Угроза биотерроризма одна из разновидностей угрозы терроризма в целом, при том что между проблемами биотерроризма и биологической войны существует несомненная близость, поскольку средствами поражения в обоих случаях выступают патогенные микроорганизмы. Благодаря подписанию (1972) и вступлению в силу (1975) международной Конвенции о запрещении разработки, производства и накопления запасов бактериологического (биологического) и токсинного оружия и об их уничтожении угроза полномасштабной биологической войны может считаться устраненной. Акты же биотерроризма в будущем не исключены, хотя и маловероятны. Более актуальной остается угроза со стороны несчастных случаев и чрезвычайных происшествий при работе с патогенами в научных исследованиях и на производстве биопрепаратов, как и угроза нозокомиальных заражений. Однако наибольшую опасность для современного человечества представляют все же не антропогенные, но естественные биологические угрозы в виде вспышек неизвестных ранее или неэндемичных для данного региона инфекционных заболеваний.

BIOTERRORISM IN SERIES OF BIOLOGICAL THREATS: PAST AND PRESENT

The threat of bioterrorism is one of the forms of terroristic threat in general, despite the fact that there is an undoubted proximity between the issues of bioterrorism and biological warfare, as in both cases pathogens are the means of the destruction. Since international Convention on the prohibition of the development, production and stockpiling of bacteriological (biological) and toxin weapons and on their destruction has been signed (1972) and came into force (1975), the threat of full-scale biological war can be considered to be eliminated. The acts of bioterrorism in the future are not excluded, although they are unlikely. The threat due to accidents and emergencies while working with pathogens in the research process and during the production of biologic products, as well as the threat of nosocomial infections, remain to be more urgent. However, not anthropogenic, but natural biological threats, in the form of outbreaks of infectious diseases, previously unknown or non-endemic for some particular region, are the greatest danger to the modern mankind.

Угроза биотерроризма - одна из разновидностей угрозы терроризма в целом, при том что между проблемами биотерроризма и биологической войны существует несомненная близость, поскольку средствами поражения в обоих случаях выступают патогенные микроорганизмы. Благодаря подписанию (1972) и вступлению в силу (1975) международной Конвенции о запрещении разработки, производства и накопления запасов бактериологического (биологического) и токсинного оружия и об их уничтожении угроза полномасштабной биологической войны может считаться устраненной. Акты же биотерроризма в будущем не исключены, хотя и маловероятны. Более актуальной остается угроза со стороны несчастных случаев и чрезвычайных происшествий при работе с патогенами в научных исследованиях и на производстве биопрепаратов, как и угроза нозокомиальных заражений. Однако наибольшую опасность для современного человечества представляют все же не антропогенные, но естественные биологические угрозы - в виде вспышек неизвестных ранее или неэндемичных для данного региона инфекционных заболеваний.

Терроризм

Терроризм, без преувеличения, представляет собой один из наиболее серьезных вызовов, с которыми сталкивается человечество на современном этапе своего развития.

Из самого термина «терроризм» (от лат. terror - букв. «страх», «ужас» [1]) следует, что сущность последнего как общественного явления состоит в сознательном и целенаправленном запугивании отдельных лиц и общественных групп, общества в целом, руководителей государственных институтов и государств как таковых с целью принудить их к исполнению тех или иных требований или направить их действия в русло, соответствующее интересам конечных бенефициаров и непосредственных заказчиков того или иного террористического акта.

К сожалению, общепринятого международного толкования термина «терроризм» до сих пор не существует. Так что в разное время и при разных обстоятельствах он применялся и применяется для обозначения как откровенно «уголовных элементов, осуществивших захват заложников», так и «участников различных сепаратистских и национально-освободительных движений, революционных деятелей, призывающих к насильственному свержению власти, членов радикальных религиозных организаций, борющихся за доминирование в обществе или регионе», и т.д. [2].

Действующее российское законодательство определяет терроризм как «идеологию насилия и практику воздействия на принятие решения органами государственной власти, органами местного самоуправления или международными организациями, связанные с устрашением населения и (или) иными формами противоправных насильственных действий» [3]. Соответственно, сам террористический акт определяется в российском законодательстве как «совершение взрыва, поджога или иных действий, устрашающих население и создающих опасность гибели человека, причинения значительного имущественного ущерба, либо наступления иных тяжких последствий, в целях дестабилизации деятельности органов власти или международных организаций, либо воздействия на принятие ими решений, а также как угроза совершения указанных действий в тех же целях» [4].

Биотерроризм

Одной из разновидностей терроризма выступает биотерроризм (БТ), который предполагает осуществление по тому или иному адресу угрозы преднамеренного, сознательного и целенаправленного использования патогенных микроорганизмов - вместо взрывчатых веществ и других средств поражения, применяемых при совершении «традиционных» террористических актов. Как и в случае последних, мотивы

для совершения актов БТ могут быть самыми разными - от намерений воздействовать на текущую общественно-политическую ситуацию в том или ином государстве или регионе до стремления установить в нем свое политическое или религиозное лидерство. При этом акты БТ, подобно актам «традиционного» терроризма, могут осуществляться как самостоятельно действующими одиночками или группами террористов, так и целыми организациями.

Существует несомненная близость между проблемами БТ и биологической войны (БВ). Последняя характеризуется как широкомасштабное, заранее спланированное применение возбудителей инфекционных болезней (патогенов) и продуктов их жизнедеятельности (токсинов) в качестве средств поражения популяции людей или ее части с целью лишить или ослабить их бое- или дееспособность, дезорганизовать управление войсками и экономикой, а также системой медицинского обеспечения, что в целом призвано способствовать достижению стратегических целей. Напротив, акты БТ направлены против отдельных лиц или групп людей и преследуют, в основном, цели устрашения и шантажа, причем не только самих инфицированных лиц и групп, но и тех, кто их окружает, но не стал прямым объектом атаки [5].

