В первой части обзора представлены данные литературы по: структурнофункциональной организации микроциркуляторного русла; регуляции сосудистого тонуса, включая местные (метаболические, миогенные механизмы, роль эндотелия и эритроцитов) и системные (нервные, гуморальные) факторы; общие вопросы нарушений микроциркуляции при кровопотере и шоке. Острая кровопотеря рассмотрена в аспекте стадийного течения компенсаторноприспособительных процессов с вовлечением системы крови, сердечнососудистой, нервной и эндокринной систем организма. Описаны изменения транспорта кислорода, патогенезгипоксии и метаболических нарушений при кровопотере и геморрагическом шоке. Представлены примеры порочных кругов патогенеза острой кровопотери, как основы развития декомпенсированного геморрагического шока, а также отмечена важная роль нарушений гемостаза и реологии крови в этом процессе.

The first part of the review highlights the data on the structural and functional organization of the microvas culature; regulation of vascular tone including local (metabolic and myogenic mechanisms, role of endothelium and red blood cells) and systemic (nervous, humoral) factors; general issues of microcirculatory alterations in blood loss and shock. Acute blood loss is considered as a phasic development of compensatory and adaptive processes involving the blood system, cardiovascular, nervous and endocrine systems. Changes in oxygen transport, the pathogenesis of hypoxia and metabolic abnormalities in blood loss and hemorrhagic shock are described. Examples of the vicious circles contributing to acute blood loss pathogenesis are considered as a base for developing decompensated hemorrhagic shock in which the role of hemostasis and blood rheology alterations are also considered.

В первой части обзора представлены данные литературы по: структурно-функциональной организации микроциркуляторного русла; регуляции сосудистого тонуса, включая местные (метаболические, миогенные механизмы, роль эндотелия и эритроцитов) и системные (нервные, гуморальные) факторы; общие вопросы нарушений микроциркуляции при кровопотере и шоке. Острая кровопотеря рассмотрена в аспекте стадийного течения компенсаторно-приспособительных процессов с вовлечением системы крови, сердечно-сосудистой, нервной и эндокринной систем организма. Описаны изменения транспорта кислорода, патогенез гипоксии и метаболических нарушений при кровопотере и геморрагическом шоке. Представлены примеры порочных кругов патогенеза острой кровопотери, как основы развития декомпенсированного геморрагического шока, а также отмечена важная роль нарушений гемостаза и реологии крови в этом процессе.

акций, соотношение и выраженность которых определяются не только и не столько этиологией основного заболевания, сколько скоростью и общим объемом кровопотери, условиями окружающей среды, реактивностью и функциональными резервами организма (пол, возраст, генетические особенности, сопутствующая патология и др.).

Пусковым звеном патогенеза острой крово-потери является уменьшение объема циркулирующей крови (ОЦК), что последовательно приводит к уменьшению венозного возврата и сердечного выброса, снижению системного артериального давления (АД) и гипоперфузии органов и тканей. Последний фактор в сочетании с нарушениями реологии крови приводит к расстройствам в системе микрогемоциркуляции и развитию капилляротрофической недостаточности, являющейся непосредственной причиной гипоксии клеток [1]. Продолжающееся кровотечение и несостоятельность компенсаторных реакций организма и лечебных мероприятий, приводят к развитию патологических «порочных кругов», активации перекисного окисления липи-дов, прогрессированию гипоксии смешанного типа, тяжелых метаболических нарушений (ацидоз, водно-электролитные нарушения, истощение энергетических и пластических ресурсов клеток) и гибели клеток по механизму некроза или апоп-тоза [2, 3]. Результатом описанных патологических процессов являются нарушение функции органов и развитие полиорганной недостаточности с летальным исходом.

Геморрагический шок представляет собой крайнюю степень перечисленных выше гемоди-намических и метаболических расстройств в результате острой массивной кровопотери. Он является разновидностью гиповолемического шока и для его развития необходим ряд условий: промежуток времени для развития ответных реакций организма; неадекватная перфузия тканей; расстройства клеточного метаболизма; потенциально смертельный характер поражений [3].

Нарушения кровообращения на микроцир-куляторном уровне, возникающие при острой кровопотере и всегда выраженные при геморрагическом шоке, являются ключевым патогенетическим фактором развития гипоксии и дисфункции органов [4, 5]. Однако, несмотря на общие пато-морфологические и функциональные проявления (снижение плотности перфузируемых капилляров, гетерогенность перфузии, замедление капиллярного кровотока, повышение вязкости крови и проницаемости эндотелия и др.), нарушения мик-рогемоциркуляции, тем не менее, имеют свою специфику в зависимости от сосудистого региона организма. The initiating factor in the pathogenesis of acute blood loss is a decrease in circulating blood volume (CBV), which successively leads to a decrease in venous return, cardiac output (CO), systemic blood pressure (BP) and to the hypoperfusion of organs and tissues. The latter factor, coupled with impaired blood rheology leads to microcirculation impairment and development of a «capillary failure» that represents a direct cause of cells' hypoxia [1]. The ongoing bleeding, failure of the body's compensatory reactions and therapeutic measures lead to the development of pathological «vicious circles», the activation of lipid peroxidation, the progression of mixed type hypoxia, severe metabolic alterations (acidosis, water and electrolyte imbalance, the exhaustion of energy and plastic resources of cells) and cells' death by the mechanisms of apoptosis or necrosis [2, 3]. Described pathological processes result in organ dysfunction and development of multiple organ failure with fatal outcome.

The hemorrhagic shock is an extreme degree of the above hemodynamic and metabolic disorders resulting from severe blood loss. It is a type of hypo-volemic shock and requires a number of conditions for development: the interval of time for the development of the body's responses; inadequate tissue perfusion; disorders of cell metabolism; the potentially lethal nature of the injury [3].

Circulatory disorders at the microcirculatory level (microcirculation) arising in acute blood loss are always severe in hemorrhagic shock and appear to be key pathogenic factors in the development of hypoxia and organ dysfunction [4, 5]. However, despite the general structural and functional manifestations (decreased in the density of perfused capillaries, the heterogeneity of perfusion, slowing of capillary blood flow, increased blood viscosity and permeability of the endothelium etc.), the microcirculatory alterations, nevertheless, have specific features depending on the vascular region of the body. These features includes the anatomicalandfunctional characteristics of blood perfusion to an organ accompanied by local vascular reactions of different intensity/direction of in response to blood loss and resulted alterations.

Part I of the review discusses general problems of the pathophysiology of acute blood loss and microcirculation. Particular changes in the regional blood flow and the microcirculation in several vascular beds will be addressed in part II of the review.

Microvasculature

The exchange of substances between blood and interstitial fluid is carried out at the level of the

Микроциркуляторное русло

Непосредственный обмен веществ между кровью и интерстициальной жидкостью осуществляется на уровне микроциркуляторного русла (МР). Условно считается, что МР составляют сосуды с диаметром менее 200 мкм. К ним относят артериолы, метартериолы с прекапиллярными сфинктерами, основной канал и истинные капилляры, венулы и артериоло-венулярные анастомозы. Однако в современном понимании микроциркуляция включает в себя: движение крови по МР (микрогемоциркуляция); движение лимфы в начальных отделах лимфатической системы; движение (обмен) интерстициальной жидкости [6]. В комплексе с паренхиматозными клетками, соединительной тканью и регуляторными механизмами приведенные компоненты образуют микро-циркуляторно-тканевую систему (синоним: функциональный элемент органа), являющуюся структурно-функциональной единицей органа [7]. В зависимости от выполняемой в системе кровообращения функции все сосуды можно подразделить на шесть групп [8]: 1) амортизирующие сосуды (аорта и крупные артерии); 2) резистив-ные сосуды (мелкие артерии и артериолы); 3) сосуды-сфинктеры (метартериолы); 4) обменные сосуды (капилляры и мелкие венулы); 5) емкостные сосуды (венулы и вены); 6) шунтирующие сосуды (артериоло-венулярные анастамозы). Как видно из приведенной классификации, на уровне микроциркуляции представлено большинство из «функциональных групп» сосудов, что еще раз подчеркивает важность этого отдела в общей системе кровообращения.

К резистивным сосудам относят главным образом концевые артерии и артериолы, имеющие относительно малый просвет и толстые стенки с развитой гладкой мускулатурой. Артериолы обладают значительно большим сосудистым сопротивлением, чем другие отделы сосудистой системы, поэтому общее сосудистое сопротивление любого органа определяется преимущественно сопротивлением артериол. Изменения степени сокращения гладкомышечных клеток этих сосудов (сосудистого тонуса) приводят к отчетливым изменениям их диаметра и, следовательно, сопротивления, что служит основным механизмом ре-

microvasculature (MV). The MV is considered to include vessels with diameters less than 200 microns. These are arterioles, metarterioles with the precapil-lary sphincters, main channel and true capillaries, venules and arteriovenous anastomoses. From the modern sense of view, the microcirculation includes: the movement of blood through the MV (microhe-mocirculation); the movement of lymph within the initial parts of the lymphatic system; the movement (exchange) of interstitial fluid [6]. In concert with parenchymal cells, connective tissue and regulatory mechanisms these components form «microcircu-lary-tissue system» (synonym: the functional element of an organ) representing a structural and functional unit of the organ [7]. Depending on the functions in the circulatory system, all the vessels can be divided into six groups [8]: 1) elastic «damping» vessels (aorta and large arteries); 2) resilient vessels (small arteries and arterioles); 3) sphincters (metarterioles); 4) exchange vessels (capillaries and small venules); 5) «capacious» vessels (venules and veins); 6) shunt vessels (arteriovenous anastomoses). As seen from the classification, the majority of «functional groups» of vessels is represented at the level of microcirculation, which once again underlines the importance of this part for the entire circulatory system.

Resilient vessels are represented by terminal arteries and arterioles of a relatively smaller diameter, with thick walls and well-developed smooth muscles. Arterioles have a significantly larger vascular resistance than other segments of the vascular system, thus the total vascular resistance of any organ is determined mainly by the resistance of the arterioles. Changes in the degree of smooth muscle contraction in these vessels (vascular tone) lead to a distinct changes in their diameter and hence in resistance. This is the principal mechanism of regulation of a volume blood flow in different vascular beds [8, 9]. Precapillary sphincters are the ultimate short parts of the precapillary arterioles (metarterioles). Their constriction or dilation determines the number of functioning capillaries, ie, exchange surface area of the capillaries.

Capillaries are the smallest blood vessels with an average diameter of 5—8 ,am, a wall thickness of 1 ^m and a length of about 0.5—0.75^m [8; 9]. There are three types of capillaries [10]: the continuous (in the muscles, connective tissue, nervous system, skin), fenestrated (in the endocrine organs, small intestine, nephrons) and sinusoidal types (in the liver, spleen and bone marrow). Structural differences between these types define different types of the exchange of various substances between blood and interstitial fluid. The capillaries and partly venules are represented by exchange vessels since the processes of filtration and diffusion occur in their walls. At the rest, blood circulates in about 25—30%

гуляции объемной скорости кровотока в различных сосудистых бассейнах [8;9]. Прекапилляр-ные сфинктеры — последние короткие отделы прекапиллярных артериол (метартериол). От их сужения или расширения зависит число функционирующих капилляров, т.е. площадь обменной поверхности капилляров.

