Длительно заживающие, хронические раны являются нерешенной проблемой современной медицины. Важнейшую роль в патогенезе таких ран при старении, диабете, многих патологических состояниях играет избыточный окислительный стресс. Обзор посвящен роли митохондрий в этом стрессе и перспективам использования новых митохондриально-направленных антиоксидантов для терапии труднозаживающих ран. Недавние исследования на старых мышах и мышах с диабетом 2-го типа показали, что отечественный митохондриально-направленный антиоксидант SkQ1 [10-(6'-пластохинонил) децилтрифенилфосфония] стимулирует заживление полнослойных кожных ран. Прием SkQ1 ускоряет протекание воспалительной фазы заживления, созревание грануляционной ткани, ангиогенез и эпителизацию ран. Противовоспалительное действие SkQ1, возможно, связано со снижением воспалительной активации эндотелия сосудов, которая характерна для старения, диабета и других патологий. Локальное введение SkQ1 также ускоряет заживление ран и имеет выраженное противовоспалительное действие при экспериментальном моделировании острого асептического воспаления. Кроме того, SkQ1 стимулирует апоптоз нейтрофилов и подавляет их активацию, а также угнетает воспалительную активность тучных клеток. В условиях моделирования раневого процесса in vitro SkQ1 ускоряет движение эпителиоцитов и фибробластов в «рану» и стимулирует миофибробластную дифференцировку подкожных фибробластов человека. Можно полагать, что препараты для локальной терапии труднозаживающих ран на основе SkQ1 послужат эффективным средством борьбы с длительно незаживающими ранами, в том числе — у пациентов с хроническими критическими состояниями.

Prospects for Drugs Based on the Mitochondria-Targeting Antioxidant SkQ1 in Treatment of Wounds with Impaired Healing

Chronic wounds with impared wound healing that require prolong time for healing remain unsolved problem of modern medicine. Excessive oxidative stress plays an important role in the pathogenesis of chronic wounds caused by aging, diabetes and other pathologies. This review is aimed at the role of mitochondria in oxidative stress and to the future prospects for using the innovative mitochondria targeted antioxidants for treatment of impaired wounds. Recent studies in old mice and mice with type 2 diabetes showed that the mitochondrial antioxidant SkQ1 [10(6'plastoquinonyl) decyltriphenylphosphonium] stimulates healing of full-thickness dermal wounds. SkQ1 accelerates inflammatory stage of wound healing, maturation of granulation tissue, angiogenesis and epithelization of wounds. The anti-inflammatory effect of SkQ1 is possibly connected to decreased inflammatory activation of the vascular endothelium, which is typical for aging, diabetes and other pathologies. Local administration of SkQ1 also accelerates wound healing and provides strong anti-inflammatory effect in the model of acute aseptic inflammation. In addition, SkQ1 to stimulate apoptosis of neutrophils and suppresses their activation, as well as suppresses inflammatory activation of mast cells. In the wound model in vitro, SkQ1 accelerates movement of epithelial cells and fibroblasts into the «wound» and stimulates differentiation of human subcutaneous fibroblasts to myofibroblasts. Reviewed data suggest that SkQ1-based topical drugs have a great potential to treat wounds that exhibit impaired healing also in patients suffering from chronic critical illness.

Введение

Длительно незаживающие или хронические раны остаются существенной медицинской проблемой. Настоящий обзор сконцентрирован на анализе значения активных форм кислорода (АФК) в патогенезе хронических ран, участия митохондрий в генерации АФК и окислительном стрессе, а также на перспективах применения нового класса соединений — митохондриально-направленных анти-оксидантов. Основная цель обзора — подробно обсудить уникальные результаты, полученные авторами данного обзора, по (а) действию мито-хондриально-направленных антиоксидантов на заживление ран в моделях с использованием лабораторных животных in vivo и (б) изучению механизмов действия митохондриально-адресованных антиоксидантов с использованием подходов in vitro.

Заживление ран является важнейшей комплексной реакцией организма на повреждения. В этом процессе участвуют многие типы клеток, в том числе лейкоциты, эпителиоциты, фибробласты и макрофаги, а также многочисленные гуморальные факторы, вырабатываемые этими клетками [1]. Успешное заживление ран достигается с помощью четырех четко запрограммированных фаз: гемостаза, воспаления, пролиферации и ремоде-линга. Эти фазы частично перекрываются, но для осуществления нормального ранозаживления, каждая из этих фаз должна начинаться и заканчиваться в определенное время [1].

Гемостаз заключается в вазоконстрикции и образовании фибринового сгустка, однако вскоре наступает процесс активной вазодилатации, который сопровождается увеличением капиллярной проницаемости [2]. Ключевым медиатором, обеспечивающим этот процесс, является гистамин, выделяемый тканевыми тучными клетками (ТК) [3]. Таким образом начинается фаза воспаления: в рану последовательно мигрируют нейтрофилы, затем моноциты, тканевые макрофаги и лимфоциты. Нейтрофилы выделяют большое количество

Introduction

Long-term non-healing or chronic wounds remain one of the urgent medical problems. This review does not pretend to fully analyze all aspects of the pathogenesis of wound healing. Rather, the review is focused most on the analysis of the significance of active forms of oxygen (ROS) in the pathogenesis of chronic wounds, the involvement of mitochondria in the generation of ROS and oxidative stress, and also on the prospects of using a new class of compounds, mitochondria-targeted antioxidant drugs (MAD). The main objective of the review is to discuss in detail the unique results obtained by the authors of this review on (a) evaluating the effects of MAD on wound healing in in vivo models using laboratory animals and (b) studying the mechanisms of action of MAD in vitro.

Wound healing includes complex reaction of an organism to injuries. Various types of cells contribute to this process, including leukocytes, epithelial cells, fi-broblasts and macrophages, as well as numerous cy-tokines produced by these cells [1]. Successful wound healing is achieved through four finely defined programmed phases: hemostasis, inflammation, proliferation and remodeling. These phases partially overlap, but each phase should begin and terminate at a certain time. Wound healing can be impaired by many different factors acting on one or more phases in this process [1].

Wound healing begins with hemostasis (blood clotting). After the injury, vasoconstriction and formation of a fibrin clot contribute to stopping the bleeding [2]. Soon, however, a process of active vasodi-lation begins, accompanied by an increase in capillary permeability [2]. The key mediator of increased permeability is histamine liberating from mast cells [3]. After the hemorrhage stops the inflammatory phase begins: neutrophils, then monocytes, then tissue macrophages and lymphocytes are subsequently migrated into the wound. The main function of neu-trophils is to destroy pathogens and damaged cells. Neutrophils produce a large number of cytokines, proteases and reactive oxygen species (ROS), which not

Макрофаги на ранних стадиях синтезируют цитокины, которые усиливают воспалительный ответ и дополнительно привлекают лейкоциты к ране. На более поздних стадиях макрофаги способствуют разрешению воспаления, очищая рану от апоптотических клеток, в первую очередь-нейтрофилов [5].