Первые достоверные сведения о планировании актов БТ («применять в случае отсутствия яда для отравления пуль разводку заразных бактерий: чахотки, столбняка, дифтерита, брюшного тифа и т.п. непосредственно перед террористическим актом») документированы применительно к началу XX века [6] - времени, когда даже в наиболее развитых на тот момент странах мира население все еще в полной мере испытывало страх перед инфекционными болезнями, и когда сложившаяся система средств массовой информации (периодическая печать, телеграфные агентства) уже обеспечивала быстрое и широкое распространение новостей. Свою роль сыграло и стремительное развитие в это время микробиологии, сопровождавшееся ростом числа специалистов в этой области, что создавало риск вовлечения некоторых из них в деятельность террористических организаций. Последнее можно было наблюдать, в частности, на примере Г.А. Гершуни (1870-1908) -

профессионального бактериолога, одного из основателей российской партии социалистов-революционеров и ее Боевой организации [7].

Позже, начиная с 1930-х гг., в продолжение обусловленного Первой мировой войной поиска новых средств ведения боевых действий, в ряде государств были запущены программы по созданию полноценного бактериологического (биологического) оружия (БО), пригодного для широкого применения со стороны вооруженных сил в качестве средства БВ [8-10]. В русле работ, проводившихся в этом направлении, была, в частности выработана соответствующая терминология: «бактериологическая (биологическая) война», «биологический поражающий агент» (БПА) и т.д. К счастью, несмотря на далеко зашедшие (особенно в период и после Второй мировой войны) работы в области создания и способов применения БО, человечество сумело избежать ужасов полномасштабной БВ, важнейшим заслоном перед которой стала международная Конвенция о запрещении разработки, производства и накопления запасов бактериологического (биологического) и токсинного оружия и об их уничтожении (КБТО), открытая для подписания в 1972 и вступившая в силу в 1975 г. [11]. К настоящему времени КБТО подписана и ратифицирована со стороны свыше 100 государств мира. Более того, в связи с несовершенством самой КБТО (прежде всего, в отношении механизма контроля за ее соблюдением), к ней еще в 1990-е гг. был подготовлен и согласован дополнительный протокол, к сожалению, до сих пор не вступивший в силу [12].

В этих условиях место БВ в ряду биологических угроз рукотворного (антропогенного) происхождения вновь занял БТ со стороны террористических групп и организаций, а также террористов-одиночек. Теоретически, нельзя полностью исключить возможность получения ими какой-либо поддержки на государственном уровне, однако вероятность такого шага со стороны правительственных структур абсолютного большинства государств мира крайне мала - даже в отсутствие полноценного механизма контроля за соблюдением КБТО.

Такая оценка вполне подтверждается реальной практикой последних десятилетий. Во всяком случае, единственным относительно

достоверным примером совершения акта БТ с предполагаемым участием государственных органов, должностных и курируемых ими частных лиц стало получившее широкую огласку убийство в 1978 г. в Лондоне болгарского диссидента Г. Маркова, которому неизвестный, подозреваемый в работе на болгарские спецслужбы, ввел капсулу с рицином с помощью специального устройства, скрытого в обычном зонтике. Впрочем, доказать это в судебном порядке так и не удалось, а в 2013 г соответствующее расследование в Болгарии было закрыто за истечением срока давности [13].

Что же касается доказанных или предполагаемых актов БТ со стороны террористических групп, организаций и террористов-одиночек, то за период, истекший с момента подписания и вступления в силу КБТО, такие инциденты имели место неоднократно. Сведения о наиболее показательных из них представлены в табл. 1, составленной на основе ряда открытых публикаций последнего времени [14-19].

Из материалов, представленных в табл. 1, следует несколько выводов:

1. Угроза БТ по-прежнему сохраняет свою актуальность, чему, в свою очередь, способствует все еще широкое распространение представлений о том, что использование патогенов в террористических целях сравнительно доступно и дешево и способно обеспечить не меньший эффект, чем «традиционные» средства поражения.

2. Реально осуществленные или предотвращенные за последние десятилетия акты БТ предполагали использование тех же патогенов, что рассматривались в качестве наиболее перспективных для создания на их основе боевых рецептур для ведения полномасштабной БВ. Критерии отбора таких патогенов по 10-15 основным показателям совпадают, а их перечни, с точки зрения вероятности их применения в зависимости от степени ожидаемого эффекта, приводятся во многих открытых публикациях [4, 5, 19, 20, 22-24]. При составлении подобных перечней широко использовался метод рейтинговой (балльной) оценки, основанной на присвоении тому или иному показателю количественного выражения в виде условных баллов.

Общее число избранных показателей, как и их значимость, в ряде случаев были обоснованы

недостаточно [4,22], что привело к выражению конечной оценки в таких расплывчатых формулировках как «группа наивысшей опасности», «группа высокой опасности», «группа повышенной опасности», и т.д. [22]. Более строгое деление патогенов на группы было выполнено сотрудниками Центров по контролю и профилактике заболеваний (CDC) Министерства здравоохранения США на основе 11 основных показателей, включая заболеваемость, контаги-озность, летальность и др. [24]. Выраженность того или иного показателя, важного для конечной оценки, при этом обозначалась знаками «+» или «—». Например, высокий рейтинг по показателю заболеваемости (++) получал возбудитель, если большинство заболевших нуждалось в госпитализации, тогда как низший (+) - если большинство заболевших могло обойтись поликлиническим лечением. Наивысший рейтинг (+++) возбудитель получал в случае летальности > 50 % от общего числа заболевших, средний - при летальности 21-49 %, меньший (+) -при летальности < 20 %. Итоговый подсчет позволил разделить весь перечень рассматриваемых патогенов на группы A, B и С [24].