Капилляры — мельчайшие кровеносные сосуды. Их средний диаметр — 5—8 мкм, толщина стенки — 1 мкм, а длина — около 0,5—0,75 мм [8, 9]. Выделяют 3 типа капилляров [10]: непрерывного (в мышцах, соединительной ткани, нервной системе, коже и др.), фенестрированного (эндокринные органы, тонкий кишечник, почечные клубочки) и сину-соидного типа (в печени, селезенке и костном мозге). Структурные отличия между ними определяют различия типа обмена веществ между кровью и ин-терстициальной жидкостью. Капилляры и в меньшей степени венулы — это обменные сосуды, так как именно в них осуществляются процессы фильтрации и диффузии. В покое кровь циркулирует лишь примерно в 25—30% капилляров. Плотность капилляров (как и отношение числа перфузируе-мых капилляров к неперфузируемым) в различных органах существенно варьирует, что определяется прежде всего потребностями того или иного органа в кровоснабжении в покое и при функциональной нагрузке. Увеличение числа перфузируемых капилляров имеет большое значение, так как при этом уменьшается диффузионное расстояние между капиллярами и клетками. Капилляры не способны к активным сокращениям. Соотношение между пре-и посткапиллярным сопротивлением имеет большое значение для гидростатического давления в капиллярах и, следовательно, для процессов фильтрации и реабсорбции («силы Старлинга»).

Капилляры объединяются в более крупные сосуды — венулы. Вены печени, мезентериального бассейна и подсосочкового сплетения кожи отличаются особенно большой емкостью, что определяет их важную роль в компенсаторно-приспособительных реакциях организма при гиповолемии.

Артериовенозные анастомозы (АВА), особенно хорошо развитые в коже дистальных отделов конечностей, выполняют роль шунтирующих сосудов. Когда эти сосуды открыты, кровоток через капилляры либо уменьшается, либо полностью прекращается [8].

На уровне микроциркуляции возникает ряд феноменов, которые не характерны для других отделов сосудистой системы. Примером может быть эффект Фареуса-Линдквиста, с помощью которого при прохождении крови по капиллярам ее эффективная вязкость снижается вдвое. Этот эффект объясняется ориентацией эритроцитов вдоль оси микрососуда, в результате чего вокруг них образуется оболочка из плазмы с низкой вязкостью, относительно которой легко скользят клетки крови [11].

The capillary density, as well as the percentage of perfused capillaries in different organs, considerably varies that is primarily due by the needs of an organ in the blood supply at rest and during functional load. An increase in the number of perfused capillaries is of great value since it reduces the diffusion distance between the capillaries and cells. The capillaries are not capable of active contraction. The ratio between the pre- and post capillary resistance is important for the hydrostatic pressure in the capillaries and, hence, for the processes of filtration and reabsorption («starling forces»).

The capillaries are combined into larger vessels — venules. The veins of liver, mesenteric region and the papillary plexus of skin characterized by a particularly large capacity that determines an important role of these vessels in the compensatory reactions of the body during hypovolemia.

Arteriovenous anastomoses (AVA), particularly well developed in the skin of the distal extremities, serve as shunt vessels. When these blood vessels open, the blood flow through the capillaries either decreases or ceases completely [8].

There are a number of phenomena at the level of microcirculation that are not typical for other parts of the circulatory system. For example, the Fahraeus-Lindqvist effect is an effect where the passage of blood through capillaries is accompanied by a decrease in its effective doubled viscosity. This effect is explained by the orientation of red blood cells along the axis of microvessels. As a result, the plas-mal sheath with a low viscosity is formed around the axial flow and red blood cells easily slide relatively to the sheath [11]. Other examples include intravascu-lar erythrocyte aggregation, a temporary block of junctional sections of microvessels by leukocytes, or the appearance of plasma capillaries filled with only plasma [6].

Tissue perfusion and oxygenation are ultimately determined by the rate of capillary blood flow (convective substance transfer) and the density of the functioning capillaries, ie, their common exchange surface (diffusive substance transfer)[12]. These two factors are determined not only by the state of systemic hemodynamics, but mainly by the tone of precapillary vessels (arterioles). Through the regulation of vascular tone, the cardiac output is distributed between/inside organs in accordance with metabolic needs of a particular tissue region at a given time, for example, during functional hyper-emia. In hypoperfusion state the adequate oxygenation of a separate cell depends on the delivery of oxygen by microvessels as well as on the intercapillary distance (the latter increases with decreasing the density of functioning capillaries) and the critical oxygen diffusion distance (the maximum distance the mitochondria can be away from an oxygen source) [13].

Перфузия и оксигенация тканей в конечном итоге определяются скоростью капиллярного кровотока (перенос веществ по механизму конвекции) и плотностью функционирующих капилляров, т.е. их общей обменной поверхностью (перенос веществ по механизму диффузии) [12]. Эти два фактора определяются не только состоянием системной гемодинамики, но и, в значительной степени, тонусом прекапиллярных сосудов (арте-риол). Посредством регуляции сосудистого тонуса сердечный выброс «распределяется» между органами и внутри органов в соответствии с метаболическими потребностями конкретного региона ткани в данный момент времени, например, при функциональной гиперемии. В условиях гипоперфузии адекватная оксигенация конкретной клетки зависит от доставки кислорода по микрососудам, а также от таких факторов как расстояние между капиллярами (увеличивается при снижении плотности функционирующих капилляров) и критическое расстояние для диффузии кислорода (максимальное расстояние от микрососуда до митохондрии клетки при диффузии кислорода) [13].

Регуляция сосудистого тонуса

Сосудистым тонусом называют постоянное напряжение стенок кровеносных сосудов, противодействующее их растяжению под воздействием давления крови [14].

Для описания движения крови по системе сосудов применимо основное уравнение гидродинамики, которое устанавливает взаимосвязь между объемной скоростью кровотока в сосуде 0, градиентом давления в этом сосуде ДР и его гидродинамическим сопротивлением К: 0=ДР/К. Объемная скорость кровотока 0 отражает кровоснабжение (перфузию) того или иного органа за определенный промежуток времени и обычно измеряется в мл/мин/100 г ткани [11]. Гидродинамическое сопротивление сосуда зависит от нескольких факторов, взаимосвязь между которыми определяется законом Хагена-Пуазейля: 0=ДР(пг4)/(8пЬ), где г — внутренний радиус сосуда, п — вязкость жидкости, Ь — длина сосуда. Из этого уравнения и основного закона гидродинамики следует, что К=(8пЬ)/(пг4), т. е. сопротивление сосуда обратно пропорционально четвертой степени его радиуса. Именно поэтому сосудистый тонус имеет ключевое значение в регуляции объемного регионарного кровотока, поскольку от него зависит величина радиуса сосудов [9].

The regulation of vascular tone

Vascular tone is a constant tension of blood vessel walls, which counteracts stretching them under the influence of blood pressure [14].

To describe the movement of blood through the vascular system, the the basic equation of hydrodynamics is applicable. It establishes the relationship between the volumetric blood flow rate in the vessel (Q), the pressure gradient in the vessel (AP) and its hydrodynamic resistance (R): Q=AP/R. The volumetric blood flow rate (Q) reflects the perfusion of an organ for a certain period of time and is usually measured in ml/min/100g of tissue [11]. The hydro-dynamic resistance of a vessel depends on several factors, the relationship between which is determined by the Hagen-Poiseuille law: Q=AP(nr4)/(8nL), where: r is the inner radius of vessel, n is the viscosity of fluid, L is the length of vessel. From this equation and the basic equation of hydrodynamics follows that R=(8nL)/(nr4), i.e. vascular resistance is inversely proportional to the radius of the vessel by the factor of four. Vascular tone is critical for the regulation of regional blood flow since it determines the radius of vessels [9].

Vascular tone value is determined by the level of individual smooth muscle cells activity in a given region of the vasculature. Vascular smooth muscle has the ability to maintain the active tension over a long period of time [9]. Most vessels have some amount of pacemaker cells that are spontaneously depolarized and excite the neighboring cells. Shortening pacemaker cells are not dependent on the innervation of the vessel and observed even after denervation. Due to this phenomenon the vessel walls are at a certain tension even at rest (myogenic basal tone). The tension of most vessels at rest is determined not only by the basal tone, but also by contracting of smooth muscle cells under the influence of vasoconstrictor impulses arriving on sympathetic nerves. Such total tension of vessels at rest is called the neurogenic tone [8, 9].

At rest, the regional blood flow is determined by the metabolic needs of a particular organ: the higher needs, the more volumetric blood flow in the organ. Adaptive blood flow changes in the peripheral vessels are due to both the local and systemic (humoral and nervous) factors, which in varying degree affect the vascular smooth muscles tone in different organs.

Metabolic and intrinsic myogenic mechanisms are conventionally attributed to the local factors of vascular tone regulation (autoregulation). Both of these mechanisms are functioning on a base of principle of negative feedback: the deviation of any parameter of homeostasis (eg, decreased tissue pO2 or the vascular wall tension) triggers a chain of reactions aimed at the return of deviated parameters to their

Величина сосудистого тонуса определяется уровнем активности отдельных гладкомышечных клеток того или иного региона сосудистой сети. Гладкая мускулатура сосудов обладает способностью сохранять активное напряжение на протяжении длительного периода времени [9]. В большинстве сосудов есть некоторое количество гладкомышечных клеток-пейсмекеров, которые спонтанно деполяризуются и возбуждают соседние клетки. Сокращения клеток-пейсмекеров не зависят от иннервации сосуда и наблюдаются даже после его денервации. Благодаря этому явлению стенки сосудов даже в покое находятся в состоянии некоторого напряжения (миогенный базальный тонус). Напряжение большинства сосудов в покое определяется не только базальным тонусом, но и сокращением гладкомышечных клеток под влиянием сосудосуживающих импульсов, поступающим по симпатическим нервным волокнам. Это суммарное напряжение сосудов называется тонусом покоя [8].

В покое регионарный кровоток определяется метаболическими потребностями того или иного органа: чем они выше, тем больше объемная скорость кровотока в этом органе. Приспособительные изменения кровотока в периферических сосудах обусловлены как локальными, так и системными (гуморальными и нервными) факторами, влияние которых на гладкую мускулатуру сосудов в разных органах различно.