Пролиферативная фаза заключается в реэпи-телизации, при которой происходит пролиферация и миграция клеток эпителия, а также в формировании грануляционной ткани. Первостепенную роль в формировании грануляционной ткани играют фибробласты, синтезирующие первичный внеклеточный матрикс [5]. В построении и последующем ремоделинге грануляционной ткани и формировании рубца важную роль играет диффе-ренцировка фибробластов в миофибробласты [6]. Миофибробласты обладают повышенной способностью к сокращению, что способствует стягиванию краев раны, а также секретируют ферменты, расщепляющие первичный внеклеточный матрикс (такие как металлопротеазы внеклеточного мат-рикса) и интенсивно синтезируют новый внеклеточный матрикс, волокна которого более упорядочены. Важную роль в заживлении играет прорастание сосудов в грануляционную ткань [7]. Этот процесс происходит главным образом благодаря пролиферации и миграции эндотелиальных клеток, которые активируются гипоксией и про-ангиогенными факторами, такими как фактор роста эндотелия (VEGF) [7].

На последней стадии ранозаживления происходит ремоделирование рубца. Происходит частичный лизис незрелых неправильно ориентированных и избыточных коллагеновых волокон под действием матриксных металлопротеаз, выделяемых макрофагами, фибробластами и эндотели-альными клетками, и постепенное их замещение более толстыми фибриллами [3].

Труднозаживающие раны и окислительный стресс

Хронические раны характеризуются нарушением нормальной смены стадий ранозаживления. Этиология хронических ран разнообразна, но подавляющее большинство из них связано с сосудистой недостаточностью, диабетом и старческими изменениями [8]. Безусловно, основным подходом для лечения подобных ран является устранение соответствующих повреждающих факторов, однако этот вопрос не является целью настоящего обзора. Несмотря на различную этиологию, нарушение заживления ран происходит по общему сценарию. Для них характерно удлинение воспалительной фазы, продолжительное персистирование нейтро-

only destroy pathogens but induce additional tissue damage [4]. Macrophages perform several functions in the process of wound healing. In the early stages they synthesize cytokines that enhance the inflammatory response and attract additional white blood cells to the wound. Later, macrophages contribute to resolving the inflammation, clearing the wound from apop-totic cells, primarily neutrophils [5].

Proliferative phase partially overlaps with inflammatory phase and includes (a) re-epithelialization where the proliferation and migration of epithelial cells occurs, and (b) formation of granulation tissue. The primary function in the formation of granulation tissue is fulfilled by fibroblasts, which synthesize the primary extracellular matrix, which consists of collagen, glycosaminoglycans and other proteoglycans. Differentiation of fibroblasts into myofibroblasts is necessary for the construction and subsequent remodeling of granulation tissue and scar formation [5]. My-ofibroblasts have an increased ability to contract which helps to tighten the edges of the wound and secrete enzymes that break down the primary extracellular matrix (such as extracellular matrix metalloproteases, MMPs) and synthesize extensively a new extracellular matrix with fibers more ordered and composed of collagen, fibronectin and other pro-teoglycans [6]. Infiltration of growing vessels within the granulation tissue seems to be essentially required for wound healing [7]. Migration and proliferation of endothelial precursor cells and endothelial cells activated by hypoxia and proangiogenic factors, such as endothelial growth factor (VEGF), significantly contribute to this stage of healing [7].

At the last phase of wound healing remodeling occurs. Remodeling includes partial lysis of imma-tured, wrongly oriented and excessive collagen fibers due to MMPs excreted by macrophages, fibroblasts and endothelial cells, with gradual (1—2 years) replacement by thicker fibers [3].

Impaired Wound Healing and Oxidative Stress

Chronic wounds are characterized by altered changes of wound healing phases. The etiology of chronic wounds is diverse but the vast majority is associated with insufficiency of blood vessels, diabetes and age-related changes [8]. Elimination of factors of alteration are considered as the main approach to chronic wounds treatment, however, this problem remains out of scope of this review. Despite different etiology, the wound healing disorders occur in accordance to common scenario. Lengthening of the inflammatory phase, prolonged persistence of neu-trophils, slowing down of granulation tissue formation, angiogenesis and epithelization remain the main patterns typical for delayed wound healing [9].

Venous insufficiency remains one of the most common causes of chronic wound appearance [10]. Al-

Артериальная недостаточность вызывается, как правило, эндотелиальной дисфункцией, сопровождающей диабет или иные патологии, и также может приводить к появлению ишемизированных незаживающих ран [11].

Возраст является важным фактором риска нарушения ранозаживления. Старение практически всегда сопровождается окислительным стрессом и изменениями в иммунной системе. С возрастом каждая фаза ранозаживления претерпевает характерные изменения: происходит повышение секреции воспалительных медиаторов, задержка инфильтрации макрофагов и лимфоцитов, нарушение фагоцитоза, снижение секреции факторов роста, отсроченный ангиогенез и реэпителизация, уменьшение скорости ремоделинга коллагена [12].

Важно отметить, что как гипоксия, так и гипероксия, приводят к повышению уровня АФК [13]. Особенно высокий уровень АФК вызывается повторяющейся ишемией-реперфузией, характерной для пролежней.

Согласно последним данным, одной из основных причин возникновения труднозаживающих ран является хронический окислительный стресс, при котором уровень АФК остается высоким в течение продолжительного времени [14]. Окислительный стресс развивается из-за действия целого ряда факторов, характерных для большинства хронических ран: бактериального осеменения, локальной гипоксии тканей и старческих изменений [1, 15, 16].

Для нормального ранозаживления требуется тонкий баланс между положительной ролью АФК и их потенциально деструктивным действием. В тканях раневой области происходит активное образование АФК, которые выполняют как бактерицидную, так и регуляторную функцию [14, 17-19]. Концентрация пероксида водорода в раневом экссудате на фазе воспаления достигает сотен микромолей на литр и затем постепенно снижается [17]. В фазе регенерации АФК в низких концентрациях стимулируют образование грануляционной ткани и ангиогенез [20].

После повреждения кожи микроорганизмы, находящиеся на ее поверхности, неизбежно попадают в ткани организма. Уничтожение микроорганизмов происходит на стадии воспаления, на которой используется богатый арсенал системы врожденного иммунитета. Однако, при недостаточно эффективной деконтаминации, воспалительная фаза продолжается, так как оставшиеся микроорга-

Arterial insufficiency is due to endothelial disfunction commonly associated with diabetes and other pathologies. It also might lead to appearance of ischemic wiuds with impaired healing [11].

Age of the patient belongs to one of key risk factors of impairment of healing process. Aging almost always is associated with increased oxidative stress and multiple alterations of immune system. With age, each phase of wound healing undergoes specific changes: an increase in the secretion of inflammatory mediators, a delay in the infiltration of macrophages and lymphocytes, alteration of phagocytosis, decreasing the secretion of growth factors, delayed angiogenesis and re-epithelization, and diminishing the rate of remodeling of collagen [12].

It is important to note that both hypoxia and hy-peroxia lead to an increase in the level of ROS [13]. A particularly high level of ROS is caused by repeated ischemia-reperfusion that is a feature of bed sores.

According to recent data one of the main causes of this problem is chronic oxidative stress with high ROS level sustained for a long period [14]. Oxidative stress develops due to the action of a number of factors typical for the majority of chronic wounds: bacterial insemination, local tissue hypoxia and senile changes [1, 15, 16].

Normal wound healing requires a delicate balance between the positive role of ROS and potentially destructive effect of ROS. In tissues surrounding the wound region, active formation of ROS occurs, which perform both bactericidal and regulatory functions [14, 17—19]. The concentration of hydrogen peroxide in the wound exudate during the inflammatory phase reaches hundreds of micromoles and then gradually decreases [17]. In the regeneration phase, ROS at low concentrations stimulates formation of granulation tissue and angiogenesis [20].