Аналогичное деление на группы с балльной оценкой каждого из показателей, включая источник получения патогена, соотношение клинически выраженных и инаппарантных форм течения болезни и др., было осуществлено в РФ. При этом конечные оценки вероятности применения того или иного патогена для совершения актов БТ были выражены в следующих формулировках: «агенты, занимающие устойчивое положение в перечнях» (реальные), «агенты, занимающие неустойчивое положение» (вероятные), «агенты, занимающие случайное положение» (маловероятные, или требующие дополнительной оценки) [5].

К настоящему времени перечень патогенов вирусной природы, использование которых вероятно при осуществлении актов БТ, сокращен до 20 [25], тогда как ранее их число доходило до 40-50. Спектр патогенов бактериальной природы по-прежнему невелик - основными из них остаются возбудители сибирской язвы, чумы, туляремии.

«Совет патриотов Миннесоты» (США, 1991). Нанести ущерб федеральному правительству, осуществить личную месть. Протест против налоговой политики правительства, « патриотиче ско е» движение правого толка. Сотрудники налоговой службы США, сотрудники правоохранительных органов. Рицин, экстрагированный из касторового семени, приобретенного посредством почтового заказа. Нанесение на кожу жертв вместе с диметил-сульфокси-дом и маслом алоэ, или в качестве сухого аэрозоля. В группировку были внедрены агенты ФБР, четыре ключевых участника арестовано.

«Аум Синрикё» (Япония, 1995). Подтвердить апокалиптическое пророчество гуру, устранить противников и соперников, приостановить судебное преследование, установить контроль над правительством Японии. Милленаристский культ из числа «религий Нового Века» («Нью Эйдж»), стремящийся к установлению в Японии теократического режима, во главе с харизматическим властным лидером. Широкие массы населения, индивидуальные противники культа, сотрудники суда и полиции. Биологические (сибирская язва, ботулиниче ский токсин, лихорадка Ку, вирус Эбола) и химические (зарин, VX, синильная кислота) агенты. По меньшей мере 10 попыток распыления БПА в форме аэрозоля, во всех известных случаях, закончившие ся неудачей. Многочисленные атаки с применением отравляющих газов, повлекшие за собой смерть по меньшей мере 20 и поражение свыше 1 ООО человек.

Не установлены, подозревался сотрудник одного из институтов Армии США (США, 2001). Запугать население США, вызвать недоверие к правительству США. Политический экстремизм. Селективное поражение сотрудников СМИ и двух сенаторов от Демократической партии. Споры сибирской язвы (возможно, из коллекции одной из лабораторий Армии США). Аэрогенное поражение в результате вдыхания аэрозоля из почтовых отправлений. На несколько месяцев в стране создана атмосфера страха, раскуплены запасы антибиотиков и противогазов, заболело 22 и умерло 5 человек.

Самата Улла и др. (Великобритания, 2017). ? Радикальный исламизм. ? Споры сибирской язвы. ? Возможная подготовка атаки с использованием спор сибирской язвы предупреждена арестом участников, у одного из которых на карте памяти был обнаружен текст книги под ред. Л. и С. Лютвик о потенциале сибирской язвы как БО.

ществлении актов БТ использовали специалисты Пекинского института биотехнологии. Перечень из примерно 50 патогенов был увязан с возможностью обеспечения безопасности работ с ними и наличием к ним средств биозащиты [26]. В итоге авторы пришли к заключению

0 том, что оценки патогенов из существующих перечней противоречивы, но в целом укладываются в отмеченные выше три основные группы.

3. Степень угрозы БТ к настоящему времени значительно снизилась. Теоретически, такая угроза продолжает существовать, но вероятность реального осуществления террористических биоатак ограничивается целым рядом сдерживающих факторов, к которым относятся, прежде всего, следующие:

• сложности с получением предполагаемыми биотеррористами пригодного для их целей патогена [5, 23,27]. Получить в свое распоряжение вирулентную культуру (например, экзотического вирусного заболевания) достаточно сложно [19,28]. Натуральная оспа, полиомиелит и эпидемический сыпной тиф практически ликвидированы. Изоляты этих нозоформ в ограниченном количестве находятся на хранении в хорошо известных и надежно контролируемых лабораториях. Так, последователи японской религиозной секты «Аум Синрикё» хотя и смогли приобрести штамм сибиреязвенной культуры, однако он оказался не вирулентным, а вакцинным, что привело к провалу предпринятых ими биоатак [16,18];

• сложности с обоснованием выбора нозо-формы и изолята (генотипа) ее возбудителя в интересах достижения цели (целей) предполагаемой биоатаки. Инфекционным заболеваниям присуще наличие инкубационного периода, а также инаппарантных (стертых) форм: от 1 : 20 до 1 : 1 000 в случае японского и от

1 : 60 до 1 : 90 в случае клещевого энцефалита в зависимости от генотипа, от 1 : 4 до 1 : 140 в случае лихорадки Западного Нила;

• сложности с реализацией на практике наиболее эффективного способа применения отобранного или имеющегося в наличии патогена;

• сложности с определением и соблюдением условий совершения биоатаки в зависимости от места, времени, состояния атмосферы, вентиляционных потоков в зданиях и сооружениях

и т.д., не говоря уже о финансовых и организационных затруднениях при подготовке самой атаки;

• сложности с поиском и привлечением к подготовке предполагаемой биоатаки специалистов, способных руководить и (или) вести работы с патогенами вирусной или бактериальной природы, стоящие на грани собственно исследовательской деятельности и требующие соответствующего лабораторного оснащения и опыта;

• отсроченность по времени ожидаемого от атаки эффекта, обусловленная продолжительностью инкубационного периода, характерного для данного заболевания, что в свою очередь, зависит не только от свойств соответствующего возбудителя, но и от гетерогенности иммунологической и возрастной реактивности людей в зоне поражения.