К местным факторам регуляции (ауторегуля-ции) сосудистого тонуса условно относят метаболические и собственно миогенные механизмы. Общим для них является принцип функционирования по механизму отрицательной обратной связи: отклонение какого-либо параметра гомеостаза (например, снижение рО2 в тканях или напряжения сосудистой стенки) запускает цепь реакций, направленных на возвращение отклонившегося параметра к исходным значениям. Так, метаболическая ауторегуля-ция периферического кровотока заключается в том, что гипоперфузия региона ткани приводит к снижению рО2 и накоплению продуктов метаболизма в этом регионе, которые обладают сосудорасширяющим действием. Развивающаяся в этих условиях ва-зодилатация ведет к увеличению объемной скорости кровотока, а, следовательно, к увеличению доставки кислорода и «вымыванию» накопившихся метаболитов. При этом метаболические сосудорасширяющие влияния часто доминируют над нервными сосудосуживающими эффектами и в некоторых случаях полностью подавляют их [8]. Прямое вазо-дилатирующее действие на гладкомышечные клетки сосудов (прежде всего артериол) оказывают следующие факторы: гипоксемия, респираторный (гиперкапния) и метаболический (гиперлактате-мия) ацидоз, гиперкалиемия и гиперосмолярность внеклеточной жидкости. Thus, the metabolic autoregulation of peripheral blood flow consists in that the hypoperfu-sion of tissue region reduces pO2 and leads to the local accumulation of metabolic products with vasodilatation properties. Vasodilatation, which develops in these conditions, leads to increasing in the volumetric flow rate and, consequently, to increased oxygen delivery and «washing out» of locally accumulated metabolites. The metabolic vasodilator effects often dominate over neural vasoconstrictor effects and in some cases completely suppress them [8]. The following factors have a direct vasodilating effect on the smooth muscle cells of the vessels (mostly arterioles): hypoxemia, respiratory (hypercapnia) and metabolic acidosis (hyperlactatemia), hyperpotassemia and the hyperosmolarity of the extracellular fluid. Pyruvate has a relatively weak vasodilating effect, whereas adenosine triphosphate (ATP), adenosine diphos-phate (ADP), adenosine monophosphate (AMP) and adenosine possess stronger effect. Currently, it is not exactly known, which of these metabolic factors more impacts the arteriolar tone in metabolic blood flow autoregulation.

The special role of the endothelium in the regulation of vascular tone should be highlighted, too. Since the eighties of XX century, the endothelium had been actively studied as an important factor in the regulation of local and systemic blood circulation, as well as a key element of the pathogenesis of various cardiovascular (hypertension, atherosclerosis) and other diseases (eg, inflammatory diseases) [9]. Blood vessels react completely differently to certain impacts in the absence of the endothelial lining. For example, acetylcholine causes vasodilation of the intact vessel, however, it causes vasoconstriction in the vessel devoided of endothelial lining [15]. It has been found that endothelial cells respond to different stimuli by producing of endothelium-derived relaxing factor (EDRF), which reduces the tone of the underlying vascular smooth muscle. This substance was subsequently identified as nitric oxide (NO). [16]. With an increase in concentration of intracellular Ca2+, NO is synthesized from L-arginine by the enzyme NO-syn-thase. The NO is a small fat-soluble molecule, so it easily diffuse into the adjacent smooth muscle cells, causing their relaxation by stimulating the production of cyclic guanosine monophosphate (cGMP).

Acetylcholine, bradykinin, vazointestinalny peptide (VIP) and substance P bind to specific membrane receptors on endothelial cells and activate Ca2+ channels, thus stimulating the production of NO by the endothelium. Another physiologically significant factor for NO synthesis is the shear stress, which is determined by the rate of blood flow and stimulates the production of NO by endothelial cells, mainly due to the activation of stretch-sensitive Ca2+ channels [17]. It is believed that in norm endothelial cells always produce a certain amount of NO, which in

С 80-х годов XX века эндотелий активно изучается как важный фактор регуляции местного и системного кровообращения, а также ключевое звено патогенеза многих сердечно-сосудистых (артериальная гипертензия, атеросклероз) и других заболеваний (например, воспалительных) [9]. Кровеносные сосуды реагируют совершенно иначе на определенные воздействия при отсутствии эндотелиальной выстилки. Например, ацетилхолин вызывает вазодилатацию интактного сосуда, но в сосуде, лишенном эндо-телиальной выстилки, вызывает вазоконстрик-цию [15]. Было выявлено, что эндотелиоциты реагируют на различные стимулы продукцией локального фактора (эндотелиальный релакси-рующий фактор, англ. БЭКР), который снижает тонус лежащей под эндотелием гладкой мускулатуры сосуда. Это вещество в последующем было идентифицировано как N0 (окись азота) [16]. При увеличении внутриклеточной концентрации Са2+, N0 синтезируется из Ь-аргинина под действием фермента N0-синтазы. Поскольку N0 — это небольшая жирорастворимая молекула, она легко диффундирует в соседние гладкомышеч-ные клетки, вызывая их релаксацию, за счет стимулирования продукции циклического гуано-зинмонофосфата (цГМФ).

Ацетилхолин, брадикинин, вазоинтести-нальный пептид (ВИП) и субстанция Р, связываясь со специфическими рецепторами на мембране эндотелиоцитов и активируя посредством этого Са2+ каналы, стимулируют продукцию N0 эндотелием. Другим физиологически значимым фактором синтеза N0 является определяемое уровнем кровотока напряжение сдвига, которое стимулирует продукцию эндотелиальными клетками N0 преимущественно за счет активации чувствительных к растяжению Са2+ каналов [17]. In recent years, increasing attention has been attracted to studies of integrating factors of metabolic blood flow regulation as a conducted vasodilatation and release of ATP by red blood cells in response to decreasing oxyhemoglobin saturation [18]. The red blood cells are seen as a «sensors» of hypoxia. The amount of ATP released by them is proportional to the degree of oxyhemoglobin desaturation in the venous part of microvasculature. ATP binds to purinergic receptors on endothelium and causes the vasodilation that conducts upstream via the capillaries to the arte-rioles and small arteries, thereby regulating blood flow distribution in the region [19]. Conducted vasodilation is based on the NO secretion by the endothelium in response to chemical (ATP) and mechanical (shear stress) exposure, whereas the transmission of information takes place along the vascular wall via the gap junctions of cells, which involve changes in membrane potential and ionic concentrations. Conducted vasodilatation is carried out via both endothelial and smooth muscle cells of vascular wall.

In addition to the local metabolic impacts on vascular tone, many specific substances with cardiovascular effects have been defined, which are acting primarily via auto- or paracrine mechanisms. [9]. Histamine, the polypeptides bradykinin and kallidin possess a pronounced vasodilating patterns and capacity to increase the permeability of capillaries. Prostaglandins (PG) have different cardiovascular effects and have a pathophysiological significance in tissue injury and inflammation. PGA1 and PGA2 cause the dilation of mesenteric arterioles, reduce blood pressure, increase renal blood flow and urine output. PGF narrows the blood vessels and increases blood pressure. Prostacyclin inhibits and thrombox-ane enhances platelet aggregation and haemostatic vasoconstriction [8].

Some vessels (renal, brain, coronary) are able to maintain a constant volumetric blood flow during blood pressure fluctuations that is the base of the myogenic autoregulation of peripheral blood flow. This phenomenon is manifested by the contraction of vascular smooth muscle with an increase of perfusion pressure and the relaxation of the muscle with a decrease in pressure (Bayliss effect). This mechanism contributes to the stabilization of the blood supply to the organ in a fairly wide range of perfusion pressure changes. The myogenic autoregulation is independent of autonomic influences, so it is retained even after the transection of the vasomotor nerves [11].

The main role in the neural regulation of vascular tone belongs to the sympathetic adrenergic

В основе проводной вазодилатации лежит секреция эндотелием N0 в ответ на химические (АТФ) и механические (напряжение сдвига) воздействия, при этом передача информации вдоль сосудистой стенки в виде изменения мембранного потенциала и проницаемости ионных каналов происходит через клеточные щелевые контакты (англ. gap junctions). Проводная вазодилатация осуществляется как через эндотелиоциты, так и через гладкомышечные клетки сосудистой стенки.

Помимо местных метаболических воздействий на сосудистый тонус установлены многие специфические химические вещества, обладающие сосудистыми эффектами, и действующие чаще всего по ауто- или паракринному механизму [9]. Гиста-мин, полипептиды каллидин и брадикинин обладают выраженным сосудорасширяющим эффектом и увеличивают проницаемость капилляров. Простаг-ландины (Pg) обладают разными сосудистыми эффектами и имеют патофизиологическое значение при повреждении тканей и воспалении. PgA1 и PgA2 вызывают расширение артериол чревной области, снижают АД и увеличивают почечный кровоток и диурез. Pg группы F суживают сосуды и повышают АД. Простациклин тормозит, а тромбоксан усиливает агрегацию тромбоцитов и вызывает ге-мостатическую вазоконстрикцию [8].

Некоторые сосуды (почечные, мозговые, коронарные) способны поддерживать постоянную объемную скорость кровотока при колебаниях АД, в основе чего лежит миогенная ауторегуляция периферического кровотока. Она обусловлена сокращениями гладких мышц сосудов при повышении перфузионного давления и их расслаблением при снижении давления (эффект Бейлиса). Этот механизм способствует стабилизации кровоснабжения органа в достаточно широком диапазоне изменений перфузионного давления. Миогенная ауторегуляция не зависит от вегетативных влияний, поэтому она сохраняется даже после перерезки сосудодвигательных нервов [11].

Основная роль в нервной регуляции сосудистого тонуса отводится симпатическим адре-

vasoconstrictor fibers, which release norepineph-rine as a neurotransmitter. Increased efferent output in the fibers of this type leads to an increase in active stress of vascular muscles. Small arteries and arterioles of the skin, kidneys and mesenteric region are richly innervated by the vasomotor fibers. In the brain and skeletal muscles these vessels are innervated relatively weak [8]. The degree of contraction of vascular muscle depends directly on the impulsation frequency of vasomotor efferent nerves. In the absence of tonic vasoconstrictor nerve impulses (denervation), the vasodilation is limited by the basal myogenic vascular tone. Thus, the total vascular tone may be altered relative to resting neurogenic tone both towards vasoconstriction and vasodilation.

The parasympathetic cholinergic vasodilator fibers innervate blood vessels of the external genitalia. This same type of nerve fibers innervate pial and coronary vessels, but their impact on the tone of the arte-rioles in these organs seem to be not essential [9].