After the skin damage microorganisms located on its surface inevitably penetrate into inner tissues. Destruction of microorganisms occurs at the inflammatory phases using a vast arsenal of innate immunity system. However, when decontamination is not successful the inflammatory phase continues and remaining microorganisms continue to cause the production of inflammatory cytokines (IL-1, TNF) and chemokines. Persistence of microorganisms in the wound may promote transition to the chronic inflammatory phase. It leads to an increased activity of matrix metalloproteinases (MMPs), which destroy the extracellular matrix and further interfere with wound healing. ROS produced by immune cells are aimed at the destruction of microorganisms, however, when prolonged, the increased production leads to additional tissue damage.

At the early stages of wound healing the tissues are always found at a hypoxia state. This is due to damage to blood vessels, which leads to a disruption in the

АФК, продуцируемые иммунными клетками, нацелены на уничтожение микроорганизмов, однако их длительная повышенная продукция приводит к дополнительному повреждению тканей.

На ранних стадиях раневые ткани всегда находятся в состоянии гипоксии. Это происходит из-за повреждения сосудов, что приводит к нарушению кровоснабжения, а также активному потреблению кислорода метаболически активными клетками. Временная гипоксия после повреждения запускает процесс ранозаживления, однако длительная или хроническая гипоксия приводит к нарушению этого процесса [21]. Если оксигенация раны не восстанавливается, то процесс ранозаживления затягивается. Хронические раны всегда гипоксичны, для них характерно пониженное парциальное давление кислорода, от 20 до 5 мм рт. ст., тогда как в контрольных тканях это значение находится в диапазоне от 50 до 30 мм рт. ст. [22]. На молекулярном уровне гипоксия тканей приводит в первую очередь к нарушению окислительного фосфорилирования в митохондриях, что снижает выработку АТФ, необходимой для осуществления практически всех метаболических процессов в клетках [23]. В частности, нехватка АТФ чревата нарушением работы кальциевых АТФ-аз и накоплением в клетках ионов кальция, что ведет к запуску внутриклеточных воспалительных каскадов. В результате высвобождаются цитокины воспаления и хемокины, привлекающие к месту повреждения иммунные клетки. Длительное уменьшение количества АТФ может приводить к дальнейшему нарушению функционирования митохондрий, повышенной продукции АФК и гибели клеток и тканей.

Кроме того, гипоксия приводит к индукции экспрессии эндотелиальных молекул адгезии, таких как ICAM-1, VCAM-1, которые способствуют проникновению нейтрофилов и макрофагов к месту повреждения [24]. Длительная экспрессия этих молекул приводит к нарушению разрешения воспаления за счет избыточной инфильтрации иммунных клеток.

На гистологических срезах хронических ран также выявляются общие закономерности. Например, гистологическое исследование трофической язвы нижней конечности при варикозном расширении вен демонстрирует наличие утолщенного и находящегося в состоянии гиперплазии края эпидермиса, примыкающего к основанию язвы, покрытому экссудатом, содержащим некротический детрит [25]. blood supply, as well as active consumption of oxygen by metabolically active cells. Temporary hypoxia after damage triggers wound healing process, however, prolonged or chronic hypoxia disrupts this process [21]. If the oxygenation of the wound is not restored, the wound healing process is delayed. Chronic wounds are always hypoxic, they are characterized by a decreased partial oxygen pressure from 20 mm Hg to 5 mm Hg, whereas in healthy tissues these values varies within the range of 50 mm Hg to 30 mm Hg. [22 ]. At the molecular level, tissue hypoxia primarily leads to a suppression of oxidative phosphorylation in the mitochondria thus reducing ATP production required for virtually all metabolic processes in cells [23]. Partcularly, the lack of ATP leads to inhibition of calcium ATPase and accumulation of calcium ions in the cells followed by the subsequent activation of intracel-lular inflammatory cascades. As a result, inflammatory cytokines and chemokines are released that attract immune cells to the site of damage. A prolonged ATP insufficiency may lead to a further mitochondrial dysfunction resulting in increased ROS production and death of cells and tissues.

In addition hypoxia leads to the expression of endothelial adhesion molecules such as ICAM-1, VCAM-1, that promote the penetration of neutrophils and macrophages to the site of injury [24]. Prolonged expression of these molecules inhibits further resolution of inflammation due to excessive infiltration of immune cells.

Histological examination of chronic wounds reveals common patterns. For example, a histological examination of the trophic ulcer of the lower limb in varicose veins demonstrates the presence of a thickened and hyperplastic edge of the epidermis adjacent to the base of the ulcer covered with exudate containing necrotic detritus. The majority of chronic wounds are poorly vascularized, there are practically no myofi-broblasts, but there is a large number of inflammatory infiltrates represented mainly by neutrophils [25].

Chronic wounds are also characterized by excessive accumulation and activation of mast cells (MC) around blood vessels [26-28]. The main functions of these cells of the immune system are associated with allergic reactions, but MCs are also one of the key initiators and regulators of the inflammatory process. Activation of MC is accompanied by degranulation and release of the contents of intracellular vesicles containing various inflammatory mediators, including his-tamine, an important player in the field of inflammation. Histamine causes an increase in the permeability of the vessels and edema of the surrounding tissues, thus facilitating the migration of leukocytes to the focus of inflammation. On the other hand, MC secrete VEGF a potent angiogenic factor required for germination of the vessels into the wound tissues. In addition, MCs participate at the final phases of wound healing and their excessive activation can cause the formation of hypertrophic scars.

Основные функции этих клеток иммунной системы связаны с аллергическими реакциями, но ТК также являются одним из ключевых инициаторов и регуляторов воспалительного процесса. Активация ТК сопровождается дегрануляцией с последующим выбросом во внешнюю среду содержимого внутриклеточных везикул, содержащих различные медиаторы воспаления, среди которых важнейшую роль играет гистамин. Гистамин вызывает увеличение проницаемости стенок сосудов и отек окружающих тканей, способствуя тем самым миграции лейкоцитов в очаг воспаления. С другой стороны, ТК сек-ретируют основной фактор роста сосудов VEGF, способствуя прорастанию сосудов в область раны. Кроме того, ТК участвуют в завершающих стадиях ранозаживления и их избыточная активация может вызывать образование гипертрофированных рубцов.

Для лечения хронических ран в клинике недавно стали применять антиоксиданты. Недавнее мультицентровое двойное слепое исследование на 200-х пациентах продемонстрировало эффективность применения пероральной смеси анти-оксидантов, аргинина и цинка для лечения пролежней [29]. Еще одно двойное слепое исследование на 20-и пациентах с нарушенным заживлением ран показало эффективность перо-ральной смеси антиоксидантов (витамина С, а-токоферола, в-каротина, цинка, селена и глутами-на) по сравнению с плацебо (мальтодекстраном). Данные антиоксиданты способствовали скорейшей эпителизации раны [30]. В отечественной литературе также описывается положительное действие приема антиоксидантов на процесс рано-заживления [31]. Антиоксиданты стандартно назначаются при лечении венозных трофических язв [32], а также при иных патологиях [33-35].