Так, при натуральной оспе срок проявления ожидаемого эффекта может составлять до 20 сут (средние инкубационный период и продромальная стадия) [29], при лихорадке Ку - до 2-3 нед [30], что существенно снижает психологический эффект от биоатаки с применением их возбудителей. Постепенное, плавное утяжеление клинической картины болезни способно еще более отодвинуть по времени срок госпитализации: при лихорадке Ку - в среднем на 9,7 ± 8,0 сут [30]. При заражении человека возбудителем мелиоидоза (B. pseudomallei) инкубационный период в зависимости от величины инфицирующей дозы составляет от 1 до 21 сут, но может достигать и 2 мес, и даже 24-29 лет [31]. Для сапа (возбудитель -B. mallei) также характерен инкубационный период неопределенной продолжительности -от нескольких дней до нескольких недель [32].

• отчетливое снижение уровня страха перед инфекционными заболеваниями, особенно характерное для представителей поколений, родившихся в конце XX - начале XXI в. Огром-

ное значение для формирования этой тенденции имела многолетняя программа расширенной иммунизации населения земного шара, предпринятая по инициативе ВОЗ и осуществляемая на протяжении свыше 30 лет в разных странах мира в рамках соответствующих программ на национальном уровне, что привело к резкому снижению заболеваемости многими ранее смертельно опасными инфекционными болезнями, особенно детскими. Не случайно среди достаточно широких кругов населения, в свое время охваченного иммунизацией, в последнее время наблюдается стойкое неприятие вакцинопрофи-лактики применительно к собственным детям, когда возможные осложнения при вакцинации вызывают больше опасений, чем сами болезни;

зованием таких доступных и не требующих сколько-нибудь сложных навыков средств, как кухонный нож, мачете, легковой или грузовой автомобиль и т.д., применение которых, как оказалось, при минимальных затратах на подготовку самой атаки способно произвести не менее устрашающее воздействие на государственные структуры и общество в целом, чем «традиционные» средства поражения.

Биотерроризм и биологические «атаки» природного происхождения

При оценке степени реальной угрозы со стороны БТ на современном этапе целесообразно сопоставить между собой последствия самой успешной за последнее время террористической биоатаки - «почтовой атаки» со спорами сибирской язвы, предпринятой в США в сентябре-октябре 2001 г. [34], и последствия природной «комариной атаки» с возбудителем лихорадки Западного Нила, имевшей место там же в августе-сентябре 1999 г. [35-37]. В первом случае, несмотря на внезапность самой биоатаки, помноженную на удачный выбор ее целей и способ применения использованного патогена, а также напряженность в стране после террористических актов 11 сентября того же года, правительственные структуры США отреагировали своевременно и квалифицированно: панические настроения в обществе были купированы, вспышка заболевания ликвидирована, соответствующие учреждения получили дополнительное финансирование, а в целом система здравоохранения США не только продемонстрировала высокую устойчивость к подобным угрозам, но и вышла на новый уровень готовности к их отражению в будущем. Напротив, в случае вспышки лихорадки Западного Нила последствия оказались гораздо более серьезными, особенно с течением времени: нозофор-ма вышла за пределы первоначального очага (Нью-Йорк), распространилась по территории США, преодолела границы страны и в настоящее время представляет собой постоянную эпидемиологическую и экологическую угрозу для населения как Северной, так и Южной Америки [35, 37]. Сравнительная оценка последствий террористической «почтовой атаки» 2001 г. и природной «комариной атаки» 1999 г. приведена в табл. 2.

Несчастные случаи и другие чрезвычайные происшествия

Из общего ряда угроз биологического характера, с которыми сталкивается современное человечество, не могут быть исключены также несчастные случаи и другие чрезвычайные происшествия при работе с биологическими материалами в научных исследованиях и на производстве биопрепаратов. Тем более что подобные инциденты с патогенами, присутствующими в перечнях потенциальных БПА, имели место в прошлом неоднократно. К сожалению, эти инциденты, как правило, не становились предметом широкой огласки - даже среди специалистов, что препятствовало своевременному и всестороннему анализу их причин в интересах предотвращения их повторения в будущем.

Тем не менее, по некоторым из инцидентов такого рода эпидемиологическая и клиническая информация все же доступна. Это вспышка венесуэльского энцефаломиелита лошадей (22 заболевших) в 1956 г. в Научно-исследовательском институте вирусологии имени Д.И. Ивановского [38], вспышка геморрагической лихорадки с почечным синдромом (113 заболевших) в 1961 г. в Научно-исследовательском институте эпидемиологии и микробиологии имени Н.Ф. Гамалеи [39], позднейшие одиночные случаи заболевания (случайные уколы иглой, кон-таминированной жизнеспособными вирусами Марбург и Эбола) в Государственном научном центре «Вектор» и Вирусологическом центре Института микробиологии Министерства Обороны РФ [40]. Аналогичные инциденты имели место и за рубежом - в частности при изучении бруцеллеза в микробиологической лаборатории в Анкаре (10 заболевших) [41].