Humoral (hormonal) regulation is represented by dozens of biologically active substances, which affect vascular tone and the permeability of vessel wall through binding with specific membrane receptors on endothelial and smooth muscle cells. The catecholamines (epinephrine and norep-inephrine), secreted by the adrenal medulla, circulate in blood in small concentrations under physiological conditions. Vascular responses to these two substances are different. The final effect of cate-cholamines on vascular wall (vasoconstriction or vasodilation) depends on the amount and the ratio of a- and ^-adrenergic receptors on vascular muscle cells. The excitation of a-adrenergic receptors is accompanied by vasoconstriction, while the stimulation of ^-adrenergic receptors lead to vasodilation. Norepinephrine acts primarily on a-adrenergic receptors and epinephrine acts on both a- and ^-adrenergic receptors. Both types of the receptors are presented in most vessels. Epinephrine causes vasoconstriction of a vessel, if a-adrenergic receptors are dominated in the vessel, and it causes vasodilation, if ^-adrenergic receptors are dominated. Epinephrine causes vasodilation at low (physiological) concentrations and vasoconstriction in high concentrations [8]. Such features of catecholamines action on vascular tone partly explain the differently directed blood flow changes in a number of organs during hemorrhage (the centralization of circulation).

Other vasoactive hormones are the renin-angiotensin-aldosterone system, antidiuretic hormone (vasopressin) and serotonin. Their role in the regulation of vascular tone is negligible in physiological conditions, but they have a leading role during blood loss and fluid and electrolyte alterations, that will be discussed in more detail hereinafter.

Таким образом, результирующий сосудистый тонус может быть изменен (относительно тонуса покоя) как в сторону вазоконстрикции так и в сторону вазодилатации.

Парасимпатические холинергические сосудорасширяющие волокна иннервируют сосуды наружных половых органов. Этот же тип волокон иннервирует сосуды мягкой мозговой оболочки головного мозга и коронарные сосуды, однако их воздействие на тонус артериол этих органов несущественно [9].

Гуморальная (гормональная) регуляция представлена десятками биологически активных веществ, которые посредством специфических рецепторов на мембранах эндотелиоцитов и гладких миоцитов влияют на сосудистый тонус и проницаемость стенки сосуда. Секретируемые мозговым слоем надпочечников катехоламины (адреналин и норадреналин) в физиологических условиях циркулируют в крови в небольших концентрациях. Реакция сосудов на эти два вещества различается. Конечный эффект влияния катехоламинов на ми-оциты сосудистой стенки (вазоконстрикция или вазодилатация) зависит от количества и соотношения в ней а- и в-адренорецепторов. Возбуждение а-адренорецепторов сопровождается вазоконст-рикцией, а в-адренорецепторов — вазодилатацией. Норадреналин действует преимущественно на а-адренорецепторы, а адреналин — и на а- и на в-ад-ренорецепторы. В большинстве сосудов представлены оба типа рецепторов. Если в сосудах преобладают а-адренорецепторы, то адреналин вызывает их вазоконстрикцию, а если в-адреноре-цепторы, то дилатацию. В низких (физиологических) концентрациях адреналин вызывает вазоди-латацию, а в высоких концентрациях — вазоконстрикцию [8]. Такими особенностями действия катехоламинов на сосудистый тонус можно частично объяснить разнонаправленные изменения кровотока в ряде органов при кровопотере (централизация кровообращения).

Compensatory and adaptive reactions of the organism in acute blood loss

Besides reducing the CBV (hypovolemia), a decrease in the oxygen capacity of the blood (anemia) is another important factor in pathogenesis of acute blood loss. As a result, oxygen delivery (DO2) to the tissues of the body decreases [20—21]. Circulatory disorders and tissue hypoxia are responsible for the activation of compensatory and adaptive reactions of the organism. Depending on the timing, the following stages of activation of compensatory processes during blood loss become usually evident [1]; they sequentially include the alterations within the ardiovascular system hydremia, protein synthesis and bone marrow hematopoiesis.

The very first protective reaction of the organism is blood coagulation and thrombotic process. Thrombus formation at the site of vascular injury (local haemostasis) helps to stop bleeding. However, in the cases of acute severe blood loss and extensive traumatic tissue injuries, the excessive blood coagulation becomes an independent pathogenic factor and can lead to the development of disseminated intravascular coagulation (DIC) [22].

Cardiovascular compensation is developing from the very first minutes of bleeding. It is manifested by stimulation of heart activity and changes in vascular tone. These changes are largely due to the reflex activation of the sympathetic system with the participation of baro- and chemoreceptors of the aortic arch and carotid sinus [1, 3]. It is known that the tonic afferent impulses from vascular baroreceptors inhibit the «pressor» part of the cardiovascular center in the medulla oblongata, whereas the activation of chemoreceptors (during hypoxia) stimulates the cardiovascular center. Arterial hypotension reduces impulse activity of the baroreceptors of the carotid sinus and aortic arch, and the developing circulatory hypoxia activates the vascular chemoreceptors. As a result, the «pressor» activity of the medullar cardiovascular center is increased, which leads to intensification in the tonic impulse activity of the sympathetic nerves that innervate the heart and vessels. Catecholamine secretion in the adrenals is also enhanced [11].

The stimulation of the heart is manifested in the form of tachycardia and increased myocardial contractility, that under the conditions of hypov-olemia and decreased venous return is aimed to maintaining an optimal level of CO. Already at an early stage of blood loss (10—15% of CBV) the tone of capacitive vessels (veins and venules) of the skin, subcutaneous fat and abdominal organs increases as a reflex reaction, which is accompanied by the venous vasoconstriction and mobilization of deposited blood into the bloodstream [23, 24]. Further activation of the sympathetic nervous system leads to an

Компенсаторно-приспособительные реакции организма при острой кровопотере

Помимо снижения ОЦК (гиповолемии), другим важным патогенетическим фактором при острой кровопотере является снижение кислородной емкости крови (анемия) и, как следствие этого, снижение доставки кислорода (Э02) к тканям организма [20, 21]. Нарушения кровообращения и гипоксия тканей обусловливают активацию компенсаторно-приспособительных реакций организма. В соответствии со сроками активации данных реакций, условно выделяют следующие стадии развития процессов компенсации крово-потери [1]: сердечно-сосудистую, гидремическую, белковосинтетическую и костномозговую.

Самой первой защитной реакцией организма является активация свертывающей системы крови и общепатологического процесса тромбо-образования. Образование тромба в месте повреждения сосуда (местный гемостаз) способствует остановке кровотечения. Однако, при острой массивной кровопотере и травматических повреждениях большого объема тканей, чрезмерная гиперкоагуляция крови становится самостоятельным патогенным фактором и может привести к развитию диссеминированного внутрисосудис-того свертывания (ДВС) [22].

Сердечно-сосудистая компенсация развивается с первых минут кровотечения. Проявляется она в стимуляции работы сердца и изменении сосудистого тонуса, которые в значительной степени обусловлены рефлекторной активацией сим-патоадреналовой системы, в том числе с участием баро- и хеморецепторов дуги аорты и каротидно-го синуса [1, 3]. Известно, что тоническая афферентная импульсация от сосудистых барорецеп-торов оказывает тормозное, а активация хеморецепторов (при гипоксии) — стимулирующее влияние на «прессорный» отдел сердечно-сосудистого центра в продолговатом мозге. Артериальная гипотензия уменьшает импульсную активность барорецепторов каротидного синуса и дуги аорты, а развивающаяся циркуляторная гипоксия активирует сосудистые хеморецепторы. В результате повышается «прессорная» активность сердечно-сосудистого центра продолговатого мозга, вследствие чего усиливается тоническая импульсная активность симпатических нервов, иннервирующих сердце и сосуды, а также стиму-

increase in the neurogenic tone of arterioles in the skin, subcutaneous fat, abdominal organs, kidneys and muscles. In these organs arteriolar vasoconstriction develops, regional vascular resistance increases and as a result, the regional blood flow decreases, i.e. the hypoperfusion of the organs occurs. In contrast, the coronary and cerebral arterioles undergo vasodi-lation, which is due to the combined effect of factors regulating vascular tone (metabolic, myogenic and neurogenic). Therefore, the blood supply of the heart and brain is maintained at the same level or only slightly decreased that is a manifestation of the ability of these organs to autoregulate blood flow across a wide range of blood pressure changes [11]. The described changes of vascular tone are known as the «centralization of circulation». During hypovolemia the physiological significance of the centralization of circulation comes from the redistribution of blood flow from the tissues and organs, that are relatively resistant to hypoxic damage, to vital organs, the functioning of which depends on an uninterrupted blood supply.

At the stage of cardiovascular compensation of blood loss, the hypovolemia is «normocytemic» (with Hb and Ht values remaining within the normal ranges), and acute hemorrhagic anemia is therefore still latent [1].

During the rapid progression of hemorrhage the adaptive hyperventilation occurs as a reflex reaction. The aim is to increase venous return to the heart via the suction effect during inhalation. This effect leads to respiratory alkalosis, which partially compensates for the metabolic acidosis that develops as a result of circulatory hypoxia [23, 24].

Hydremic compensation, aimed at restoring the lost volume of circulating plasma, is also launched from the first minutes of hemorrhage [25], but fully develops within 1—2 days [24, 26]. There are several mechanisms for its implementation. The main one is autohemodilution, which develops due to the mobilization of extracellular fluid from the interstitial space into the bloodstream. Hypovolemia and arteri-olar spasm lead to a decrease in hydrostatic pressure in capillaries, and pericapillary fluid moves into the lumen of capillaries by the action of Starling forces (the differences of hydrostatic and oncotic pressures inside and outside the vessel). The described mechanism is illustrated by data demonstrating that from the first five minutes of hemorrhage, a volume of fluid correspondent to 10—15% of normal CBV is moved to the bloodstream [23].

At the same time, the plasma osmotic pressure increases rapidly as a result of hyperglycemia and accumulation of carbohydrates and fat catabolism products. This mechanism activates a flow of fluid from the intracellular compartment into the extracellular space (along the gradient of osmotic pressure), into the lymph vessels and then further in the

Дальнейшая активация симпатического отдела нервной системы приводит к повышению нейрогенного тонуса артериол кожи, подкожной жировой клетчатки, органов брюшной полости, почек и мышц. В указанных органах развивается вазоконстрикция артериол, повышение регионарного сосудистого сопротивления и, как следствие, снижение регионарного объемного кровотока, т.е. возникает их гипоперфузия. Напротив, артерио-лы коронарного и церебрального сосудистых бассейнов подвергаются вазодилатации, которая обусловлена совокупным действием факторов регуляции сосудистого тонуса (метаболические, миогенные, нейрогенные). В результате кровоснабжение сердца и мозга поддерживается на прежнем уровне или снижается незначительно, что является проявлением выраженной способности этих органов к ауторегуляции кровотока в широком диапазоне изменения АД [11]. Описанные изменения сосудистого тонуса известны как феномен «централизации кровообращения», физиологическое значение которого в условиях ги-поволемии заключается в перераспределении кровотока от тканей и органов, относительно устойчивых к гипоксическому повреждению, к жизненно важным органам, функционирование которых зависит от бесперебойного кровоснабжения.

На стадии сердечно-сосудистой компенсации кровопотери гиповолемия является нормо-цитемической (концентрационные показатели НЬ и Н остаются в пределах нормальных значений), а острая постгеморрагическая анемия поэтому носит еще «скрытый» характер [1].

