119±18.2 mm Hg. The administration of Surfactant-BL was started within 2 days after the CMV beginning. Six-eight hours after the first surfactant administration, PaO2/FiO2 ratio increased to 220—240 mm Hg (by 100— 130% compared to initial level, (P<0.001). During the first day of combination therapy, FiO2 dropped to 40—50% and PEEP went down to 10 cm H2O. The maintenance of that level of oxygenation required Surfactant-BL administration every 12 hours during 3—5 days. In most cases, PEEP went down to 8 cm H2O by the 2nd—3d day and to 5 cm H2O by the 5th day. At the same time, PaO2 was kept at 100 mm Hg, and Pa02/FiO2 ratio was kept at 240 and

За рубежом для лечения этих больных использовали противовирусную терапию и комплекс реанимационных мероприятий, в том числе ИВЛ и ЭКМО. Сурфактант-терапию не применяли в связи с отсутствием препаратов сурфактанта, разрешенных для применения у взрослых [45]. Однако имеются данные об исследовании эффективности препарата Curosurf у 24 больных (12 в основной и 12 в контрольной группах) в пилотном исследовании в Чехии [46]. Авторы показали существенное уменьшение летальности у пациентов, получавших препарат сурфактанта.

В России использовали сурфактант-терапию для лечения больных гриппом A/H1N1 с двусторонней пневмонией и ОРДС в ряде регионов. Первая работа была опубликована в разгар эпидемии в декабре 2009 г. А. М. Алексеевым и соавт. (инфекционная больница им. С. П. Боткина в Санкт-Петербурге) [19]. Описано лечение 8 случаев тяжелых больных ОРДС на фоне подтвержденного гриппа A/H1N1 с неблагоприятным прогнозом. Наряду с противовирусной терапией, респираторной поддержкой и другими реанимационными мероприятиями пациентам вводили по 150 мг сурфак-танта-БЛ 2 раза в сутки в течение 3—5 дней. Все 8 больных выжили.

В дальнейшем были опубликованы результаты лечения 48 больных в возрасте от 22 до 59 лет (из них 21 беременная и 9 больных с ожирением III—IV степени) с ОРДС [20, 21]. Диагноз A/H1N1 подтвержден с помощью серологических реакций и ПЦР. Больные были разделены на две группы. Первая группа состояла из 23 больных (включая 12 беременных), которые получали респираторную и противовирусную терапию (осельтамивир, 150 мг 2 раза в сутки в течение 10 дней) и сурфактант-БЛ по 150 мг 2 раза в сутки в течение 3— 5 дней эндобронхиально. Вторая группа состояла из 25 больных (9 беременных), с той же степенью тяжести, получавших аналогичную терапию, за исключением сурфактанта-БЛ. При госпитализации все больные сразу поступали в ОРИТ. Больные имели частоту дыханий 28—34 в минуту и Sat02 < 92% при подаче кислорода с потоком 7—10 литров/минуту через маску. Если в течение трех часов не удавалось улучшить состояние больного, ему осуществляли инвазивную вентиляцию легких (BIPAP, CMV) со следующими параметрами: Fi02 = 0,8—1,0; PEEP = 14—20 см H20, Pinsp = 30—35 см H20. В течение первых двух суток применяли седацию и миорелаксацию из-за необходимости поддерживать высокий уровень PEEP. Как правило, эта терапия не давала стойкого положительного эффекта. В большинстве случаев показатель статического комплайенса не повышался выше 35—40 мл/см H20. Рентгенологически у всех больных регистрировали двустороннюю пневмонию и ОРДС.

higher (FiO2 = 0.4—0.5). CMV lasted 12—30 days as in most cases, hospital-acquired pneumonia joined. On the average, the period of CMV in the first group was 16.0±0.89 days. One of 23 patients of the treated group died on the 8th day after the beginning of the therapy. She had suffered HIV and narcomania. The mortality in this group was 4.3%. Surfactant-BL administration started as early as possible, and the preparation was efficient even when initiated on 3d—4th day of CMV.

In the second group (25 patients that did not receive surfactant), the initial Pa02/FiO2 ratio was 123±20.8 mm Hg at the moment of CMV start, and its growth was +22.7% vs. initial mean up to 151±24.3 mm Hg (P<0.05). Sixteen of 25 patients in this group died (64.0% mortality, P<0.001). The period of CMV in the survivors in the second group was 29.3±2.8 days (P<0.001) [20, 21].