Таким образом, антиоксиданты при перораль-ном приеме могут быть успешно использованы для лечения труднозаживающих ран. При этом следует отметить несколько обстоятельств. Во-первых, количество клинических исследований перораль-ной эффективности антиоксидантов до сих невелико и требуются дальнейшие испытания. Во-вторых, зачастую в таких исследованиях одновременно использовалось несколько различных антиокси-дантов, а также аминокислоты. Антиоксиданты различной химической природы могут иметь разнообразные биологические активности, и их совместное применение может приводить к разнообразным последствиям. Аминокислота аргинин, например, является субстратом для синтеза оксида азота II

Recently, antioxidants have been used to treat chronic wounds in the clinic. One of recent multicen-tre, double-blind study in 200 patients have demonstrated the effectiveness of using an oral mixture of antioxidants, arginine and zinc, to treat bed sores [29]. Another double-blind clinical trial in 20 patients with impaired wound healing studied the effectiveness of the oral antioxidant mixture (vitamin C, a-toco-pherol, ^-carotene, zinc, selenium and glutamine) compared to placebo (maltodextran). These antioxi-dants contributed to the early epithelization of the wound [30]. The domestic literature also describes the positive effect of prescribed antioxidants on the process of wound healing [31]. Antioxidants are routinely prescribed in the treatment of venous trophic ulcers [32], as well as in other pathologies [33-35].

Thus, when administered orally, antioxidants can be successfully used to treat hardly healing wounds. Several circumstances should be noted. Firstly, the number of clinical studies of the oral effectiveness of antioxidants is still small and further tests are required. Secondly, common for such studies, several different antioxidants, alone or in concert with amino acids, are used. Antioxidants of different chemical nature may have a variety of biological activities, and their combined use causes various consequences. Amino acid arginine, for example, is a substrate for the synthesis of nitric oxide II (NO), which causes vasodi-lation and has many other physiological activities including anti-bacterial effect. Thirdly, it cannot be ruled out that antioxidants do not reach the wound surface in a significant amount and might act indirectly, affecting the body systems. The effectiveness of topical application of antioxidants, however, has not been thoroughly studied. One of the limited number of studied drugs is mexidol that has been used for the therapy of trophic ulcers [36].

Mitochondria as a Source of ROS

NADPH oxidase is the main source of exogenous ROS during inflammation [35, 36]. This enzyme is present both in phagocytes, ensuring elimination of microorganisms in the inflammatory site and in non-phagocytic cells contributing to regulation of various intracellular signaling pathways.In a case of oxidative stress caused by external factors the mitochondria are considered as the main source of endogenous ROS [39]. The main types of ROS include hydrogen peroxide (H2O2), superoxide anion radical (02) and hy-droxyl radical (OH ).

Over the past few years there has been a breakthrough in understanding the role of mitochondrial ROS in various inflammatory processes. In general, oxidative stress can cause an inflammatory response and excessive inflammation, which in turn cause ox-idative stress leading to damage of tissues and organs. Such a vicious circle is described, for example, for activation of inflammasome NLRP3. This inflammasome

Митохондрии как источник АФК

Основным источником экзогенных АФК при воспалении является НАДФН-оксидаза [37, 38]. Этот фермент присутствует как в фагоцитах, обеспечивая уничтожение микроорганизмов в очаге воспаления, так и в нефагоцитирующих клетках, принимая участие в регуляции различных внутриклеточных сигнальных путей. При окислительном стрессе, вызванном внешними окислителями, основным источником эндогенных АФК являются митохондрии [39]. Основными формами АФК являются перекись водорода (Н202), радикал супероксид-аниона (О2) и гидроксильный радикал (ОН ).

За последние несколько лет произошел прорыв в понимании роли митохондриальных АФК в различных воспалительных процессах. В целом, окислительный стресс может вызывать воспалительный ответ, а избыточное воспаление, в свою очередь, вызывает окислительный стресс, что приводит к повреждению тканей и органов. Такой порочный круг описан, например, при активации инфламмасомы NLRP3. Эта инфламмасома типична для миелоидных клеток и ответственна за созревание ^-1Ье1а и ^-18. NLRP3 активируется под действием молекулярных образов опасности (ОАМРэ и PAMPs), которые в большом количестве присутствуют в ранах. Недавно было обнаружено, что митохондриальные АФК вызывают чрезмерную активацию NLRP3 [40] и подавление АФК с помощью митохондриально-направленных антиоксидантов снижает ее активацию [41].

Митохондриальные АФК также принимают участие в патогенезе атеросклероза [42] и гипертен-зии [43]. В развитии окислительного стресса при ишемии-реперфузии первоочередную роль играет выброс митохондриями супероксид-аниона [44].

Митохондриально-направленные антиоксиданты

В лаборатории проф. В. П. Скулачева были созданы новые эффективные митохондриально-направленные антиоксиданты семейства БкО, [45]. Они состоят из антиоксидантной части (пластохи-нона) и катиона, способного проникать через мембрану (рис. 1). Благодаря положительному заряду эти соединения избирательно накапливаются в

is typical for myeloid cells and is responsible for the maturation of IL-1 beta and IL-18. NLRP3 is activated by danger-associated and pathogen-associated molecular patterns (DAMPs and PAMPs, respectively), which are present in large amount in the wounds. Recently, it has been found that mitochondr-ial ROS cause excessive activation of NLRP3 [40] and inhibition of ROS by mitochondria-targeted antioxi-dants reduces NLRP3 activation [41].

Mitochondrial ROS also participate in the pathogenesis of atherosclerosis [42] and hypertension [43]. Mitochondrial release of superoxide anion is necessary in the development of oxidative stress in is-chemia-reperfusion [44].

Mitochondrially Targeted Antioxidants

New effective mitochondrial antioxidants of the SkQ family were created in the lab of prof. V. P. Sku-lachev [45]. They consist of an antioxidant part (plas-toquinone) and a cation capable of penetrating through the membrane (Fig. 1). Due to the positive charge, these compounds selectively accumulate in the mitochondria, which allows them to be used at extremely low concentrations. High efficiency of SkQ against a wide range of pathologies has been confirmed in numerous experiments on cell cultures and in vivo using experimental animal models of human diseases [46].

The key differences between the mitochondrial antioxidants of the SkQ family (SkQs) from conventional antioxidants include:

1) SkQs are accumulating to act directly at the site of ROS formation, in the inner membrane of the mitochondria [45];

2) SkQs act repeatedly; upon neutralization of ROS, the formulation is oxidized, and then restored by the respiratory chain of the mitochondria. In contrast, classical antioxidants fulfill their function only once: after oxidation, they turn into useless, or even toxic compounds [47];

3) SkQs prevent the oxidation of cardiolipin, a key regulator of cell apoptosis [48].

Currently available, clinically developed SkQs formulation is represented by SkQ1-containing eye drops (Visomitin), and this preparation has been officially registered as a drug for dry eye syndrome treatment (LP-001355 from December 13, 2011). Now SkQ1 formulation undergoes clinical trials for the treatment of glaucoma, maculodystrophy and uveitis.