При быстром прогрессировании кровопотери рефлекторно возникает приспособительная гипервентиляция легких, направленная, в том числе, на увеличение венозного возврата присасывающим действием грудной клетки. Это ведет к респираторному алкалозу, который частично компенсирует развивающийся в результате циркуляторной гипоксии метаболический ацидоз [23, 24].

Гидремическая компенсация, направленная на восстановление утраченного объема циркулирующей плазмы, также запускается с первых минут кровопотери [25], но полноценное развитие получает в течение 1—2 суток [24, 26]. Выделяют несколько механизмов ее реализации. Основным

blood [27]. Within an hour from the start of hemorrhage this osmotic mechanism is capable to provide up to 60% of the total fluid volume when entering the bloodstream [25]; it also facilitates the «drainage» of interstitial fluid protein through the lymphatic system into the blood. The degree and rate of autohemodilution depends on the initial hydration of the organism and the quantity of fluid in the intestinal lumen. For example, an experimental animal study by Darlington et al. [28] showed that in the posthemorrhagic period the degree of Ht decrease was less in the rats fasted during 24 hours, compared to the animals that had been provided with food and water ad libitum. The compensatory mechanism was less effective in fasted animals. Another important mechanism of the hidremic compensation is an increase of antidiuretic hormone (ADH) secretion, as well as the activation of renin-angiotensin-aldosterone system [1]. ADH release by the hypothalamus increases in response to changes in afferent impulses from vascular baroreceptors (during hypovolemia and hypotension) and osmorecep-tors of the hypothalamus that results in increasing the plasma osmolality [29]. The ADH stimulates the reabsorption of water from the collecting ducts of the kidneys causing renal arteriolar vasoconstriction, which leads to a decrease in the glomerular filtration rate and urine output. Hypoperfusion of the juxta-glomerular complex of kidneys stimulates the production of renin, followed by the synthesis of angiotensin II (strong vasoconstrictor) and aldos-terone. The latter stimulates sodium reabsorption in the renal tubules and increases the osmotic-dependent fluid flow into the bloodstream.

Hypoproteinemia is developing as a result of replenishment of lost plasma volume with water and electrolytes from the interstitium. Lymph mobilization and protein synthesis activation (including clotting factors) in the liver contribute to the restoration of plasma protein deficiency usually within 1.5—3 weeks).

The described compensatory reactions, as well as fluid resuscitation with crystalloid and colloid solutions, lead to the development of hypo- or oligocitemic normovolaemia. This is manifested by decreasing Hb and Ht values per blood volume unit and therefore by blood oxygen capacity reduction. The circulatory, hemic and respiratory hypoxia stimulates the production of erythropoietin in the juxta-glomerular apparatus of the kidneys, which in turn enhances erythropoiesis in bone marrow. It is considered that the bone marrow compensation develops on the 4—5th days after the blood loss, and an increase in reticulocyte content in the peripheral blood (normal range is 5—10 cells per 1000 red blood cells) is the criterion of bone marrow regenerative capacity [24]. Several studies, however, have demonstrated that the ratio of various populations of red blood cells changes in the first minutes and hours of

Ясно, что степень и скорость развития аутогемо-дилюции зависит от исходной гидратации организма и количества жидкости в просвете кишечника. Так, в экспериментальном исследовании Darlington D. N. и соавт. [28] показано, что в постгеморрагическом периоде степень снижения ге-матокрита была меньше у крыс, предварительно голодавших в течение 24 ч, по сравнению с животными, не лишавшихся воды и пищи. Другим важным механизмом гидремической компенсации является повышение секреции антидиуретического гормона (АДГ), а также активация ренин-ан-гиотензин-альдостероновой системы [1]. Секреция АДГ гипоталамусом увеличивается рефлекторно в ответ на изменение афферентной импульсации от сосудистых барорецепторов (ги-поволемия и гипотензия) и осморецепторов самого гипоталамуса (повышение осмолярности плазмы) [29]. АДГ увеличивает реабсорбцию воды из собирательных трубочек почек, а также обладает сосудосуживающим действием, вызывая вазокон-стрикцию почечных артериол, что приводит к снижению клубочковой фильтрации и темпа диуреза. Гипоперфузия юкстагломерулярного аппарата почки стимулирует выработку ренина, с последующим синтезом ангиотензина II (сильный вазоконстриктор) и альдостерона. Последний стимулирует реабсорбцию ионов натрия в почечных канальцах и усиливает осмотически зависимый ток жидкости в сосудистое русло.

acute blood loss[25, 30] . This is manifested by changing the size (anisocytosis) and shape (poikilo-cytosis) of red blood cells. In particular, the average red blood cell diameter increases (releasing relatively «young» cells into the bloodstream), but during blood loss decompensation the percentage of red blood cells of smaller diameter («aging» of blood) and modified forms (stomatocytes, echinocytes et al.) increases [31]. In severe trauma the degree of anisocytosis and poikilocytosis, as well as changes in the nanostructure of erythrocyte membrane surface, dependon the amount of blood loss [32].

Along with increased erythropoiesis, leukocy-tosis with a shift to the left and increased platelet count are observed in peripheral blood. At the same time, leukopenia and thrombocytopenia may represent the early signs of bleeding [24]. Depending on the blood loss volume (assuming no blood transfusions) the globular blood volume is restored during 1—2 months. The acute hemorrhagic anemia according to hematological parameters becomes normocyt-ic, normochromic and hyper/normoregenerative.

Hormonal changes during acute blood loss

As mentioned in the previous section, the activation of the renin-angiotensin-aldosterone system and the increased secretion of ADH significantly contribute to reducing the renal loss of water and sodium during hypovolemia. Angiotensin II is a strong vasoconstrictor, at the same time it stimulates the thirst center of the hypothalamus and induces the characteristic symptom of acute blood loss, the strong thirst [33]. The activities of a number of other hormonal systems are also significantly changing. Frequently phasic in nature, these changes are involved in compensatory reactions of the body aimed at mobilization of resources and maintenance of the integrity of the organism during the stress response to acute blood loss [27, 34].

Simultaneously with the activation of the sympathetic nervous system, the epinephrine secretion in the adrenal medulla and the norepinephrine release in peripheral tissues are sharply increasing. Under these conditions, the catecholamines provide complex hemodynamic and metabolic effects. Peripheral vascular resistance, CO and BP is increasing. Metabolic effects include the stimulation of renin secretion, hyperglycemia (mainly as a result of acceleration of hepatic glycogenolysis) and lipolysis activation.

Hyperglycemia in the early stages of acute blood loss is also caused by a decrease in insulin secretion by pancreatic /3-cells and an increase in glucagon secretion by pancreatic a-cells. Hypoglycemia develops in decompensated hemor-rhagic shock and depletion of energy resources.

Однако, есть работы [25, 30], в которых показано, что соотношение различных популяций эритроцитов меняется уже в первые минуты и часы острой кровопотери, что проявляется изменением размера (анизоцитоз) и формы (пойкилоци-тоз) красных кровяных клеток. В частности, характерно увеличение среднего диаметра эритроцитов (выброс в кровоток относительно «молодых» клеток), а при декомпенсации кровопотери увеличивается процентное содержание эритроцитов малого диаметра («старение» крови) и измененных форм клеток (стоматоцитов, эхиноцитов и др.) [31]. При тяжелой травме выраженность анизо- и пойкилоцитоза, а также изменений поверхностной наноструктуры мембран эритроцитов зависит от объема кровопотери [32].

Наряду с усилением эритропоэза, в периферической крови отмечается лейкоцитоз со сдвигом лейкоцитарной формулы влево, увеличивается содержание тромбоцитов, при этом ранними признаками кровотечения могут быть лейкопения и тромбоцитопения [24]. В зависимости от объема кровопотери и при отсутствии гемотранс-фузий глобулярный объем крови восстанавливается в течение 1—2 месяцев, а острая постгеморрагическая анемия по гематологическим показателям является нормоцитарной, нормо-хромной и гипер-/норморегенераторной.

Гормональные изменения при острой кровопотере

Активация ренин-ангиотензин-альдостеро-новой системы и увеличение секреции АДГ, как уже было сказано, играют ключевую роль в снижении почечных потерь воды и натрия при гипо-волемии. Ангиотензин II, будучи сильным вазо-

Blood loss and trauma stimulate the production of adrenocorticotropic hormone (ACTH) in the pituitary gland, which in turn stimulates the production of glucocorticoids (cortisol) in the adrenal cortex. The main effects of glucocorticoids are increased lipolysis, gluconeogenesis, glycogenolysis in the liver and muscles, the stabilization of lysosomal membranes, decreased capillary permeability and immunosuppression. As a result of describedhormon-al changes and increased catabolism, the blood concentration of amino acids, fatty acids and plasma osmolality increase.

The concentration and ratio of reproductive steroid hormones in blood plasma are changing in hemorrhagic shock and in other critical states. Gender differences in the dynamics of these parameters are observed in the postresuscitation period that may affect the course and outcome of recovery processes, particularly in the brain [35, 36]. In males the plasma concentrations of progesterone and estrogens increase, but androgens levels decrease. In females, along with an increase in progesterone and estrone content, androgen levels also are increasing.

Paracrine vasoactive substances and arachidon-ic acid metabolites also have pathophysiological significance. Endothelin, PGF2a and PGA2 have a vasoconstrictor effect, but bradykinin, histamine, PGI2 and PGE2 are vasodilators. These effects in blood loss and hemorrhagic shock may have both protective and adverse effects on organ blood flow and metabolism [34, 37].

Oxygen transport and metabolic alterations in acute blood loss

The main parameters of oxygen transport in the body are the delivery (DO2), consumption (VO2) and oxygen extraction ratio (02ER) [20, 21, 38]. Table 1 shows the simplified formulas for calculating these parameters and their normal values for humans at rest. The formulas for calculating the oxygen content in the arterial and venous blood (CaO2 h CvO2) do not include the oxygen physically dissolved in blood plasma due to an insignificant contribution of this fraction (about 3 ml/L) to the total oxygen capacity of blood. In normal aerobic conditions, the systemic VO2 is determined by the rate of metabolic processes (tissues demand for oxygen and nutrients) and is varied within the average levels 200—250 ml/min at rest. In this case DO2 exceeds VO2 4-fold, and during the passage of arterial blood through the microvasculature only 20—30% of oxygen volume is extracted. These «reserves» of oxygen transport are important for the development of compensatory reactions of the organism in severe blood loss and other critical states, and significantly contribute to the transport of CO2 (by bounding to hemoglobin) from the tissues to the lungs.