In Russia during A/H1N1 epidemia of 2009—2010 more than 200 patients underwent surfactant therapy. Approximately 10% of the patients with bilateral virus pneumonia received preparation at a dose of 75 mg 2 times a day by means of inhalation through a mask while keeping an independent breath. This therapy prevented ARDS development and patient transfer to CMV [20, 21].

2.6. Surfactant prophylaxis and treatment of primary lung graft dysfunction. M. Sh. Habutia and others [47] (Research Institute of First Aid N. A. Sklifasovsky) reported that one of the main clinical tasks after lung transplantation is prophylaxis and efficient treatment of primary lung graft dysfunction (PLGD). According to International Society for Heart and Lung Transplantation (ISHLT), PLGD is one of the main reasons for mortality (up to 50%) during the first month after transplantation. Clinical picture of PLGD is similar to ARDS to a large extent. The authors studied the effect of early endo-bronchial Surfactant-BL administration on post-transplantation PLGD development. Eight patients 24—55 years old (36.3±4.0) were observed. Surfactant-BL was administered to lobar, segmental and accessible sub segmental bronchi via bronchoscope. Surfactant-BL dose was 300 mg (5.3±0.3 mg/kg). The preparation at that dose was administered immediately after the operation, in 24 and 48 hours. Early endobronchial Surfactant-BL administration according to the schedule 0—24—48 was accomponied by significant growth of Pa02/FiO2 ratio compared to initial values after the operation (Pa02/FiO2<0.05). By 48 hours after the surgery Pa02/FiO2 growth was 42.7%. The trend for increasing the dynamic toracopulmonary compliance was more than 50% (P>0,05), and was accompanied by the reduction of the PLGD grade (P>0.005). After 48 hours PGD had the score of 1.1±0.5, which allow 4 of 5 patients (80.0%) to waive CMV within the first 5 days after transplantation. Two patients were excluded from the study: one patient had total pulmonary edema and oxygenation function failure (patient A), while the another one had mismatch of anthropometric measurements of the donor and recipient lungs (patient B) . Extracorporeal membrane oxygenation was arranged for these patients urgently. This measure together with intensive respiratory therapy was

ИВЛ продолжали в течение 12—30 суток, так как в большинстве случаев развивалась нозокомиальная пневмония. У выживших больных первой группы длительность ИВЛ составила в среднем 16,0±089 суток. Из 23 больных умерла одна пациентка (беременная, страдавшая ВИЧ, наркоманией) на 8-е сутки после начала лечения. Летальность в этой группе составила 4,3%. Введение сурфактанта-БЛ стремились начинать как можно раньше, однако препарат давал четкий эффект и в случае его применения на 3—4 сутки проведения ИВЛ.

Среди 25 больных второй группы, не получавших сурфактант, исходное соотношение PaO2/FiO2 при начале ИВЛ составило 123±20,8 мм Hg, а прирост показателя по отношению к исходной величине составил 22,7% до 151±24,3 мм Hg, (^>0,05). В этой группе из 25 больных умерли 16 (64,0%), (р<0,001). Время ИВЛ у выживших больных во второй группе составило 29,3±2,8 суток (р<0,001) [20, 21].

В России во время эпидемии гриппа A/H1N1 2009—2010 гг. лечение сурфактантом-БЛ получили более 200 больных. Около 10% пациентов с явлениями двусторонней вирусной пневмонии получали препарат в дозе 75 мг 2 раза в сутки ингаляционно через маску при самостоятельном дыхании. Эта терапия предотвращала развитие ОРДС и использование ИВЛ [20, 21].

2.6. Профилактика и терапия препаратом сурфак-танта первичной дисфункции легочного трансплантата. М. Ш. Хубутия и соавт. [47] (НИИ скорой помощи им. Н. В. Склифосовского) сообщили, что ведущей клинической задачей после трансплантации легких является профилактика и эффективное лечение первичной дисфункции легочного трансплантата (ПДЛТ). По данным Международной ассоциации трансплантации сердца и легких, ПДЛТ является одной из основных причин смертности (до 50%) пациентов в первый месяц после трансплантации. Клиническая картина ПДЛТ во многом сходна с ОРДС. Авторы изучили влияние раннего эндо-бронхиального введения сурфактанта-БЛ на развитие ПДЛТ после трансплантации легких. Всего обследовали 7 больных в возрасте 24—55 лет (36,3±4,0). Сурфактант-БЛ вводили в долевые, сегментарные и доступные субсегментарные бронхи при помощи бронхоскопа. Доза сур-фактанта-БЛ составляла 300 мг (5,3±0,3 мг/кг). Препарат в этой дозе вводили непосредственно после операции, через 24 и 48 часов. Оказалось, что раннее эндобронхиаль-

efficient for patient A (24 years old). She waived extracor-poreal membrane oxygenation on the second day, and in 24 hours she also waived CMV. This therapy was not efficient in patient B, who died.