A large complex of preclinical studies of SkQ1 as an eye drops formulation «Visomitin» has been initiated that include three clinical trials in patients suffering from dry eye syndrome. The lack of eye hydration in this disease leads to damage to the corneal epithelium. The effective restoration of the integrity of the corneal epithelium, i.e. healing of superficial injuries, is an important clinically significant parameter in the medical treatment of dry eye syndrome. This parameter has become a clinical target in

Ключевые отличия митохондриальных анти-оксидантов семейства SkQ от обычных антиокси-дантов заключается в том, что они:

1) действуют непосредственно в месте образования АФК - во внутренней мембране митохондрий [45];

2) действуют многократно, при нейтрализации АФК они окисляются, а затем восстанавливаются дыхательной цепью митохондрий. Классические антиоксиданты выполняют свою функцию однократно. Окислившись, они превращаются в бесполезные, или даже токсичные соединения [47];

3) предотвращают окисление кардиолипина -ключевого регулятора апоптоза клеток [48].

На основе SkQ1 были созданы глазные капли (Визомитин), данный препарат официально зарегистрирован как лекарственное средство от синдрома сухого глаза (ЛП-001355 от 13 декабря 2011 г.), в процессе клинические испытания по применению SkQ1 в лечении глаукомы, макуло-дистрофии и увеита.

Большой комплекс доклинических экспериментов лекарственной формы SkQ1 - глазных капель «Визомитин» позволил провести 3 отдельных клинических исследования с участием пациентов, страдающих синдромом сухого глаза. Недостаток увлажненности глаза при этом заболевании ведет к повреждению эпителия роговицы. Важным клинически значимым параметром при медикаментозном лечении синдрома сухого глаза является эффективное восстановление целостности эпителия роговицы, т.е. заживление поверхностных повреждений. В клинических исследованиях, а именно открытое рандомизированное российское исследование 2-й фазы [49], мультицентровое двойное слепое плацебоконтролируемое исследование 3-й фазы [50] и проведенное под эгидой управления по санитарному надзору за качеством пищевых продуктов и медикаментов (FDA) исследование в США [51] показали выраженный эффект SkQ1 по ускорению заживления ран роговицы по сравнению с эффектом применения увлажняющих глазных капель того же состава, но без SkQ1.

SkQ1 ускоряет заживление полнослойных кожных ран у старых мышей

Для старых животных характерно нарушение репарационных процессов, что приводит к удлинению воспалительной фазы раневого процесса и к значительному замедлению заживления ран.

recent clinical trials. They included: an open, randomized Russian phase 2 study [49], a multicenter doubleblind placebo-controlled study of the 3rd phase [50] and a study conducted in the United States under the control of Food and Drug Administration, USA [51]. Trials have demonstrated a significant therapeutic effect of SkQ1 on accelerating the healing of the corneal wounds compared to the effect of the same composition of moisturizing eye drops without SkQ1.

SkQ1 Accelerates the Healing of Full-Thickness Skin Lesions in Older Mice

Old animals are characterized by impaired repair processes leading to an elongation of the inflammatory phase of the wound and to a significant delay in wound healing.

In experiments with old (24 months) female CBAxC57Bl/6 hybrids, a significant effect in wound area reduction was observed at the early stages of healing (1—5 days) [52]. In the experimental group (n=10) the animals orally received SkQ1 at a daily dose of 100 nmol/kg of body mass for 8 months starting from the age of 16 month. The dynamics of wound area reduction in old mice receiving SkQ1 was almost identical to the dynamics observed in young animals (Fig. 2, a). Histological analysis of wounds on the 7th day showed that in the group of young animals the damage area was completely filled with granulation tissue, and in old mice the centers of granulation formation were located only along the edges of the wound (Fig. 3, a). In the old SkQ1 mice, an abundant vascularized granulation tissue filled almost the entire area of the wound defect. Granulations contained more mature and regularly oriented collagen fibers and less leukocyte infiltration. The administration of SkQ1 increased the content of myofibroblasts in the granulation tissue, which, apparently, accelerated its maturation. Morphometric analysis revealed a significant decrease in the volume density

В экспериментах на старых (24 мес.) самках мышей гибридов СВАхС57В1/6 наблюдалось значительное замедление сокращения площади ран на ранних этапах заживления (1—5 сут.) [52]. В опытной группе (п=10) животные перорально получали БкОД в суточной дозе 100 нмоль/кг веса в течение 8 мес. начиная с 16-ти мес. возраста. Динамика сокращения площади ран у старых мышей, получавших БкОД, была практически идентична динамике, наблюдавшейся у молодых животных (рис. 2, а). Гистологический анализ ран на 7-е сутки показал, что в группе молодых животных область повреждения была целиком заполнена грануляционной тканью, а у старых мышей очаги формирования грануляций были расположены только по краям раны (рис. 3, а). У старых мышей, получавших обильно васкуляризованная

грануляционная ткань заполняла практически всю область раневого дефекта, в ней располагались более зрелые и регулярно ориентированные колла-геновые волокона, реже встречались очаги лейкоцитарной инфильтрации. Прием повышал содержание миофибробластов в грануляционной ткани, что, по-видимому, ускоряло ее созревание. Морфометрический анализ выявил значительное снижение объемной плотности сосудов в грануляционной ткани у старых животных по сравнению с молодыми. Значение этого показателя у старых мышей, получавших достоверно повышалось, и было сопоставимо с объемной плотностью сосудов в ранах молодых животных. Этот эффект во многом способствовал заполнению поврежденной области грануляционной тканью, поддерживая ее оксигенацию и питание. У старых животных наблюдалось замедление эпителизации раны, при этом БкОД вызвал значительное ускорение этого процесса, не влияя на толщину эпидермиса. Никакого эффекта на толщину рубца также не было обнаружено. В целом, динамика заживления ран у старых мышей, получавших БкЦ1, напоминала таковую у молодых животных.

Анализ воспалительного процесса выявил значительное усиление нейтрофильной инфильтрации ран у старых мышей по сравнению с молодыми, а вызвал достоверное снижение числа нейтрофилов. У старых животных наблюдалось заметное снижение макрофагальной инфильтрации по сравнению с молодыми на 7-е сут. заживления, а на 13-е сут. число макрофагов в ранах старых мышей примерно в 2 раза превышало его значение у молодых. В ранах старых мышей, получавших БкОД, численность макрофагов на 7-е сут. значительно возрастала и сохранялась на том же уровне и на 13-е сут. Таким образом, полученные результаты указывали на нормализацию воспалительной фазы заживления ран у старых животных под действием БкОД.

БкОД ускоряет заживление полнослойных кожных ран у мышей с диабетом 2 типа.

of blood vessels in granulation tissue in old animals compared to young ones. This value in the old mice receiving SkQ1 was significantly increased and was comparable to the bulk density of the vessels in the wounds of young animals. This effect of SkQ1 greatly contributed to the filling of the damaged area with granulation tissue, supporting its oxygenation and nutrition. In old animals, a delay in epithelialization of the wound was observed, while SkQ1 caused significant acceleration of this process, without affecting the thickness of the epidermis. No effect of SkQ1 on the thickness of the scar was found. In general, the dynamics of wound healing in old mice that received SkQ1 resembled that of young animals.