Катехоламины в этих условиях оказывают комплексное гемодинамическое и метаболическое действие. Повышается периферическое сосудистое сопротивление, СВ и АД. Метаболические эффекты включают в себя стимуляцию секреции ренина, гипергликемию (главным образом как результат ускорения гликогено-лиза в печени) и ускорение липолиза. Гипергликемия на ранних стадиях острой крово-потери обусловлена также снижением секреции инсулина в-клетками и повышением секреции глюкагона а-клетками поджелудочной железы. При декомпенсации геморрагического шока и истощении энергетических ресурсов развивается гипогликемия. Кровопотеря и травма стимулируют выработку адренокортикотропного гормона (АКТГ) в гипофизе, который, в свою очередь, стимулирует выработку глюкокортикоидов (корти-зол) в коре надпочечников. Основными эффектами глюкокортикоидов при этом являются усиление липолиза, глюконеогенеза в печени и гликогенолиза в мышцах, стабилизация лизосо-мальных мембран, уменьшение проницаемости капилляров и иммуносупрессия. В результате описанных гормональных перестроек и усиления катаболизма, в крови повышается также концентрация аминокислот, жирных кислот, и увеличивается осмолярность плазмы.

При геморрагическом шоке и других терминальных состояниях изменяется концентрация и соотношение репродуктивных стероидных гормонов в плазме крови. При этом отмечаются половые различия в динамике этих показателей в постреанимационном периоде, что может оказать влияние на течение и исход восстановительных процессов, прежде всего, в мозге [35, 36]. Так, у самцов повышается плазменная концентрация прогестерона, эстрогенов и снижается уровень ан-дрогенов, в то время как у самок наряду с увеличением содержания прогестерона и эстрона, повышается также уровень андрогенов.

Maintaining the systemic VO2 under these conditions is achieved through the redistribution of blood flow to organs and tissues with higher metabolic demands, ie, through the centralization of circulation [39]. The other main mechanism for maintaining aerobic metabolism under conditions of reduced oxygen delivery is the increase in oxygen extraction by tissue from the inflowing blood. This is manifested by decreasing CvO2, increasing the arteriovenous oxygen difference and O2ER (Table 1). Decreased affinity of hemoglobin for oxygen under the conditions of metabolic acido-sis plays an important role in this process [23].

The restoration of circulating plasma volume through autohemodilution and fluid resuscitation leads to lowering the values of Hb and Ht (anemia), and hence reduces the oxygen capacity of the blood. Under these conditions, a decrease in the DO2 is due to hypoxia of both hemic and circulatory types. The normovolemic anemia is characterized by an increase in CO, a decrease in peripheral vascular resistance and a higher oxygen tension in the mixed venous blood compared with the values of these parameters during hypovolemia and hypoxemia [40].

At any stage of acute blood loss DO2 level may be reduced to values at which the oxygen demand of cells is not enough, and VO2 also begin to decline. Such DO2 values are called «critical level of oxygen delivery» (DO2 crit) [20], below which an «oxygen debt» appears, ie metabolism from aerobic turns to anaerobic. Experimental animal studies have shown that the DO2 crit is practically independent of the factor causing a decrease in DO2, whether it is hypo-volemia, anemia or hypoxemia [41, 42]. DO2 crit was about 10 ml/min/kg, while the blood loss volume reached 50% of the CBV, normovolemic anemia reached values of Ht <8% and Hb 4g/dL, and O2ER increased to 0.8. When reaching these values, a sharp decrease in VO2 appears and the animal is dying soon after. R. B. Weiskopf et al. [43] have shown that in healthy volunteers Hb was decreased to 5 g/dL at the conditions of normovolemic hemodilution without disturbing the tissue oxygenation: VO2 and blood lactate levels did not change. The above data were obtained in animal studies and apparently healthy patients and reflected the large functional reserves of the organism in the course of blood loss compensation. However, extrapolation of these data to critically ill patients might not quite appropriate.

Persisting mixed type hypoxia in acute blood loss and hemorrhagic shock is accompanied by metabolic alterations and cell death, which are manifested by organ dysfunction and homeostasis disorder

Так, эндотелин, простагландин (Pg)F2a и PgА2 оказывают сосудосуживающее действие, а брадикинин, гистамин, PgI2 и PgE2 — вазодилата-торы, что в условиях кровопотери и геморрагического шока может оказывать как защитное так и неблагоприятное действие на органный кровоток и метаболизм [34, 37].

Транспорт кислорода и метаболические нарушения при острой кровопотере

Основными показателями транспорта кислорода в организме являются доставка (Э02), потребление (Ю2) и коэффициент экстракции кислорода (О2ЕК) [20, 21, 38]. В таблице приведены упрощенные формулы расчета этих показателей и их нормальные значения для человека в покое. В формулах расчета содержания кислорода в артериальной и венозной крови (Са02 и су02) не учитывается кислород, физически растворенный в плазме крови, ввиду незначительного вклада данной фракции (около 3 мл/л) в общую кислородную емкость крови. В нормальных аэробных условиях общее V02 определяется скоростью метаболических процессов (потребностью тканей в кислороде и ну-триентах) и в среднем составляет 200—250 мл/мин в покое. При этом Э02 превышает V02 в 4 раза, а при прохождении артериальной крови через мик-роциркуляторное русло экстрагируется только 20—30% содержащегося в ней кислорода. Эти «резервы» транспорта кислорода имеют важное значения для развития компенсаторно-приспособительных реакций организма при тяжелой кровопотере и других критических состояниях, а также необходимы для транспорта СО2 (связанного с гемоглобином) от тканей к легким.

На начальных стадиях тяжелой острой кровопотери Э02 снижается преимущественно за счет гиповолемии и снижения сердечного выброса. V02 при этом может оставаться на прежнем уровне или даже увеличиваться при усилении метаболичес-

After declining of oxygen delivery to tissues below DO2 crit, ATP synthesis in the cells is carried out by both aerobic (oxidative phosphorylation) and anaerobic sources (glycolysis). During shock decompensation these combined energy sources are not enough to provide basic metabolic needs of the cells, which leads to cell death [21]. Membrane ion pumps cease to function that leads to a «rearrangement» of the ions (the phenomenon of transmineralization). K+ leaves the cell, but Na+ and Ca2+ with the water enter into the cell [3]. Cell swelling and membrane defects (breaks) occur. In this regard, the fluid resuscitation in acute blood loss (ionic compositions, osmolality and volume of solutions) should be carried out taking into account the type of arising dyshydration (intracellular, extracellular and mixed hypo- or hyperhydration). Of particular importance is the correction of potassium homeostasis, as it is the main intracellular ion. The other factors leading to irreversible cell damage during hypoxia, are the depletion of energy resources, acidosis, free radicals, the cascade of enzymatic reactions (autolysis), mito-chondrial damage and loss of nucleotides from the cells [3, 21].

Cells of various organs differ in the degree of resistance to hypoxia. Some cells die by apoptosis mechanism, «sacrificing oneself» in resource-limited settings. Another important mechanism of cellular adaptation is hibernation. The process of filtration in the kidney cortex ceases before reaching hypoxia level, in which necrosis develops. The intestinal peristalsis ceases as well [2]. The skin and muscles, in which the blood flow is significantly reduced during the centralization of circulation, relatively resistant to hypoxia and can tolerate it for several hours. On the other hand, in the myocardium and particularly in the brain, irreversible damage occurs within a few minutes of severe hypoperfusion [44]. During acute hemorrhage gastrointestinal cells switch to anaerobic metabolism (regional VO2 starts to decrease) prior to the decline in systemic VO2 [42]. The region-

Важную роль в этом процессе играет снижение сродства гемоглобина к кислороду в условиях метаболического ацидоза [23].

По мере восстановления объема циркулирующей плазмы за счет аутогемодиллюции, приема жидкости внутрь и инфузионной терапии снижаются показатели НЬ и Ш (анемия), а, следовательно, снижается и кислородная емкость крови. В таких условиях снижение Э02 в значительной степени обусловлено уже гипоксией гемического, а не только циркуляторного типа. Для нормово-лемической анемии характерны увеличение сердечного выброса, снижение периферического сосудистого сопротивления и более высокое напряжение кислорода в смешанной венозной крови по сравнению со значениями этих параметров при гиповолемии и гипоксемии [40].

На любом из этапов острой кровопотери уровень Э02 может снизиться до таких значений, при которых потребности клеток в кислороде уже не будут удовлетворяться, при этом V02 также начнет снижаться. Такое значение Э02 называют «критическим уровнем доставки кислорода» (Э02 егй) [20], ниже которого возникает «кислородная задолженность», т.е. метаболизм из аэробного превращается в анаэробный. Экспериментальные исследования на лабораторных животных показали, что Э02 егй практически не зависит от фактора, вызывающего снижение Э02, будь то гиповолемия, анемия или гипоксемия [41, 42]. Так, Э02 егй составил около 10 мл/мин/кг, при этом объем кровопотери доходил до 50% от ОЦК, нормоволемическая анемия достигала значений Ш<8% и НЬ 4 г/дл, а О2ЕК увеличивался до 0,8. При достижении этих значений происходило резкое снижение V02 и вскоре наступал летальный исход. Исследования Weiskopf КБ с со-авт. [43] показали, что у здоровых добровольцев снижение НЬ до 5 г/дл в условиях нормоволеми-ческой гемодиллюции не приводило к нарушению оксигенации тканей: V02 и уровень лактата в крови не изменялись. Приведенные данные получены в исследованиях на животных или практически здоровых пациентах и отражают большие функциональные резервы организма в процессе компенсации кровопотери, однако, их экстрапо-

al increase in lactate level and the acidosis of mucous membranes occur even earlier [45], which is the base of the digestive tract dysfunction and translocation of intestinal microflora in any kind of shock. An important role in the development of endogenous intoxication in the hemorrhagic hypotension plays increasing the concentration of low/medium molecular weight substances, as well as oligopeptides in the portal vein system, particularly in the small and large intestines [46].

Hyperlactataemia and lactic acidosis are the main laboratory signs of anaerobic metabolism and inadequate tissue perfusion in shock [3]. In the experimental study, Torres Filho IP et al. [47] investigated the early functional and metabolic changes during acute severe blood loss (40% of the CBV). The severity of these changes in the initial stages of blood loss was associated with the mortality in laboratory animals, i.e. studied parameters possessed a prognostic value. In the group of non-survivors during the experiment, lower values of BP, minute ventilation, CaO2, CvO2, pH and bicarbonate were as significant as hypoglycemia was. At the same time, base deficit, raised serum concentration of K+ and the level of arterial blood glucose were greater in non-survivors. Lactate level in arterial blood was significantly greater in non-survivors only at later stages of blood loss.

Hyperenzymemia is a marker of massive cellular damage in hemorrhagic shock and enhances during reperfusion of ischemic tissues [34]. Among the enzymes, the content and activity of which increases, the lactate dehydrogenase (LDH), creatine phos-phokinase (CK), CK-MB and transaminases have the greatest diagnostic value. The isoforms of cardiac troponins are also important as biomarkers of myocardial damage.