Скрываемые в свое время вспышки натуральной оспы в 1971 г. в Аральске (10 заболевших, 3 умерших) [42], острого бруцеллеза в 1976 г. в Москве (304 или даже 465 заболевших) [43] и сибирской язвы в 1979 г. в Свердловске (65 умерших) [ 44, 45] стали достоянием гласности с большим опозданием, избежав своевременного и полноценного расследования.

Нозокомиальные заражения

К числу биологических угроз, известных в прошлом и настоящем и неизбежных в будущем, безусловно, относятся и инциденты,

обусловленные нозокомиальными (внутри-больничными) заражениями. Опасность с их стороны носит постоянный характер и будет сохраняться до тех пор, пока существуют сами инфекционные болезни и человек. Эпидемиологически, такие инциденты, в большинстве случаев, следуют за постановкой ошибочного диагноза у первичного больного, поступившего в лечебный стационар. Ошибка с диагнозом у первичного больного приводит к появлению все новых и новых больных, образуя цепочки от нескольких до десятков и даже сотен случаев. Так, во время вспышки натуральной оспы в 1960 г. в Москве число заболевших достигло 45 человек, в 1970 г в ФРГ - 20 человек, в 1972 г в Югославии - 175 человек. Во время двух вспышек лихорадки Эбола в 1976 г. в Южном Судане в одном случае (Нзара, июль-октябрь) заболело 34 человека, в другом (Мариди, август-ноябрь) - 213 человек. При практически одновременной (сентябрь-октябрь 1976 г.) вспышке лихорадки Эбола в Заире (Ямбуку) - 318 человек. Тяжелый острый респираторный синдром (ТОРС/SARS) при вспышке в 2003 г. в Канаде (Торонто) охватил около 345 человек, при вспышке в том же году на Тайване - 668 человек.

Заключение

Подводя итоги, можно заключить, что из всех рассмотренных выше биологических угроз, включая угрозу БТ, наибольшую опасность представляют собой все же не антропогенные (рукотворные), но естественные (природные) -в виде вспышек неизвестных ранее или неэндемичных для данного региона инфекционных заболеваний. Опыт показывает, что последствия природных биологических «атак» могут принимать весьма впечатляющие масштабы, примеры чего приводятся в табл. 3.

Микромир постоянно эволюционирует в дикой природе, особенно в регионах тропического пояса земного шара [46-50]. Вторжение в нее человека сопровождается изменением ее региональных экосистем, миграциями птиц и животных, что неизбежно ставит человеческую популяцию перед угрозой со стороны вновь возникающих или повторно возникающих инфекционных заболеваний (англ.

emerging and re-emerging diseases). Эта угроза всегда неожиданна и практически непредсказуема [46, 47, 50-53].

Столь же непредсказуема, но реальна и угроза вспышек инфекционных заболеваний, обусловленная нозокомиальными заражениями — как и чрезвычайными происшествиями при работе с патогенами в научно-исследовательских учреждениях и на производстве биопрепаратов. Неосторожное движение экспериментатора, неожиданная реакция подопытного животного, усталость оператора или конструкции ферментера (прокладок, трубопроводов и т.д.), прорыв фильтра воздушной очистки - все это может стать причиной очередного инцидента.

Что же касается угрозы актов БТ, то они, как представляется, по-прежнему не исключены в будущем, хотя и маловероятны. Тем не менее, готовность к предотвращению и ликвидации последствий возможных актов БТ, как и готовность к парированию всех других угроз биологического характера, безусловно требует подготовки и совершенствования квалификации специалистов в этой и смежных областях (инфекционистов, эпидемиологов, молекулярных биологов и т.п.), постоянного и широкого международного сотрудничества по линии ВОЗ и других международных организаций, последовательной и системной работы по иммунизации населения и пропаганде в средствах массовой информации ее важности и достигнутых результатов.

The threat of bioterrorism is one of the forms of terroristic threat in general, despite the fact that there is an undoubted proximity between the issues of bioterrorism and biological warfare, as in both cases pathogens are the means of the destruction. Since international Convention on the prohibition of the development, production and stockpiling of bacteriological (biological) and toxin weapons and on their destruction has been signed (1972) and came into force (1975), the threat offull-scale biological war can be considered to be eliminated. The acts of bioterrorism in the future are not excluded, although they are unlikely. The threat due to accidents and emergencies while working with pathogens in the research process and during the production of biologic products, as well as the threat of nosocomial infections, remain to be more urgent. However, not anthropogenic, but natural biological threats, in the form of outbreaks of infectious diseases, previously unknown or non-endemic for some particular region, are the greatest danger to the modern mankind.

Terrorism

Terrorism, without any exaggeration, represents one of the most serious challenges the mankind faces with at the present stage of its development.

From the very term "terrorism" (derived from a Latin word terror - "fear", "horror" [1]) it is clear that the meaning of the latter one as of a public phenomenon is in conscious and purposeful intimidation of individual persons and public groups, society as a whole, heads of the state institutions and the states with the purpose to force them to perform some requirements or to direct their actions to the channel corresponding to the interests of the final

beneficiaries and direct customers of the terrorism act.

Unfortunately, there does not exist any standard international interpretation of the term "terrorism". So, in different periods of time and under different circumstances it was applied and is still being applied to designate both "frankly criminal elements made the capture of hostages", and "participants of various separatist and national-liberation movements, revolutionaries calling for violent overthrow of the authorities, members of radical religious organizations struggling for domination in a society or region", etc. [2].

The current Russian legislation defines terrorism as "ideology of violence and influence practice on the decision-making by public authorities, local governments or international organizations connected with the population intimidation and (or) other forms of illegal violent acts" [3]. Accordingly, the act of terrorism in the Russian legislation is defined as "fulfilment of explosion, arson or other actions frightening the population and creating danger of a person destruction, considerable property damage, or of other severe consequences, with the aim of either destabilization of authorities activity or international organizations, or influence on the decision acceptance made by them, as well as a threat of the mentioned above actions with the same purposes" [4].