An analysis of the inflammatory process revealed a significant increase of neutrophil infiltration in the wounds of old mice compared to young mice, and SkQ1 caused a significant decrease in the number of neutrophils. Old animals showed a marked decrease in macrophage infiltration compared to the young animals observed on the 7th day of healing, and on the 13th

Формирование незаживающих ран является одним из основных осложнений при диабете. В качестве модели этой патологии были использованы мыши линии С57БЬ 1ер^Ь ^Ь^Ь) с врожденным диабетом 2-го типа [53]. Длительный прием не снижал основных признаков диабета (гипергликемия, ожирение, полиурия) у этих мышей, но снижал выраженность маркеров окислительного стресса. Заживление ран у диабетических мышей было резко замедлено по сравнению с гетерозиготными животными (С57БЬ 1ерг Л/+). Мыши опытной группы (п=8) ежедневно получали перорально в дозе 250 нмоль/кг веса в сутки в течение 12 недель, начиная с 10-недельного возраста. Прием БкОД практически полностью нормализовал заживление ран у Л/Л мышей (рис. 2, Ь). Гистологический анализ ран выявил у Л/Л мышей те же патологические изменения, что и у старых животных. вызывал нормализацию

как формирования грануляционной ткани, так и воспалительной фазы процесса. Особо ярко был выражен эффект стимуляции ангиогенеза под действием БкЦ1. У Л/Л мышей не наблюдалось заметного торможения эпителизации раны, но и в

Thus, the results demonstrate the abrogation of the inflammatory phase of wound healing in old animals under the action of SkQ1.

SkQ1 Accelerates Healing of Full-Thickness Skin Wounds in Mice with Type 2 Diabetes

Non-healing wounds represent one of the major complications in diabetes mellitus. As a model of this pathology the C57BL leprdb (db/db) mice genetically predisposed to type 2 diabetes development [53] were used. Long-term administration of SkQ1 did not reduce the main features of diabetes in these mice (hy-perglycemia, obesity, polyuria), but reduced the expression of markers of oxidative stress. Wound healing in diabetic mice was significantly delayed compared to heterozygous animals (C57BL lepr db/+). Mice of the experimental group (n=8) daily received

Локальное введение SkQ1 ускоряет репаративные процессы в полнослойных кожных ранах у крыс

В первой серии экспериментов крысам ежесуточно под кожу вокруг ран инъецировали 50 мкл 200 нМ раствора SkQ1. Забор материала для гистологического анализа проводили через 7 часов, 1—3, 5 и 14 сут. Было обнаружено статистически значимое уменьшение площади раны на пятый день эксперимента. Подсчет количества нейтрофилов на ранних сроках заживления ран показал более чем двукратное уменьшение количества нейтрофилов через 7 час. В фазе созревания грануляционной ткани SkQ1 вызывал 3-х кратное увеличение количества миофибробластов, экспрессирующих глад-комышечный бета-актин [54].

Во второй серии экспериментов крысам линии Sprague-Dawley на раны накладывали пленочное покрытие «Биоплен» с 0,019 мкг SkQ1/T пленки. Смену пленок проводили один раз в 3 дня в течение 14 сут. Статистически значимое уменьшение площади раны по сравнению с контролем было обнаружено на 4-ый и 7-ой дни. Гистологический анализ на 14-е сутки эксперимента показал, что в затянувшихся к этому времени ранах у животных, получавших SkQ1, содержатся более толстые пучки коллагеновых волокон, чем в контроле, что может быть связано с повышением активности миофибробластов [54].

Исследования противовоспалительного действия SkQ1 in vivo

Для старения характерен повышенный уровень системного воспаления. В экспериментах со старыми мышами было установлено, что длительный прием SkQ1 снижает воспалительную активацию эндотелия сосудов, измеренную по уровню экспрессии молекул адгезии [55]. В модели синдрома острого воспалительного ответа вызванного внутривенным введением летальных или сублетальных доз фактора некроза опухолей (TNF). SkQ1 также снижал воспалительную активацию эндотелия [56]. Интраперитонеальное введение SkQ1 не только понижало экспрессию молекул адгезии в эндотелии сосудов, но и предотвращало гибель мышей в этой модели. Подавление избы-

SkQ1 orally at a dose of 250 nmol/kg of weight per day for 12 weeks, starting at 10 weeks of age. SkQ1 almost completely normalized wound healing in db/db mice (Fig. 2, b). Histological analysis of wounds revealed the same pathological changes in db/db mice as in old animals. SkQ1 caused normalization of granulation tissue formation and abrogation of the inflammatory phase. SkQ1 effect of stimulation of angiogenesis was particularly pronounced. In db/db mice no inhibition of epithelialization of the wound was observed but SkQ1 markedly accelerated this process. An elongated inflammatory phase continued in db/db to the same extent as in old animals, and the SkQ1 administration normalized it. Probably, the ox-idative stress accompanying diabetic pathology causes impaired wound healing in many ways. SkQ1 significantly accelerated the formation of granulation tissue in diabetic mice without scar hypertrophy (Fig. 3, b).

Local Administration of SkQ1 Accelerates Reparative Processes in Dermal Wounds in Rats

In the first series of experiments, rats were injected subcutaneously daily with 50 ^l of 200 nM SkQ1 solution in the area around the wound. Sampling of material for histological analysis was performed after 7 hours, 1—3, 5 and 14 days. A statistically significant reduction in wound area was observed on the 5th day of the experiment. Counting the number of neutrophils in the early stages of wound healing showed a more than twofold decrease in the number of neutrophils after 7 hours in SkQ1 treated rats. In the maturation phase of the granulation tissue, SkQ1 caused a 3-fold increase in the number of myofibroblasts expressing smooth-muscle beta-actin [54].

In a second series of experiments, wounds in the Sprague-Dawley rats were covered with «Bioplene» film containing 0.019 ^g of SkQ1/g. Change of films was carried out once in 3 days for 14 days. A statistically significant reduction in wound area compared to the control was detected on the 4th and 7th days. Histological analysis on the 14th day of the experiment showed that in the wounds that were closed by this time, the animals receiving SkQ1 contained thicker bundles of collagen fibers than in the control that presumably caused by an increased activity of myofibroblasts [54].

Studies of the Anti-inflammatory Effect

of SkQ1 in vivo

Aging is characterized by an elevated level of systemic inflammation. In experiments with old mice, long-term application of SkQ1 has been shown to reduce inflammatory vascular endothelial activation, as measured by the expression level of the adhesion molecules [55]. In the model of systemic inflammatory response syndrome (SIRS) caused by intravenous administration of lethal or sublethal doses of tumor

Действие SkQ1 было исследовано также в моделях острого асептического воспаления [54]. В первой модели воспаление индуцировали путем введения под кожу в межлопаточной области покровных стёкол. У мышей, которые получали SkQ1 в дозе 250 нмоль на 1 кг массы тела ежедневно с питьевой водой в течение 3 недель до операции, количество нейтрофилов в очаге воспаления, измеренное через 12 ч после операции, снижалось в 2 раза. Одновременно под действием SkQ1 существенно повышалось относительное содержание макрофагов, что указывало на ускоренный переход к макрофагальной стадии воспалительной реакции. Через 2 сут нейтрофилы практически исчезали из очага воспаления, как у мышей, которые получали SkQ1, так и в контрольной группе. На более поздних сроках (5—14 сут) наблюдалось накопление фибробластов в очаге воспаления, причем SkQ1 существенно ускорял этот процесс.