The authors concluded that early usage of Surfactant-BL after lung transplantation contributed to the optimization of lung biomechanical properties and oxygenation function. They believe that surfactant therapy should be used in combination for PGD prophylaxis and treatment. By May 2014, 22 patients experienced lung transplantation in Research Institute of First Aid N. A. Sklifasovsky. PLGD surfactant prophylaxis was employed in this institution as a standard procedure. The data are currently being prepared for publishing (A. A. Romanov, personal communication).

To conclude this section, it is necessary to emphasize that the long-term experience in ARDS surfactant therapy in many hospitals of RF and Belarus (more than 5,000 patients totally) has demonstrated therapeutic efficacy and high potential of surfactant preparations for ARDS prophylaxis and treatment.

It should be mentioned that natural preparations, which have more complex composition and are closer to the structure and properties of lung surfactant in situ, show stronger therapeutic effect compared to synthetic preparations.

Controversial results of MRCT performed in negatively in different countries had a negative impact on acceptance of surfactant therapy by international community of anesthesiologists and resuscitationists, however, positive results of surfactant therapy obtained in Russia and Belarus are promising.

3. Surfactant therapy of other lung diseases

The use of lung surfactant preparations for other diseases, excepting IRDS and ARDS in children and adults, is rather limited. Nevertheless, the attempts to employ different surfactant preparations in treatment of severe pneumonia [20, 21, 48, 49], atelectasis in children and adults [50], bronchial asthma [51—53] and lung tuberculosis [54—56] are promising.

Surfactant-BL efficacy was studied in a pilot trials for the treatment of sustained atelectasis caused by acute bronchopneumonia in 60 children from 9 months to 14 years old. Lobe or segment density registered in X-ray pictures within 14—60 days were considered as sustained atelectasis. Selective Surfactant-BL administration with fiber-optic bronchoscope at a dose of 75—225 mg per administration resulted in the reduction of therapeutic bronchoscopy number from 3.4 to 1.3. The increase in the frequency of complete atelectasis recovery varied from 57% to 86% that resulted in reduction of period of hospi-talization [50].

Experimental studies demonstrated the feasibility of bronchial asthma surfactant therapy, however, clinical studies were limited [51, 52]. S. B. Oetomo and others [53]

ПДЛТ через 48 часов оценивалась в 1,1±0,5 балла, что позволило в 4-х случаях из 5 (80,0%) выполнить экстуба-цию трахеи в течение первых 5 суток после трансплантации. Двое больных были исключены из исследования: одна — из-за возникновения тотального отека легких и несостоятельности его оксигенирующей функции (пациентка А), а другая (пациентка В) — из-за несоответствия антропометрических характеристик легких донора и реципиента. Этим больным в экстренном порядке была налажена ЭКМО. Пациентку А., 24-х лет на фоне описанного методического приема и интенсивной респираторной терапии удалось «отлучить» от ЭКМО на вторые сутки, а еще через 24 часа — экстубировать трахею. В другом наблюдении (пациентка В), где также было использовано ЭКМО, отмечено отсутствие эффекта от терапии. Больная умерла. Авторы приходят к заключению, что раннее использование сурфактанта-БЛ после трансплантации легких способствовало оптимизации биомеханических свойств и оксигенирующей функции легких. Они полагают, что сурфактант-терапия должна занять прочную позицию в комплексной профилактике и лечении ПДЛТ. К маю 2014 года трасплантацию легких в НИИ скорой помощи им. Н. В. Склифосовского осуществили 22 больным. Сурфактант-профилактика ПДЛТ используется в этом учреждении в качестве стандарта. Результаты этого исследования готовятся к публикации (персональное сообщение А. А. Романова).

В заключении этого раздела необходимо подчеркнуть, что многолетний опыт сурфактант-терапии ОРДС во многих клиниках РФ и республики Беларусь (всего более 5000 больных) убеждает в эффективности и перспективности использования препаратов сурфак-танта для профилактики и лечения ОРДС.

Следует еще раз напомнить, что природные препараты, более сложные по составу и близкие по структуре и свойствам легочному сурфактанту in situ, обладают большим терапевтическим эффектом в сравнении с синтетическими.