Bioterrorism

One of terrorism variants is biological terrorism (BT) which is supposed to be a threat implementation of a deliberate, conscious and purposeful use of pathogenic microorganisms towards some address - instead of explosives and other means of killing applied in "traditional" terrorism acts. The motives of BT acts as well as in the case of the latter ones can be absolutely different - from the intentions to influence the present political situation in some state or region to aspiration to establish another political or religious leadership. BT acts, like "traditional" terrorism acts, can be carried out by both independent persons or groups of terrorists, and whole organizations.

There is a doubtless proximity between the problems of BT and biological warfare (BW). The latter is characterized as a wide-scale, planned in advance application of infectious disease agents (pathogens) and products of their life (toxins) as the means to damage some population of people or its part with the purpose to deprive or weaken their fighting or working capacity, to disorganize army and economy management, and also the system of medical maintenance that is in general aimed to achieve some strategic targets. On the contrary, BT actions are directed against separate persons or groups of people and basically have the purpose of deterrence and blackmail, not only of the infected people and groups, but also of those who surround them not being a direct object of the attack [5].

The first authentic data on BT acts planning ("to apply distribution of infectious bacteria: consumption, tetanus, diphtheria, typhoid fever, etc. directly before the act of terrorism in case of poison absence to poison bullets") were documented at the beginning of the 20th century [6] - the time when even in the most developed countries of the world the population was still afraid of infectious diseases and when the developed mass-media system (periodicals, telegraph agencies) could already provide fast and wide spread of news circulation. Rapid microbiology development of that period accompanied by the growth of experts number in this area that created a risk of their involvement in the activity of terrorist organizations also had a significant role. This could be observed, in particular, by way of G.A.Gershuni (1870-1908) example - a professional bacteriologist, one of the founders of the Russian party of socialists-revolutionaries and its Fighting organization [7].

Later, since 1930-es, to continue the search of new means of conducting combat operations caused by the First World War, some states developed the programs to create full-fledged bacteriological (biological) weapons (BWs) suitable for wide application within armed forces as BW means [8- 10]. The studies made in this sphere developed, for example, corresponding terminology: "bacteriological (biological) war", "biological agent (BA)", etc. Fortunately, in spite of a thorough work in the field of BWs creation and application (especially during and after the Second World War), the mankind managed to avoid BW horrors due to the main barrier in the form of the international Convention on the prohibition of the development, production and stockpiling of bacteriological (biological) and toxin weapons and on their destruction (BTWC) opened for signing in 1972 and come into force in 1975 [11]. By this time BTWC has been signed and ratified by over 100 states of the world. Moreover, in connection with BTWC imperfection (first of all, concerning the mechanism of observance control) there was developed an additional protocol in 1990-es that did not come into force [12].

In these conditions BW place among biological threats of man-made (anthropogenic) origin was again occupied by BT of terrorist groups and organizations, as well as separate terrorists. Theoretically, it is impossible to exclude completely the

possibility of their support at the state level, however, the probability of such steps from the government bodies of the vast majority of the world states is quite small - even taking into account the lack of reliable mechanism of BTWC observance control.

Such estimation is proven by the real practice of the last decades. Anyway, the only relevant example of BT acts with supposed participation of government bodies, officials and private persons coordinated by them was a high-profile murder of the Bulgarian dissident G.Markov in 1978 in London when an unknown person suspected to work for the Bulgarian special service injected a ricin capsule by a special device hidden in an ordinary umbrella. However, it was not proven juridically, and in 2013 a corresponding inquiry in Bulgaria was stopped due to lapse of time [13].

As to proven or suspected BT acts made by terrorist groups, organizations and separate terrorists within the period from the moment of BTWC signing and coming into force, these incidents took place repeatedly. The data on most significant ones are presented in Table 1 composed on the basis of some recent open access publications [14-19].

On the basis of the materials presented in Table 1 there can be made some conclusions.

1. BT threat is still an urgent challenge that is promoted by a wide-spread ideas that pathogens use for terrorist purposes is rather available and cheap and is able to provide the effect comparable with "traditional" weapons.

2. The real or prevented BT actions of the last decades intended to use the same pathogens that were considered to be the most perspective for the creation of fighting formulations to conduct a full-scale BW. The inclusive criteria of these pathogens have the same 10-15 basic indicators, and their lists describing the probability of their application depending on the expected effect level, are available in many open access publications [4, 5, 19, 20-24]. The method of rating (point-marking) estimation based on giving to the indicator some quantitative form in conditional points was widely used in the development of these lists.

In some cases the total number of the selected indicators, as well as their significance, were insufficiently proven [4, 22] that led to such vague definitions as "group of the highest danger", "group of high danger", "group of increased

danger", etc. [22]. A more strict classification of pathogens into the groups was made by the employees of the Centers for disease control (CDC) of the Ministry of Health of the USA on the basis of 11 basic indicators, including morbidity, contagiousness, mortality, etc. [24]. Intensity of this or that indicator, significant for a final estimation, was marked by the signs «+» or «-». For example, a high rating of the disease morbidity (++) was given to the indicator if the majority of the infected people were to be hospitalized whereas the lowest one (+) - if the majority of the infected people could manage polyclinic treatment. The highest rating (+++) was given to the indicator in case of >50% mortality from the total number of the infected people, the average one - in case of 21-49% mortality, the lowest one (+) - in case of <20% mortality. The total calculation allowed to divide the whole list of considered pathogens into groups A, B and C [24].