In hemorrhagic shock, as well as in other types of shock, the processes of free radical oxidation are activated. The concentration of reactive oxygen species in blood and tissues increases, antioxidant system resources are depleted, lipid peroxidation and cell damage is enhanced, the activity of enzymes changes [25, 34]. With the progression of the shock, as well as during reperfusion syndrome, free radical alterations are most significant in cardiovascular system [48] (myocardial and endothelial dysfunction, microcirculatory alterations) and in blood cells itself (leukocyte activation, the «aging» of red blood cells). These primary and secondary alterations, in turn, become one of the major factors of inflammatory response and organ dysfunction development.

An experimental study by Y. P. Orlov et al. [49] has demonstrated the important role of iron metabolism disorders in the pathogenesis of severe bone injuries accompanied bya blood loss. The concentrations of serum iron and transferrin in blood decreased, but, on the contrary, ferritin content

Персистирующая гипоксия смешанного типа при острой кровопотере и геморрагическом шоке сопровождается нарушениями клеточного метаболизма и гибелью клеток, что проявляется дисфункцией органов и нарушением гомеостаза (отклонением лабораторных показателей от нормальных значений). После снижения доставки кислорода к тканям ниже Э02 cгit синтез АТФ в клетках осуществляется как за счет аэробных (окислительное фосфорилирование), так и анаэробных источников (гликолиз). При декомпенсации шока этих объединенных источников энергии уже не хватает для обеспечения базовых метаболических потребностей клеток, что и приводит к их гибели [21]. Прекращают функционировать мембранные ионные насосы, что ведет к «перегруппировке» ионов (феномен трансминерализации): К+ покидает клетку, а №+ и Са2+ вместе с водой устремляются внутрь клетки [3], возникает ее отек и дефекты (разрывы) мембраны. В связи с этим, инфузионно-трансфузионная терапия при острой кровопотере (ионный состав, осмолярность и объем растворов) должна проводиться с учетом вида возникающей дисгидрии (внутриклеточная, внеклеточная и смешанная ги-по- или гипергидратация), при этом особое значение приобретает коррекция калиевого гомеостаза, как основного внутриклеточного иона. Другими факторами, приводящими к необратимому повреждению клеток при гипоксии, являются истощение энергетических ресурсов, ацидоз, свободные радикалы, каскад ферментативных процессов (аутолиз), повреждение митохондрий и выход нуклеотидов из клетки [3, 21].

Клетки разных органов различаются по степени устойчивости к гипоксии. Часть клеток гибнет по механизму апоптоза, «принося себя в жертву» в условиях ограниченных ресурсов. Другим важным механизмом клеточной адаптации является гибернация. Так, процессы фильтрации в корковом слое почки прекращаются до достижения уровня гипоксии, при котором развивается некроз; также останавливается перистальтика кишечника [2]. Кожа и мышцы, в которых при централизации кровообращения кровоток значительно снижен, относительно устойчивы к гипоксии и могут переносить ее в течении нескольких часов. С другой стороны, в миокарде и, особенно, в мозге необратимые повреждения возникают уже через несколько минут выраженной гипоперфузии [44]. При острой кровопотере клетки ЖКТ переключаются на анаэробный метаболизм (начинает снижаться регионарное ^Э2) до начала снижения системного V02 [42], а регионарная гиперлактате-мия и ацидоз слизистых оболочек возникают еще раньше [45], что лежит в основе дисфункции

increased. At the same time, total iron content (Fe2+) in the blood increased. Such changes lead to a potentiation of free radical oxidation, blood rheology alterations and an increase in the concentration of low/medium molecular weight substances in the blood of portal vein. This, in turn, leads to the progression of endothelial dysfunction and microcircu-latory disorders.

The damaging effect of acute blood loss may affect the cellular genome including blood cells [50]. Studies by V. L. Kozhura et al. [51] using the micronucleus test had shown that the acute blood loss followed by blood reinfusion caused the destabi-lization of the genome of polychromatic immature red blood cells in murine bone marrow. This destabi-lization was maximally expressed at 24 hours from the start of hemorrhage and after reinfusion. Similar damage to the genetic material of cells occured following ionizing radiation that was mainly due to activation of free radical processes.

Decompensation mechanisms in acute blood loss

The mechanisms of acute blood loss compensation are aimed at restoring the impaired homeostasis. However, during the massive, ongoing or untreated blood loss the adaptive reactions are not able to compensate the occurred alterations and decompensation starts. The metabolic manifestations of decom-pensated hemorrhagic shock are described above in the article. Formation of abnormal positive feedbacks in the regulation of body functions stands in the center of shock progression and development of so-called vicious circles of pathogenesis of shock. This is when the deviation of homeostasis parameters via the intermediate chain of events leads to further homeostasis alterations.

Reducing CO and persistent arterial hypotension in the later stages of hemorrhagic shock in combination with tachycardia lead to a decrease in a coronary blood flow. Myocardial hypoperfusion in combination with the increased metabolic needs (sympathoadrenal system activation, increased afterload) as well as acidosis and other metabolic disturbances cause decreasing myocardial contractility and the development of acute heart failure. The latter in turn leads to a further reduction of CO and BP, i.e. a vicious circle closes. Therefore, the acute heart failure is the direct cause of death in hemorrhagic shock in most cases [11, 52]. Not less dramatical events develop at the microcirculatory level. At the stage of acute blood loss compensation, the increased sympathetic outflow leads to the contraction of both arterioles and venules. The resistance of these vascular bed sections increases. At the same time, arterio-lar resistance increases to a greater extent than venu-lar one, which leads to reducing hydrostatic pressure

Гиперлактатемия и лактат-ацидоз являются основными лабораторными признаками анаэробного метаболизма и неадекватной перфузии тканей при шоке [3]. В экспериментальной работе Torres Filho IP с соавт. [47] исследовались ранние функциональные и метаболические сдвиги при острой тяжелой кровопотере (40% от ОЦК), выраженность которых на начальных стадиях кро-вопотери ассоциировалась с летальностью лабораторных животных, т.е. имела прогностическое значение. Так, в группе не выживших в ходе эксперимента животных отмечались меньшие значения АД, минутной вентиляции легких, CaO2 и CvO2, рН, бикарбоната, гипогликемия. В то же время дефицит оснований (BE), сывороточная концентрация K+ и уровень экстракции глюкозы из артериальной крови в группе не выживших были больше. Уровень лактата в артериальной крови был существенно больше у не выживших животных только на более поздних стадиях кро-вопотери.

Гиперферментемия является маркером массивного клеточного повреждения при геморрагическом шоке и усиливается при реперфузии ише-мизированных тканей [34]. Среди ферментов, содержание и активность которых увеличивается, наибольшее диагностическое значение имеют лактатдегидрогеназа (ЛДГ), креатинфосфокина-за (КФК), КФК-МВ и трансаминазы, а также изоформы сердечных тропонинов как маркеры повреждения миокарда.

При геморрагическом шоке, как и при других видах шока, активируются процессы свободно-радикального окисления: повышается содержание в крови и тканях активных форм кислорода, истощаются ресурсы антиоксидантных систем, усиливаются процессы перекисного окисления липи-дов, повреждение клеточных структур и изменяется активность ферментов [25, 34]. При прогрессировании шока, а также при реперфузи-онном синдроме свободно-радикальные повреждения наиболее выражены в сердечно-сосудистой системе [48] (дисфункция миокарда и эндотелия, микроциркуляторные нарушения) и в клетках самой крови (активация лейкоцитов, «старение» эритроцитов). Эти первичные и вторичные повреждения, в свою очередь, являются одними из ведущих факторов развития воспалительной реакции и дисфункции органов.

В экспериментальном исследовании Орлова Ю. П. с соавт. [49] показана важная роль нару-

in the capillaries and contributes to entering interstitial fluid into the bloodstream. However, the metabolites with vasodilating properties accumulate in tissues during the progression of hypotension and shock. Precapillary vessels (arterioles) are more sensitive to these metabolic impacts compared to the venules and therefore dilate first. This leads to increased capillary pressure and fluid displacement already from the vascular bed into tissues [53]. The result of the described process is increased concentration and stasis of blood within the microvascula-ture. The CBV and CO decrease progressively.

Hemostasis activation occurs from the first minutes of tissue damage and the occurrence of bleeding. In the later stages of blood loss and hemor-rhagic shock alterations in hemostasis and blood microrheology play a major role in the circulatory decompensation, primarily at the microcirculatory level. Coagulopathy in acute massive blood loss is manifested by the successive hypercoagulation syndrome, hyper- and hypocoagulation phases of acute DIC [22]. The hypercoagulation syndrome represents an increased likelihood of blood clotting in the absence of thrombosis. It may represent a consequence of a trauma and/or blood loss (eg, elderly patients with atherosclerosis), and might even be more aggravated by vascular wall injury, activation of clotting factors and tissue thromboplastin entering the bloodstream. However, due to the simultaneous activation of anticoagulation and fibrinolytic systems, thrombotic process is limited to the damaged portion of vessel. Severe hypotension, slowing of blood flow in the microvasculature (stasis) and ongoing bleeding contribute to the development of the hypercoagulation phase of DIC. This phase is characterized by multiple microthrombi formation mainly in the capillaries and venules, as well as by the consumption of plasma coagulation factors, platelets and the components of fibrinolytic system. In the absence of adequate treatment the hypercoag-ulation phase of DIC contributes greatly to the development of multiple organ failure and is quickly replaced by the next phase. The hypocoagulation phase is manifested by the laboratory evidence of hypocoagulation and clotting factors consumption, elevated levels of fibrin degradation products and diffuse bleeding (haematomas and petechiae) [22].

Disturbances of blood rheology in hemorrhagic and other types of shock are caused not only by an increase in blood viscosity due to its concentration or slowing of blood flow, but also by the structural and functional changes of red blood cells. These changes determine microvascular blood flow and «responsi-ble» for the development of microrheological alterations [54]. The reduced deformability and increased aggregation of red blood cells are the main microrheo-logical alterations. Повреждающее действие острой кровопоте-ри может распространяться и на клеточный геном, в т.ч. клеток самой крови [50]. В исследованиях Кожура В. Л. с соавт. [51] с использованием микроядерного теста показано, что острая крово-потеря с последующей реинфузией крови вызывают дестабилизацию генома полихроматофиль-ных эритроцитов, максимально выраженную через 24 ч от начала кровопотери и после реинфу-зии. Аналогичные повреждения генетического материала клетки наступают при действии ионизирующей радиации и в значительной степени определяются активацией свободнорадикальных процессов.