Отрицательное отношение к сурфактант-терапии за рубежом, основанное на ошибочных результатах МРКИ, оказывает негативное влияние на сообщество анестезиологов-реаниматологов несмотря на очевидные положительные результаты сурфактант-терапии в России и республике Беларусь.

3. Сурфактант-терапия других заболеваний легких

Опыт применения препаратов легочного сурфак-танта при других состояниях, помимо РДС новорожденных и ОРДС у детей и взрослых, достаточно ограничен.

studied the efficiency of Surfacten inhalation for asthma attack relief in 12 children. They did not notice any positive changes in respiratory function. Kurashima [52], in contrast, described significant improvement in respiratory function in 11 patients with persistent airway obstruction. Preparation Curosurf deteriorated respiratory function in cases of allergen provocation in moderate asthma patients [53]. Clinical trials of preparation Surfacten in 50 patients during asthma attack led to distinctive positive effect.

There are few publications on surfactant use prospective in COPT patients. A.Anzuetto and others [57] showed essential dose-dependent improvement in mucociliary clearance and respiratory function in patients with chronic bronchitis who received surfactant inhalation compared to the patients of control group.

3.1. Surfactant therapy of lung tuberculosis. Surfactant-BL has been efficiently used for lung tuberculosis treatment [54—56]. MRCT was carried out in 140 patients: 70 patients received two-month treatment with Surfactant-BL inhalation together with 4—5 antituberculosis drugs; the other 70 patients received only antituberculosis drugs [54, 55]. More than 80% of patients noticed easier sputum excretion and increase in its quantity, the reduction or disappearance of coughing painfulness after 2—5 surfactant inhalation. The patients with massive excretion of bacteria had fewer bacteria in sputum, and some of the patients demonstrated no bacteria the month after the start of surfactant therapy. There disappeared intoxication manifestations: night sweating, poor exercise tolerance, weight deficit, coughing. 4 months after surfactant therapy start, disappearance of bacteria in sputum was found out in 83% of patients compared to 64.3 in control group; resolution of infiltrative lesions — in 94.1% of patients compared to 67.1% in control. Eighty-four percent of patients who received surfactant treatment had pulmonary cavern closure, which is one of the most important indicators of treatment efficacy. In control group, pulmonary cavern closure was found out only in 47% of patients. Later it was found out that after surfactant-treatment, abacillation level in newly diagnosed TB patients reaches 83—85% in both patients with chemo sensitivity and patients with multiple and broad drug resistance. Complete closure or essential reduction in size was discovered in patients with multiple destruction sites from 3 to 8 cm in diameter [56].

The mode of Sutfactant-BL curative action in combination treatment of severe lung tuberculosis is not clear, however, the patients experience the improvement in mucociliary clearance and bronchial drainage function [58, 59]. It seems to be a reason for fast intoxication manifestation decrease and improvement in exercise tolerance. It was demonstrated that the structure of alveolar macrophage population isolated from BAL fluid had changed, and the normalization of their differentiation took place after 20—23 preparation inhalation [59]. By now more than 2500 patients have received complication treatment of lung tuberculosis which included surfactant therapy. Besides the reduction of treatment period, the

Однако попытки использования различных препаратов сурфактанта при тяжелой пневмонии [20, 21, 48, 49], ателектазах у взрослых и детей [50], бронхиальной астме [51—53] и туберкулезе легких [54—56] обнадеживают.

Так, эффективность сурфактанта-БЛ была изучена в пилотных испытаниях для лечения стойких ателектазов, развившихся на фоне острой бронхопневмонии у 60 детей от 9 месяцев до 14 лет. Под стойкими ателектазами понимали регистрируемое рентгенологически в течение 14—60 дней затемнение сегмента или доли легкого. Селективное введение сурфактанта-БЛ с помощью фибробронхоскопа в дозе 75—225 мг на 1 введение приводило к уменьшению количества санацион-ных фибробронхоскопий с 3,4 до 1,3; увеличению частоты полного разрешения ателектазов с 57% до 86% и уменьшению продолжительности госпитализации [50].

Экспериментальные исследования убеждают в обоснованности попыток сурфактант-терапии бронхиальной астмы, однако подобные клинические исследования малочисленны [51, 52]. Ое1ошо 8. В. и соавт. [53] исследовали эффективность купирования астматического приступа у 12 детей ингаляционным введением препарата 8иг£ае1еп и не обнаружили позитивных изменений функции внешнего дыхания, тогда как К. КигазЫша [52] выявил существенное улучшение функции дыхания у 11 больных со стойкой обструкцией дыхательных путей. У больных с «мягким» течением бронхиальной астмы введение препарата Сигозиг! ухудшило функцию дыхания при аллергенной провокации [53], а клинические испытания препарата 8иг£ае1еп у 50 больных во время приступа бронхиальной астмы дали отчетливый положительный эффект.