A similar division to the groups with a point-marking estimation of every indicator, including a pathogen source, correlation of clinically expressed and inapparent forms of a clinical course, etc., was carried out in the Russian Federation. The final estimations of the probability to use a pathogen for BT actions were given by the following definitions: "agents with a stable list position" (real), "agents with unstable list position" (probable), "agents with a casual list position" (improbable or requiring an extra estimation) [5].

By the present period of time the list of pathogens with viral origin which are probable to be used for BT actions has been reduced to 20 items [25] whereas earlier their number amounted to 40-50. The spectrum of pathogens with bacterial nature is still insignificant - the basic of them are anthrax, plague, and tularemia agents.

An unusual approach to estimate the probability of a pathogen application for BT actions was used by the experts of Peking institute of biotechnology. The list of approximately 50 pathogens was coordinated with the possibility of safe work with them and the presence of bioprotection means availability [26]. The authors made the conclusion that estimations of pathogens included in the existing lists were inconsistent, but in general corresponded to three basic groups mentioned above.

3. The level of BT threat has considerably decreased by the present period of time. Theoretically, such threat continues to exist, but the probability of actual terrorist biological attacks is limited by a variety of deterrents:

1) Problems for bioterrorists to get a suitable pathogen [5, 23,27]. It is quite difficult to obtain a virulent culture (for example, of an exotic viral disease) [19, 28]. Smallpox, poliomyelitis and epidemic typhus have been practically eliminated. Limited number of the isolates of these nosoforms are stored in well-known and reliably controlled laboratories. So, though the followers of a Japanese religious sect "Aum Shinrikyo" managed to get an anthrax culture strain, it was not a virulent sample but a vaccinal one that led to their bioattacks failure [16, 18].

2) Problems in explaining the choice of agent nosoform and isolate (genotype) to achieve the target (targets) of prospective bioattack. Infectious diseases are characterized by some incubation period and inapparent (causing no noticeable signs or symptoms) forms: from 1:20 to 1:1000 in case of Japanese and from 1:60 to 1:90 in case of tick-borne encephalitis depending on the genotype, from 1:4 to 1:140 in case of West Nile fever.

3) Problems with practical implementation of the most effective way of selected or available pathogen application.

4) Problems with determination and observance of bioattack conditions depending on the place, time, atmosphere condition, ventilation air flows in the buildings and constructions, etc., without mentioning financial and organizational difficulties to prepare for the attack.

5) Problems with search and involvement in the preparation of prospective bioattack of the experts capable to manage and (or) to conduct works with pathogens of viral or bacterial nature, to carry out independent research activity requiring appropriate laboratory equipment and experience.

6) Time delay of the expected effect after the attack caused by the incubation period duration characteristic for the disease that, in its turn, depends not only on the agent features, but also on immunologic heterogeneity and age reactivity in the affected zone population.

So, in case of smallpox the time of expected effect manifestation can take up to 20 days (average

incubation period and prodromal stage) [29], in case of Q fever - up to 2-3 weeks [30] that essentially reduces psychological effect caused by the bioattack with these agents application. Gradual, stepless aggravation of clinical presentation of the disease can produce a longer delay of hospitaliza-tion time: in case of Q fever it is on the average 9,7±8,0 days [30]. In case of pseudocholera agent infection (B. pseudomallei) the incubation period depending on the amount of infecting dose is from 1 to 21 days, but can last for 2 months, and even 24-29 years [31]. Glanders (agent - B. mallei) is also characterized by the incubation period of uncertain duration - from some days to some weeks [32].

7) The danger of retrograde effect on the people directly working with the "material" for prospective bioattack [26]. The work with a filovirus infection agent (Marburg fever, Ebola fever), Lassa fever, equine encephalomyelitis, some types of ricketsiosis requires both professional skills and laboratory equipment of BSL-4 biosafety level. Otherwise, the work performers are threatened by danger to get infected and to infect the people around them, and in case of the work with filovi-rus infection agents - to die earlier than the work is completed.

8) Practical introduction of modern methods of verification of fever diseases with unclear clinical picture (enzymoimmunoassay, polymerase chain reaction) that twice reduced the time of diagnosis making in clinical practice, and up to two hours in endemic area. This gave the possibilities for simultaneous differentiation of up to 9 and even more rare viral haemorrhagic fevers which agents are constantly presented in well-known lists of potential BA [33].

9) Distinct decrease of the level of infectious diseases fear especially characteristic for the generations born at the end of the 20th century - the beginning of the 21st century. A huge value to form this tendency is given to a long-term program of a wide-spread immunization of the world population launched by WHO initiative and carried out for over 30 years in different countries of the world within the frames of corresponding national programs that led to a dramatic decrease in mortality caused by previously fatal diseases, especially pe-diatric ones. lation groups widely involved in the immunization programs in due time period there can be observed a stable refuse to use vaccinal prevention for their children explained by the fact that possible complications after vaccination are more dangerous to compare with the disease itself.

10) Recently spread occurrence (at first in Israel, and later in other countries of the world) of terrorist actions with the use of available and not requiring any specific skills items such as a kitchen knife, machete, car or lorry, etc. which application at the minimum expenses for the attack preparation can produce an influential effect that is comparable with "traditional" means used to frighten the state structures and the society in general.