Механизмы декомпенсации при острой кровопотере

Механизмы компенсации острой кровопоте-ри направлены на восстановление нарушенного гомеостаза. Однако, при массивной, продолжающейся или не вовремя восполненной кровопотере приспособительные реакции не способны компенсировать возникшие нарушения и наступает декомпенсация. Метаболические проявления де-компенсированного геморрагического шока описаны выше. В основе прогрессирования шока и развития так называемых порочных кругов патогенеза лежит формирование патологических положительных обратных связей в регуляции функций организма, когда отклонение параметров гомеостаза через цепь промежуточных событий приводит к еще большему нарушению гомеостаза. Так, снижение СВ и персистирующая артериальная гипотензия на поздних стадиях геморрагического шока в сочетании с тахикардией приводят к снижению коронарного кровотока. Гипоперфузия миокарда в сочетании с его возросшими метаболическими потребностями (активация симпатоадре-наловой системы, увеличение постнагрузки), а также ацидоз и другие метаболические нарушения вызывают снижение сократимости миокарда и развитие острой сердечной недостаточности. Это, в свою очередь, ведет к дальнейшему снижению СВ и АД — порочный круг замыкается. Поэтому

Such pathologic changes, exacerbated by aniso- and poikilocytosis, lead to the formation of cell aggregates («rouleaux»), the impossibility of passing the rigid red blood cells through the capillaries, hemolysis and further progression of microcir-culatory disorders and hypoxia [56]. Changing structural and functional properties of red blood cells in blood loss and reinfusion is largely due to changes in surface nanostructure of erythrocyte membrane, revealed by atomic force microscopy [30, 57]. The nanostructure of red blood cells membrane surface (the «heights» of a different order) have a characteristic temporal dynamics, which also depends on the volume of blood loss. It should be noted that the perfluo-rocarbon emulsion ( «Perftoran») has a strong corrective action to the nanostructure of erythrocyte membranes, thereby improving the flow properties of blood [58]. Moreover, in an experimental study [59], it has been shown that in vitro the nanoparticles of perftoran significantly accelerate the reduction of methemoglobin to oxyhemoglobin (inside the red blood cells), i.e. facilitate the transition of iron from the trivalent state to the divalent (Fe3+^Fe2+). This may additionally improve the oxygen transport function of red blood cells during hypoxia and long-term storage of donated blood.

The development of multiple organ failure in hemorrhagic shock is largely due to the systemic inflammatory response of the body to emerging ischemic and reperfusion cell damage [2, 60]. The initial hypoxic damage of cells contributes to entering into the bloodstream of large amounts of toxic metabolites (lactate, free radicals and other), which are potent immune modulators, and induce the initial activation of neutrophils and other immune cells. In turn, this triggers a cascade of cellular signaling and response (amplified response) with intensive release of both pro-inflammatory and anti-inflammatory cytokines and other mediators. Another vicious circle is formed, since activated neutrophils and «cytokine storm» contribute to the further progression of microcirculatory disorders, cell damage of other organs and the development of their dysfunction.

It should be noted that despite the complexity and severity of microcirculatory alterations in hem-orrhagic shock, they are largely reversible, and respond better then in septic shock to intensive treatment aimed to stabilize the systemic hemody-namics and oxygen transport [5, 61].

Conclusion

Acute blood loss is a polyethiological pathological process. The major pathogenic factors are hypo-volemia and anemia, which lead to mixed type hypoxia and severe deterioration of cellular metabolism. The homeostasis of the organism as a whole is disrupted.

Не менее драматично события развиваются и на уровне микроциркуляции. На стадии компенсации острой кровопотери, повышение симпатической импульсации приводит к сужению как арте-риол, так и венул, и увеличению сопротивления в этих участках сосудистого русла. При этом сопротивление артериол увеличивается в большей степени, чем венул, что приводит к снижению гидростатического давления в капиллярах и способствует поступлению интерстициальной жидкости в сосудистое русло. Однако, при про-грессировании гипотензии и шока в тканях накапливаются метаболиты, обладающие сосудорасширяющим действием. Прекапиллярные сосуды (артериолы) более чувствительны к таким метаболическим воздействиям по сравнению с венула-ми и расширяются первыми. Это приводит к повышению капиллярного давления и перемещению жидкости уже из сосудистого русла в ткани [53]. Результатом описанного процесса является сгущение крови, ее застой в микроциркуляторном русле, еще большее снижение ОЦК и СВ.

Как уже упоминалось выше, активация системы гемостаза происходит с первых минут повреждения тканей и возникновения кровотечения. На более поздних стадиях кровопотери и при геморрагическом шоке нарушения гемостаза и микрореологии крови играют одну из основных ролей в декомпенсации кровообращения, прежде всего на уровне микроциркуляции. Коагулопатия при острой массивной кровопотере проявляется последовательно сменяющими друг друга гипер-коагуляционным синдромом, гипер- и гипокоагу-ляционными фазами острого ДВС-синдрома [22]. Гиперкоагуляционный синдром — повышенная «готовность» крови к свертыванию при отсутствии тромбозов. Он может быть исходным фоновым состоянием организма, пострадавшего от травмы и кровопотери (например, у пожилого пациента с атеросклерозом) и в еще большей степени усиливаться при повреждении сосудистой стенки, активации факторов свертывания и попадании в кровоток тканевого тромбопластина. Однако, вследствие одновременной активации про-тивосвертывающей и фибринолитической систем, процесс тромбообразования ограничен поврежденным участком сосуда. Выраженная артериальная гипотензия, замедление кровотока в микрососудах (стаз) и продолжающееся кровотечение способствуют развитию гиперкоагуляционной фазы ДВС-синдрома, которая характеризуется образованием множественных микротромбов, прежде всего в капиллярах и венулах, а также признаками потребления плазменных факторов свертывания крови, тромбоцитов, компонентов

Direct exchange of substances between blood and interstitial fluid is carried out at the level of microcirculation. Changes in vascular tone and microcirculatory alterations are involved in compensatory reactions of the organism to acute blood loss, and in the subsequent processes of decompensation.

The main compensatory reactions in acute blood loss are activation of blood coagulation and the sympathetic nervous system, stimulation of heart activity, centralization of circulation, autohemodilu-tion, protein synthesis and hemopoiesis activation.

Alterations in macrohemodynamics (hypov-olemia, decreased myocardial contractility and CO, hypotension) and microcirculation (reduction in the density of perfused capillaries, slowing of blood flow, concentration and stasis of blood, paresis of microvessels, DIC and others) progress during acute blood loss decompensation and development of hypovolemic shock.

Decreased oxygen delivery to the tissues at below critical level leads to cell metabolism switching to the anaerobic pathway, depletion of energy and the plastic resources of cells, acidosis, lipid per-oxidation, transmineralization and cell membrane damage. The result of acute blood loss decompensation is cell death by necrosis or apoptosis followed by the emergence of multiple organ failure.

Нарушения реологии крови при геморрагическом и других видах шока обусловлены не только повышением вязкости крови вследствие ее сгущения или замедления кровотока, но и структурно-функциональными изменениями эритроцитов, которые определяют текучесть крови на уровне микроциркуляции и «ответственны» за развитие микрореологических нарушений [54]. Основными микрореологичекими нарушениями являются снижение деформируемости и повышенная агрегация эритроцитов, причем степень изменения этих параметров пропорциональна объему и тяжести кровопотери [55]. Такие патоло-

гические изменения, усугубленные анизо- и пой-килоцитозом, приводят к образованию клеточных агрегатов («монетных столбиков»), невозможности прохождения ригидных эритроцитов через капилляры, гемолизу и дальнейшему прогрессиро-ванию микроциркуляторных нарушений и гипоксии [56]. Указанные изменения структурно-функциональных свойств эритроцитов при крово-потере и реинфузии во многом обусловлены изменениями наноструктуры поверхности их мембран, выявленными методом атомной силовой микроскопии [30, 57]. Параметры наноструктуры эритроцитов (высоты различного порядка на поверхности мембраны) имеют характерную динамику во времени, которая также зависит от объема кро-вопотери. Здесь следует отметить, что перфторуг-леродная эмульсия («Перфторан») оказывает выраженное корректирующее действие на наноструктуру мембран эритроцитов, тем самым улучшая реологические свойства крови [58]. Более того, в эксперименте [59] было показано, что in vitro наночастицы «Перфторана» значительно ускоряют восстановление метгемоглобина в оксиге-моглобин (внутри эритроцитов), т.е. способствуют переходу железа из трехвалентного состояния в двухвалентное (Fe3+^Fe2+), что дополнительно может улучшать кислородтранспортную функцию эритроцитов при гипоксии и длительном хранении донорской крови.

Развитие полиорганной недостаточности при геморрагическом шоке во многом обусловлено неадекватными иммунными и воспалительными реакциями организма на возникающие ише-мические и реперфузионные повреждения клеток [2, 60]. Первичное гипоксическое повреждение клеток способствует поступлению в кровоток большого количества токсических метаболитов (лактат, свободные радикалы и др.), которые являются сильными иммуномодуляторами и вызывают первичную активацию нейтрофилов и других клеток иммунной системы. В свою очередь, это запускает каскад клеточных взаимодействий с вовлечением все большего количества иммунных клеток (англ. amplified response) и интенсивным высвобождением как провоспалительных, так и противовоспалительных медиаторов и других ци-токинов. Формируется еще один порочный круг, поскольку активированные нейтрофилы и «цито-киновая буря» способствуют дальнейшему про-грессированию нарушений микроциркуляции, повреждению клеток первично не ишемизирован-ных органов и формированию их дисфункции.

Следует отметить, что несмотря на всю сложность и тяжесть описанных нарушений микроциркуляции при геморрагическом шоке, они все же в большей степени обратимы и лучше реагируют на интенсивную терапию, направленную на стабилизацию системной гемодинамики и транспорта кислорода, по сравнению с гиповоле-мическим шоком при сепсисе [5, 61].

Заключение

Острая кровопотеря — полиэтиологический патологический процесс, основными патогенетическими факторами которого являются гиповолемия и анемия, приводящие к гипоксии смешанного типа и тяжелым нарушениям клеточного метаболизма и гомеостаза организма в целом. На уровне мик-роциркуляторного русла осуществляется непосредственный обмен веществ между кровью и межклеточной жидкостью, а изменения сосудистого тонуса и микроциркуляции задействованы как в компенсаторно-приспособительных реакциях организма на острую кровопотерю, так и в процессах последующей декомпенсации. Основными компенсаторно-приспособительными реакциями являются активизация свертывающей системы крови и симпато-адреналовой системы, стимуляция работы сердца, централизация кровообращения, аутогемо-дилюция, активизация синтеза белка и гемопоэза. При декомпенсации острой кровопотери и развитии гиповолемического шока прогрессируют нарушения системной гемодинамики (гиповолемия, снижение сократимости миокарда и СВ, артериальная гипотензия) и микроциркуляции (снижение плотности перфузируемых капилляров, замедление кровотока, сгущение и стаз крови, парез микрососудов, ДВС-синдром и др.). Снижение доставки кислорода к тканям ниже критического уровня приводит к переключению клеточного метаболизма на анаэробный путь, истощению энергетических и пластических ресурсов клеток, развитию ацидоза, перекисного окисления липидов, трансминерализации и повреждению клеточных мембран. Итогом декомпенсации острой кровопотери является гибель клеток по механизму некроза или апоптоза с развитием полиорганной недостаточности.