Во второй модели в межлопаточной области мышей формировали подкожные воздушные мешки и через 7 сут индуцировали в них острое воспаление инъекцией 1% раствора каррагинана. Эта модель является одной из общепринятых для доклинического исследования эффективности фармацевтических противовоспалительных препаратов [57] и позволяет исследовать как клеточные, так и экссудативные реакции в очаге подкожного воспаления. У мышей, получавших SkQ1 по той же схеме, что и в предыдущей модели, содержание клеток воспаления в экссудате, полученном через 4 часа после введения каррагинана, было снижено почти в 2 раза по сравнению с контролем [58]. Около 90% клеток в экссудате составляли нейтрофилы. Содержание основных про-воспалительных цитокинов TNF и IL-6 под действием SkQ1 снижалось, но этот эффект был небольшим. Более выраженные эффекты SkQ1 наблюдались в той же модели, но при внутрибрю-шинном введении препарата (250 нмоль/кг массы тела) ежедневно в течение 7 сут до индукции воспаления [58]. Таким образом, SkQ1 вызывал снижение интенсивности привлечения нейтрофилов в очаг воспаления и, в меньшей степени, снижал продукцию провоспалительных цитокинов.

В модели воздушного мешка было исследовано содержание клеток воспаления в выстилающих тканях. Было показано, что численность нейтрофи-лов в верхней внутренней выстилке «воздушного мешка» у мышей, получавших SkQ1, была снижена в 1,5 раза по сравнению с контролем. В то же время относительное содержание моноцитов/макрофагов в выстилке у мышей, получавших SkQ1 было достоверно выше, чем в контрольной. Эти данные свидетельствуют о том, что SkQ1 способствует более быстрому переключению к макрофагальной стадии воспалительной реакции [58].

necrosis factor (TNF) SkQ1 also reduced inflammatory endothelial activation [56]. The intraperitoneal administration of SkQ1 not only reduced the expression of the adhesion molecules in the vascular en-dothelium, but also prevented the death of mice in this model. Suppression of excessive activation of endothe-lial cells appears to be the main mechanism for reducing neutrophil infiltration of wounds.

The effect of SkQ1 was also investigated in models of acute aseptic inflammation [54]. In the first model, inflammation was induced by inserting under the skin in the interscapular region of the cover glasses. In mice that received SkQ1 at a dose of 250 nmol per 1 kg of body mass daily with drinking water for 3 weeks prior to surgery, the amount of neutrophils in the inflamed area, measured 12 hours after surgery, was reduced by a factor of 2. Simultaneously, in SkQ1 treated animals, the relative content of macrophages significantly increased, which indicated an accelerated transition to the macrophage stage of the inflammation. After 2 days neutrophils practically disappeared from the inflamed area, both in mice that received SkQ1 and in the control group. At later periods (5-14 days), the accumulation of fibroblasts in the inflamed area was observed, and SkQ1 significantly accelerated this process.

In the second model, subcutaneous air sacs were formed in the interscapular region of mice and after 7 days acute inflammation was induced in the sacs by injection of a 1% solution of carrageenan. This model is one of the generally accepted for preclinical studies of the effectiveness of anti-inflammatory drugs [57] and allows to study both cellular and exudative reactions in the subcutaneous inflamed area. In mice that received SkQ1 in the same way as in the previous model, the content of inflammatory cells in the exudate obtained 4 hours after the administration of car-rageenan was reduced almost 2-fold compared to the control [58]. About 90% of the cells in the exudate were neutrophils. SkQ1 induced decrease in the content of the main pro-inflammatory cytokines TNF and IL-6, but this effect was not large. More significant effects of SkQ1 were observed in the same model using intraperitoneal administration of the drug (250 nmol / kg body mass) daily for 7 days before induction of inflammation [58]. Thus, SkQ1 caused a decrease in the intensity of neutrophil infiltration into the inflamed area and, to a lesser extent, reduced the production of pro-inflammatory cytokines.

In the air sac model, the content of inflammatory cells in the lining tissues was examined. It was shown that the number of neutrophils in the upper inner lining of the air sac in mice receiving SkQ1 was reduced by 1.5 times in comparison with the control. At the same time, the relative monocyte / macrophage content in the lining in mice treated with SkQ1 was significantly higher than in the control. These data suggest that SkQ1 promotes faster transition to the macrophage stage of the inflammatory response [58].

В той же модели было исследовано влияние SkQ1 на содержание и активацию тучных клеток в очаге воспаления. В модели воздушного мешка было показано, что общая численность ТК и в особенности содержание дегра-нулирующих ТК в верхней внутренней выстилке была снижена у мышей получавших SkQ1 с питьевой водой [57, 58]. Как и в случае нейтрофилов, более выраженное влияние SkQ1 на содержание и степень активации ТК наблюдались в той же модели, но при внутрибрюшинном введении препарата [59]. Исследования in vitro (см ниже) подтвердили подавление дегра-нуляции ТК под действием SkQ1. Эти наблюдения позволяют предполагать, что SkQ1 эффективно понижает активацию ТК в ранах, что предотвращает гистамин-зависимое повышение проницаемости сосудов и отечность.

Механизмы стимуляции

регенеративных процессов под действием SkQ1

К настоящему времени установлено, что SkQ1 оказывает влияние как на воспалительную фазу рано-заживления, так и на пролиферативную стадию. Мишенями SkQ1 являются клетки разных типов, ответственные за ранозаживление. Ниже представлены основные результаты исследования механизмов действия SkQ1 на заживление ран in vitro (рис. 4).

Для исследования механизмов действия SkQ1 на воспалительную активацию эндотелия были использованы культуры клеток эндотелия, активированные TNF, основным провоспалитель-ным цитокином. Было установлено, что:

1) SkQ1 подавляет TNF-зависимую экспрессию молекул адгезии и их экспозицию на поверхности клеток. Как следствие, под действием SkQ1 снижается адгезия нейтрофилов к монослою клеток эндотелия [55, 56].

2) SkQ1 подавляет TNF-зависимую экспрессию провоспалительных цитокинов [55, 56].

3) SkQ1 предотвращает TNF-зависимое нарушение цитоскелета и разборку межклеточных контактов, что ведет к нарушению изолирующих свойств эндотелия [56].

4) Противовоспалительное действие SkQ1 определяется подавлением активности транскрипционного фактора NFkB, который контролирует экспрессию молекул адгезии и ММР-9, расщепляющий основной белок межклеточных контактов VE-кадгерин [56].

5) SkQ1 защищает клетки эндотелия от апоп-тотической гибели, вызванной высокими дозами TNF [60].

In the same air sac model, the effect of SkQ1 on the mast cells (MC) in the inflamed area was investigated. It was shown that the total number of MCs, and particularly the content of degranulated TCs in the upper inner lining tissues, was reduced in mice that received SkQ1 with drinking water [58]. As in the case of neutrophils, more significant effects of SkQ1 on the content and degree of activation of MC were observed when intraperitoneal administration of the drug was employed [59]. In vitro studies (see below) confirmed the suppression of MC degranulation by SkQ1. These observations suggest that SkQ1 can effectively reduce MC activation in wounds, which prevents histamine-dependent vascular permeability and tissue swelling.

Mechanisms of Regenerative Processes Stimulation Induced by SkQ1

To date, it has been established that SkQ1 accelerates both the inflammatory phase of wound healing and the proliferative phase. SkQ1 targets the different cell types responsible for wound healing. Below the main mechanisms of SkQ1 action on wound healing using the in vitro studies data is proposed (Fig. 4).

