ТREATMENT OF ARTERIAL HYPERTENSION IN POSTMENOPAUSAL WOMEN CONSIDERING THE TIME ORGANIZATION OF CARDIOVASCULAR SYSTEM

The study was conducted in 70 women aged 50—62 years with Stage I — II degree. Patients were randomly divided into 2 groups: 30 people receiving moexipril by conventional methods (the drug was administered in the morning, the initial dose of 7.5 mg) and 40 men whose treatment was carried out taking into account the provisions of akrofazy AD — preventive chronotherapy. In order to customize treatment options identified three typical temporal organization of the cardiovascular system: morning, evening chronotype and independent. The number of significant rhythms in patients who were on hronoterapevticheskom treatment increased by 20.7%, and the first by only 4.8%. In 20 patients recovered tsirkadiannost individual rhythms, 11 — in-phase conjugate indices, disappeared with the rhythms of small amplitude, a 23% reduction in the number of persons with DS, doubling the number of matches akrofaz studied rhythms with chronotype surveyed. Installed a high efficiency of chronothe-rapymoexipril carried out taking into account Biorhythmological approach that allows us to recommend a preferred drug for the treatment method of chronotherapy in this category of patients.

ДИНАМИКА ВЫЗОВОВ СКОРОЙ ПОМОЩИ МОСКВЫ ЗА ПОСЛЕДНИЕ 5 ЛЕТ

Приведены данные о динамике вызовов скорой помощи (ВСП) Москвы за период с апреля 2006 по ноябрь 2011 г. В этот интервал времени входит аномально жаркое лето 2010 г., влиянию которого на медицинские показатели уделено особое внимание. Для многих заболеваний (но не для всех) характерен сезонный ритм ВСП, который обусловлен природными — климатическими — особенностями. Вызовы скорой помощи для ряда заболеваний, выявляемых не сразу, а по истечении некоторого времени (туберкулез, злокачественные новообразования), сезонного ритма не имеют. Число вызовов по случаям обострения гипертонической болезни заметно возрастает после встречи Нового года во время каникул и держится более продолжительное время, чем в годы, когда каникулы еще не были

введены. Для других заболеваний эффект или отсутствует, или не столь ярко выражен. Недельный ритм вызовов скорой помощи отмечен во всех обработанных нами временных рядах, в том числе и для вызовов по не сразу выявляемым случаям туберкулеза и злокачественных новообразований. По нашему мнению, это свидетельствует о социальном генезисе этого ритма. Материалы последних лет позволяют детально просмотреть влияние аномально жаркого лета 2010 г. на частоту вызовов скорой помощи, поскольку больные целым рядом заболеваний заметно реагируют на жаркие дни. Состав таких заболеваний вызывает некоторые вопросы. Например, почему динамика вызовов по поводу сердечно-сосудистых заболеваний, обострения гипертонической болезни проявляется не столь сильно, как можно было бы ожидать. Наиболее значительный рост числа вызовов отмечен для случаев заболеваний сосудов головного мозга, ангины, пневмонии, нервных болезней. Достаточно выражена специфика частоты вызовов по поводу различных заболеваний, обусловленная полом и возрастом пациентов.

This article gives certain data about the dynamics of ambulance calls (AC) in Moscow from April 2006 to November 2011. This time interval includes the anomalously hot summer of 2010, the effect of which on medical indicators drew our particular attention. The AC seasonal rhythm, conditioned by natural, i.e., climatic, features, is characteristic of many (butnot all) diseases. The AC statistics for some diseases that are revealed not right away but after some time (tuberculosis or malignant neoplasms) have no seasonal rhythm. The number of calls owing to the aggravation of hypertonic disease markedly increases during the holidays after the New Year celebration and is sustained for a longer period than in years when these holidays had not yet been introduced. For other diseases, this effect is either absent or not so explicit. The AC weekly rhythm is marked in all the time series processed, including the calls owing to not immediately revealed tuberculosis and malignant neoplasms. In our opinion, this evidences the social genesis of this rhythm. The latest materials allow us to track in detail the effect of the anomalously hot summer of 2010 on the AC frequency, since patients with many diseases markedly respond to hot days. The variety of such diseases raises some questions. For example, why is the dynamics of calls concerning cardiovascular diseases or aggravations of hypertonic diseases manifested not as intensively as might be expected. The most significant growth in the AC number is marked for the cases of cerebral vascular disorders, hypotonic, tonsillitis, pneumonia, and nervous diseases. The gender and age related specificity is quite sufficiently expressed in the AC rate concerning various diseases.