Bioterrorism and biological "attacks" of natural origin

To evaluate the degree of present actual threat due to BT it seems to be reasonable to compare the consequences of the most successful recent terrorist bioattack - "post attack" with anthrax spores initiated in the USA in September-October, 2001 [34], and the consequences of natural "mosquito attack" with West Nile fever agent performed in the same place in August-September, 1999 [35-37]. In the first case, despite the suddenness of the bioattack multiplied by a successful choice of its purposes and the way of the pathogen application as well as the stressful atmosphere in the country after the terrorism acts on September, 11 of the same year, the governmental structures of the USA reacted in due time and skillfully: the panic moods in the society were stopped, the disease outbreak was eliminated, the corresponding institutions received extra financing, and the whole system of public health service of the USA demonstrated not only a high stability to similar threats, but also achieved a new level of readiness to repel attacks in the future. On the contrary, in case of West Nile fever outbreak the consequences were much more complicated, especially in the course of time: the nosoform broke the limits of the initial centre (New York), was transmitted to the territory of the USA, overcame the borders of the country and is now a constant epidemiological and ecological threat for the population of North and South America [35, 37]. A comparative assessment of the consequences of terrorist "post attack" of 2001 and natural "mosquito attack" of 1999 is given in Table 2.

Accidents and other emergencies

Accidents and other emergencies happening during the research work with biological materials and manufacture of biological products cannot be excluded from the general threats of biological character which the modern mankind faces with. Such incidents with pathogens included in the lists of potential BA had quite a regular character in the past. Unfortunately, these incidents, as a rule, did not become a subject of wide publicity - even among the experts that interfered with the timely and detailed analysis of their reasons to prevent their repetition in the future.

Nevertheless, some epidemiological and clinical information on such incidents is available. This is an outbreak of Venezuelan equine encephalomyelitis (22 infected people) in 1956 at Scientific research institute of virology named after D.I.Ivanovsky [38], an outbreak of hemorrhagic fever with nephrotic syndrome (113 infected people) in 1961 at Scientific research institute of epidemiology and microbiology named after N.F.Gamaleya [39], the latest single cases (casual pricks by a needle contaminated by viable Marburg and Ebola viruses) at the State scientific centre "Vector" and Virology centre of the Institute of microbiology of the Ministry of Defense of the Russian Federation [40]. Some similar incidents were registered abroad - in particular, during the study of brucellosis at microbiological laboratory in Ankara (10 infected people) [41].

Concealed outbreaks of smallpox in 1971 in Aralsk (10 infected people, 3 patients died) [42], acute brucellosis in 1976 in Moscow (304 or even 465 infected people) [43], and anthrax in 1979 in Sverdlovsk (65 people died) [44, 45] became available for public with a big delay and avoided the timely and detailed investigation.

Nosocomial infections

Incidents caused by nosocomial (hospital-acquired) infections are considered to be the biological threats well-known in the past and the present and inevitable in the future. Their danger has a constant character and will remain until there exist both infectious diseases and people. There were developed the programs of corresponding studies and trainings, t he corresponding classes were delivered. 1. In 2003 2866 people had the disease, 264 people died (mortality - 9,2%). 2. Within the following 5 years the virus was introduced in 40 states of the USA, Canada, Mexico and Latin America. By 2007 there were registered 16706 cases of the disease in the USA, 7096 of them were of neuroinvasive form. 3. The ecological balance of local fauna was broken, there was a constant threat of destruction for 27 species of birds, horses and other types of animals. 4. There was an additional loading for the institutions of public health service, veterinary supervision and nature protection. An extra number of the devices for artificial lung ventilation were necessary.

General measures of counteractions Increase of vigilance and knowledge among the workers of the institutions of public health service, law enforcement bodies and doctors of "the first line". Organization of constant epidemiological and epizootological supervision, training of experts of the corresponding profile (clinical physicians, epidemiologists, etc.), further study of the infection.

to the primary patient leads to the appearance of new patients forming the chains from several people to dozens and even hundreds of cases. So, during an outbreak of smallpox in 1960 in Moscow the number of the infected people achieved 45 patients, in 1970 in Germany - 20 people, in 1972 in Yugoslavia - 175 people. During two outbreaks of Ebola fever in 1976 in South Sudan in the first case (Nzara, July-October) there were 34 infected people, and in the second one (Maridi, August-November) - 213 infected people. In case of almost simultaneous (September-October, 1976) outbreak of Ebola fever in Zaire (Yambuku) there were infected 318 people. An outbreak of severe acute respiratory syndrome (TOPC/SARS) in 2003 in Canada (Toronto) infected about 345 people, and 668 people in Taiwan.

Conclusion

Summing up, it is possible to conclude that from all considered above biological threats, including BT threat, the greatest danger is produced not by anthropogenous (man-made) threats, but by natural ones - in the form of outbreaks of infectious diseases unknown earlier or non-endemic for the region. The experience demonstrates that consequences of natural biological "attacks" can have rather impressive coverage, the examples of that are presented in Table 3.

threat of emerging or re-emerging infectious diseases. This threat is always unexpected and almost unpredictable [46, 50-53].

The threat of outbreaks of infectious diseases caused by nosocomial infections is unpredictable in the same way as the emergencies happening during the work with pathogens at research institutions and manufacture of biological products. Careless movement of an experimenter, unexpected reaction of an experimental animal, weariness of an operator or a fermenter design (linings, pipelines, etc.), break of the air clearing filter - all of these can become a reason of the next incident.

As to the threat of BT actions, they are still not excluded in the future though are quite unlikely. Nevertheless, the readiness for prevention and liquidation of the consequences of possible BT actions, as well as the readiness for parrying of all other threats of biological character, certainly demands preparation and improvement of expert qualification in this and adjoining areas (specialists in infectious diseases, epidemiologists, molecular biologists, etc.), constant and wide international cooperation within the line of WHO and other international organizations, consecutive and systematic work on population immunization and the propagation of its importance and the achieved results in mass-media.