To study the mechanisms of SkQ1 action on inflammatory activation of the endothelium, endothelial cell cultures were activated by the major pro-inflammatory cytokine, TNF. It has been found that:

1) SkQ1 suppresses TNF-dependent expression of the adhesion molecules and their exposure at the cell surface. As a consequence, the adhesion of neu-trophils to the monolayer of endothelial cells decreases under SkQ1 action [55, 56].

2) SkQ1 suppresses TNF-dependent expression of pro-inflammatory cytokines [55, 56].

Описанное выше противовоспалительное действие SkQ1 на эндотелий во многом объясняет снижение инфильтрации нейтрофилов в раны. Однако ускоренный переход к макрофагальной стадии воспалительной реакции наблюдавшийся под действием SkQ1 in vivo указывает на возможное участие митоАФК в регуляции активности нейтрофилов и других иммунных клеток. На иммунных клетках человека ex vivo было установлено, что:

1) SkQ1 стимулирует как спонтанный апоп-тоз нейтрофилов, так и, в особенности, апоптоз, частично подавленный под действием патоген-ассоциированных сигнальных молекул (РАМР) [56] или сигнальных молекул, связанных с повреждением (DAMP) [55].

2) SkQ1 подавляет активацию НАДФН окси-дазы и связанный с этим окислительный взрыв при активации нейтрофилов компонентами, имитирующими бактериальные патогены [56].

3) SkQ1 ингибирует экзоцитоз (дегрануля-цию) внутриклеточных везикул при активации нейтрофилов [56]. Сходное подавление дегрануля-ции под действием SkQ1 наблюдалось в тучных клетках [61].

4) Структурно сходный с SkQ1 митохондри-ально-направленный антиоксидант MitoQ ингиби-ровал продукцию провоспалительных цитокинов и повышению продукции противовоспалительных цитокинов (сдвиг к фенотипу М2) в макрофагах [41].

Подавление активности нейтрофилов под действием SkQ1 может препятствовать их бактерицидному действию. В тоже время установлено, что высокие концентрации SkQ1 обладают антисептическим действием против широкого круга Грам-положительных и Грам-отрицательных бактерий [62]. Таким образом, SkQ1 при локальном применении может способствовать уничтожению патогенных микроорганизмов в ране, компенсируя недостаток нейтрофилов.

Исследования механизмов действия SkQ1 на прохождение пролиферативной фазы ранозажив-ления показали, что:

1) В культуре подкожных фибробластов человека SkQ1 вызывает дифференцировку части популяции (около 30%) фибробластов в миофиб-робласты [63]. Этот эффект был связан с активацией TGF/31, основного цитокина, регулирующего дифференцировку фибробластов. Вместе с тем SkQ1подавляет накопление миофибробластов, вызванное избытком TGF/31, ингибируя TGF/31-зависимые сигнальные пути [63]. Эти данные соответствуют отсутствию гипертрофированных рубцов и иных признаков фиброза у животных получавших SkQ1;

2) SkQ1 стимулирует подвижность фибробластов в модели «раны» in vitro при гипергликемии [51];

3) SkQ1 стимулирует ангиогенез in vitro благодаря активации в фибробластах TGF/31 и стиму-

3) SkQ1 prevents TNF-dependent disruption of the cytoskeleton and disassembly of the intercellular contacts, which leads to permeabilization of the en-dothelium [56].

4) The anti-inflammatory effects of SkQ1 are related to inhibition of the pro-inflammatory transcription factor NFkB, which controls the expression of the adhesion molecules and matrix metalloprotease MMP-9. The latter cleaves VE-cadherin, the main protein of the intercellular contacts [56].

5) SkQ1 protects endothelial cells from apop-totic death caused by high doses of TNF [60].

The anti-inflammatory effects of SkQ1 on the endothelium largely explain the reduction of neu-trophil infiltration into the wounds. However, the in vivo SkQ1-induced accelerated transition of inflammation reaction toward the macrophage stage demonstrates the possible involvement of mitochondrial ROS in regulation of the activity of neutrophils and other immune cells. Experiments using human immune cells ex vivo have found that:

1) SkQ1 stimulates both spontaneous apoptosis of neutrophils and, in particular, apoptosis, partially suppressed by PAMP [56] or DAMP [55].

2) SkQ1 suppresses the activation of NADPH oxidase and the associated oxidative burst when neu-trophils are activated by compounds that mimic bacterial pathogens [56].

3) SkQ1 inhibits the exocytosis (degranulation) of intracellular vesicles upon activation of neutrophils [56]. A similar suppression of the degranulation by SkQ1 was observed in mast cells [61].

4) MitoQ, a mitochondria-targeted antioxidant structurally similar to SkQ1, inhibited production of pro-inflammatory cytokines and increased production of anti-inflammatory cytokines (shift to M2-pheno-type) in macrophages [41].

Suppression of neutrophils activity by SkQ1 can prevent their bactericidal effect. It was found that high concentrations of SkQ1 have an antiseptic effect against a wide range of Gram-positive and Gram-negative bacteria [62]. Thus, local application of SkQ1 contributes to the destruction of pathogenic microorganisms in the wound, compensating the deficit of neutrophils.

Evaluation of mechanisms of SkQ1 action in the proliferative phase of wound healing demonstrated that:

1) In the culture of human subcutaneous fibroblasts, SkQ1 causes differentiation of a part (approx. 30%) of fibroblasts toward myofibroblasts [63]. This effect was associated with the activation of TGF/31, the main cytokine regulating the differentiation of fibroblasts. At the same time, SkQ1 suppresses the accumulation of myofibroblasts caused by an excess of TGF/31, inhibiting TGF/31-dependent signaling pathways [63]. These data correspond to the absence of hy-pertrophic scars and other signs of fibrosis in animals receiving SkQ1.

Заключение

Митохондрии играют определяющую роль в формировании окислительного стресса, который является ключевым фактором патогенеза трудно-заживающих и хронических ран. Высокоэффективный митохондриально-направленный анти-оксидант SkQ1 стимулирует заживление кожных ран, действуя на воспалительную и пролифера-тивную фазы ранозаживления в различных экспериментальных моделях in vivo и in vitro. Имеются основания полагать, что препараты для локальной терапии труднозаживающих ран на основе SkQ1 послужат эффективным средством борьбы с длительно незаживающими ранами у разных категорий пациентов, в том числе — при сахарном диабете, а также у пациентов с хроническими критическими состояниями и рецидивирующими инфекциями.

2) SkQ1 stimulates the mobility of fibroblasts in the «wound» model in vitro under hyperglycemia [53].

3) SkQ1 stimulates angiogenesis in vitro due to activation of TGF/31 in fibroblasts and stimulation of synthesis of other growth factors accelerated vascular growth [52].

4) SkQ1 promotes epithelization in the «wound» model in the monolayer of immortalized rat epithelial cells, accelerating the migration of these cells to the edges of the «wound» [54].

Conclusion

Mitochondria play a critical role in the formation of oxidative stress, which is a key factor in the pathogenesis of wounds with impaired healing and chronic wounds. Highly effective mitochondrial antioxidant SkQ1 stimulates the healing of cutaneous wounds by accelerating both the inflammatory and the proliferative phases of wound healing in various models in vivo and in vitro. Reviewed data suggest that SkQ1-based topical drugs for the local wound healing possess high potential to become effective treatment for wounds with impared healing in patients suffering from diabetes, chronic critical illness and recurring infections.