Имеются единичные публикации о перспективах использования препаратов сурфактанта у больных ХОБЛ. А. Амиейо и соавт. [57] показали существенное дозо-зависимое улучшение мукоциллиарного клиренса и функции дыхания у больных хроническим бронхитом, получавшим ингаляции сурфактанта в сравнении с больными контрольной группы.

3.1. Сурфактант-терапия туберкулеза легких. Опубликованы данные об эффективном применении сурфактанта-БЛ у больных туберкулезом легких [54—56]. Проводилось многоцентровое РКИ у 140 больных, 70 из которых получали двухмесячный курс ингаляций сурфактанта-БЛ в комплексе с 4—5 противотуберкулезными препаратами, а 70 — только противотуберкулезные препараты [54, 55]. Более 80% больных после двух-пяти ингаляций отмечали облегчение выведения мокроты и увеличение ее количества, уменьшение или исчезновение болезненности кашля. У больных с массивным бактериовыделением наблюдалось уменьшение числа бактерий в мокроте через месяц после начала курса сурфактант-терапии, а у ряда больных наблюдалось абациллирование. Исчезали явления интоксикации: потливость по ночам, плохая переносимость физической нагрузки, дефицит веса, кашель. Через 4 месяца после начала курса сурфактант-терапии исчезновение бактерий в мокроте было выявлено у 83%

Conclusion

Surfactant therapy of IRDS is a generally accepted method of treatment, which results in reduction of neonatal mortality. The efficacy of surfactant use is not doubtful for the correction of syndromes caused by secondary surfactant deficit in newborns like meconium aspiration, postnatal pneumonia and bronchopulmonary dysplasia. The prospective of the use of lung surfactant preparations for lung disease treatment in adults is also obvious. Two surfactant preparations Surfactant-BL and Infasurf reduce mortality rate in ARDS significantly. Gradually these preparations are being included into resuscitation system. Surfactant-BL is also used for the treatment of lung tuberculosis. The study of surfactant efficacy is being carried out for the treatment other lung diseases: chronic bronchitis, bronchial asthma and others [60].

У 84% больных, получивших курс сурфактанта, отмечали закрытие каверн, что является важнейшим критерием эффективности лечения. В контрольной группе закрытие полостей наблюдали только у 47% больных. Впоследствии было выяснено, что уровень абациллирования у больных с впервые выявленным туберкулезом легких как при сохраненной чувствительности к химиотерапии, так и при множественной и широкой лекарственной устойчивости после проведения курса сурфактант-терапии также достигает 83—85%. У больных с множественными очагами деструкций величиной от 3 см до 8 см в диаметре обнаружена возможность или их полного закрытия, или существенного уменьшения размеров [56].

Механизм действия сурфактанта-БЛ в комплексном лечении тяжелых форм туберкулеза не ясен, однако обнаружено, что у больных происходит улучшение мукоциллиарного клиренса и дренажной функции бронхов [58, 59]. По-видимому, этим обусловлено быстрое уменьшение проявлений интоксикации и улучшение переносимости физической нагрузки. Показано также, что структура популяции АМ, выделенных из жидкости БАЛ, изменяется, и нормализация их диффе-ренцировки происходит после 20—23 ингаляций препарата [59]. К настоящему времени более 2500 больных получили комплексное лечение туберкулеза легких, включающее курс сурфактант-терапии. Помимо сокращения сроков, наиболее важным результатом лечения является быстрое абацилирование (более 85%) как у впервые выявленных, так и у больных с рецидивами, что предполагает существенное улучшение эпидемио-

логической обстановки в случае широкого внедрения сурфактант-терапии.

Заключение

Сурфактант-терапия РДС новорожденных является общепризнанным методом лечения, внедрение которого привело к существенному снижению неонаталь-ной смертности. Не вызывает сомнения эффективность применения препаратов сурфактанта для коррекции синдромов с вторичным дефицитом сурфактанта у новорожденных, таких, как аспирация мекония, постна-

Проводится изучение эффективности препаратов сурфактанта при других хронических заболеваниях легких: хронический бронхит, бронхиальная астма и другие [60].
